Estranged bodies, simulated harmony, and misplaced cultures: neurasthenia in contemporary Chinese society.

OBJECTIVE: To study the sociocultural transformation of neurasthenia (shenjing shuairuo, SJSR), as both disease and illness, in Chinese society. METHOD: This is based on a critical review of evidence drawn from the psychiatric and anthropological literature, and the use of a single case study. RESULTS: SJSR remains a ubiquitous illness in socio-politically different Chinese societies, but the Americanization of Chinese psychiatry has paradoxically made the "same" disease category languish rapidly in professional practice. Although it engages bodily modes of attention, SJSR is far from being

Body dysmorphic disorder: does it have a psychotic subtype?

BACKGROUND: Although body dysmorphic disorder (BDD) is classified in DSM-III-R as a nonpsychotic somatoform disorder, controversy exists as to whether BDD can present with psychotic features. If it can, this raises the possibility that its DSM-III-R psychotic counterpart-delusional disorder, somatic type--may not be a separate disorder. The purpose of this study was to determine whether patients with nonpsychotic BDD (defined according to DSM-III-R criteria, i.e., with maintenance of some insight) were different from patients with psychotic BDD (those whose preoccupation was without insight an

[State-sponsored doping in GDR elite sport: health consequences, reappraisal, and questions of legal compensation].

Against the backdrop of the ideological rivalry between communism and capitalism, sport in the GDR was exploited for political ends. This was evident, among other things, in a state-directed, controlled, hierarchically organized and, in parts, totalitarian competitive sports system. A key component was a conspiratorial, state-controlled program of compulsory doping, which was systematically implemented from the 1970s onwards and affected an estimated 15,000 underage athletes. Performance-enhancing substances, primarily anabolic-androgenic steroids, were administered covertly, without the knowl

The relationship between residential proximity to outdoor play spaces and children's mental and behavioral health: The importance of neighborhood socio-economic characteristics.

Urban outdoor play spaces are reported to improve children's health. However, there is little empirical evidence on the impact of outdoor play spaces on childhood mental and behavioral health. To fill this gap, we investigated the associations between residential proximity to outdoor play spaces and the prevalence of diagnosed mental and behavioral disorders. We explored whether these associations differ by individual and area-level socio-economic status (SES). This cross-sectional study included 151 110 children who were 0-12 years old in 2014 and were visited in public primary health care ce

Somatization in cancer.

Somatic symptoms can occur in disease-free cancer patients. The causes of such symptoms in cancer can be many and varied. These could be due to anxiety, depression, somatization or a manifestation of illness behaviour. Somatic symptoms can also arise out of treatments for the cancer like radiation treatment or chemotherapy. Cancer related somatic symptoms have cognitive, psychological, and physiological causes, each of which is amenable to treatment. The occurrence of somatoform disorders in cancer patients is likely to complicate the treatment and outcome of the cancer. Common somatic symptom…

Child psychiatry training for pediatricians: Japanese perspectives in infant psychiatry.

Since August 1993, child psychiatry training by a full-time child psychiatrist has become an essential component of the postgraduate training programme at the Department of Paediatrics, Keio University Hospital, Japan. Thirty trainees, 15 first-years and 15 second-years, had first-hand experience in residential child psychiatric treatment concurrently with pediatric training. With the trainees as primary care workers, 31 cases have been successfully treated, of whom seven were school refusals, 11 were anorexia nervosas and eight somatoform disorders. Consistent maternal care, such as three spo…

Saline activation of pseudoepileptic seizures: clinical EEG and neuropsychiatric observations.

Twenty patients with suspected pseudoseizures underwent saline infusion during prolonged sleep deprived EEG with video monitoring and nasopharyngeal electrodes. No patients had definite clinical evidence of epilepsy, but 35% had abnormal EEGs, 10% with epileptiform activity. Patients with epileptiform discharges did not experience pseudoseizures with saline infusion. Patients with generalized EEG slowing responded to saline infusion with characteristic seizures. Three groups of patients were differentiated. Eight patients had characteristic attacks with saline infusion, and in these patients f

The Association between General and Pathological Dissociation and Disaster-Related Psychopathology in Directly Exposed Survivors.

Objective: Dissociation and trauma have a well-documented relationship, and dissociation is assumed to result from trauma exposure. If trauma generates dissociative psychopathology, it should be observed after exposure to disaster and be associated with disaster-related psychopathology. Few studies have focused specifically on dissociation as an outcome of disaster trauma. This study examined dissociation and its association with disaster-related psychopathology in survivors of five different disasters. Methods: In the first 6 postdisaster months, directly exposed survivors (N = 216) of mass s

A telephone- and text-message based telemedical care concept for patients with mental health disorders--study protocol for a randomized, controlled study design.

BACKGROUND: As in other countries worldwide, the prevalence of mental disorders in Germany is high. Although numerically a dense network of in- and outpatient psychiatric health services exists, the availability in rural and remote regions is insufficient. In rural regions, telemedical concepts can be a chance to unburden and complement the existing healthcare system. Telemedical concepts consisting of video or telephone consulting show first positive results, but there are only a few studies with a randomized controlled design. To improve the treatment of patients with mental disorders in rural

Delusional versus nondelusional body dysmorphic disorder: clinical features and course of illness.

DSM-IV's classification of body dysmorphic disorder (BDD) is controversial. Whereas BDD is classified as a somatoform disorder, its delusional variant is classified as a psychotic disorder. However, the relationship between these BDD variants has received little investigation. In this study, we compared BDD's delusional and nondelusional variants in 191 subjects using reliable and valid measures that assessed a variety of domains. Subjects with delusional BDD were similar to those with nondelusional BDD in terms of most variables, including most demographic features, BDD characteristics, most

This article gives an updated review of the history, epidemiology, etiology, and evidence-based treatment of eating disorders (anorexia nervosa, bulimia nervosa, and binge eating disorder), with specific reference to the Norwegian situation. In order to improve clinical skills there is an emphasis on the need for more treatment resources, particularly for the most serious cases, better implementation of evidence-based treatment and on continuous medical education. Given the somatic manifestations of eating disorders, medical doctors are essential in assessment and treatment. Many patients can
