The authors present a classification of trauma to the cranial base, based on observation in 80 cases. There are five types. Type I : involves only the anterior wall of the frontal sinus. Type II : involves the face (craniofacial disjunction of the Lefort II type or crush face) and extend upward to the cranial base and, in occurency, to the anterior wall of the frontal sinus, because of the facial retrusion. Type III : ivolves frontal part of the skull and extend down to the cranial base. Type IV : is a combination of types II and III. Type V : involves only ethmoidal or sphenoidal bones. Cereb

The lateral recess of the sphenoid sinus is one of the most common sites of meningocele and spontaneous cerebrospinal fluid (CSF) leak. Despite the availability of several techniques for closure of skull base defects occurring in this location, recurrence still poses a major challenge. This report reviews the experience of surgical repair of lateral sphenoid sinus recess CSF leak at a tertiary referral center and provides a brief discussion of this rare lesion. Nine surgeries were performed for six cases of spontaneous lateral sphenoid sinus recess CSF leak (two revisions and one repair of a n

BACKGROUND: Cerebrospinal fluid (CSF) leaks in the lateral recess of the sphenoid sinus (LRSS) are typically spontaneous in nature and require surgical repair. Endoscopic endonasal approaches have become the mainstay of CSF leak repair in the LRSS, though they remain technically challenging and place the vidian nerve (VN) and sphenopalatine artery (SPA) at risk. Here we present a lateral paraorbital approach (LPOA) as a minimally invasive transcranial VN and SPA sparing alternative for LRSS CSF leak repairs.

The human sphenoid sinus is an extremely variable cavity and an important landmark in hypophyseal surgery. The aim of this study was to investigate the relationship between the sphenoid sinus type, size, extent of pneumatization and occurrence of protrusions of the adjacent neurovascular structures. A total of 51 randomly selected skulls (≥20 years of age, 33 male; 102 sinuses) were analyzed using cone beam computed tomography to estimate pneumatization extension beyond the body of the sphenoid (planum sphenoidale, pterygoid process, greater wings, clivus, dorsum sellae) and protrusions of the

OBJECTIVE: This study aimed to classify pterygoid process fractures associated with maxillary transverse fractures.
STUDY DESIGN: Pterygoid process fractures in 100 patients with maxillary transverse fractures were observed 2- and 3-dimensionally using image processing software. Fracture line course and height and sphenoid sinus involvement were recorded.
RESULTS: Pterygoid process fractures were classified as follows: class I, vertical (simple separation between medial and lateral plates); or class II, transverse (3 subcategories according to location of fracture line: II-1, within pterygoi

AIM: To measure the linear cephalometric dimensions of anterior and posterior segments of the craniofacial complex sagittally, to establish ratios between different linear dimensions of sagittal segments and check for dimensional balance among the various segments in subjects with normal occlusion, pleasing profile and facial harmony.
SETTING AND SAMPLE POPULATION: Department of Orthodontics, Saveetha University. Lateral cephalograms of 120 subjects of both sexes in the age group of 17-28 years with normal occlusion belonging to Chennai, India.
MATERIALS AND METHODS: Linear dimensions of ant

OBJECTIVE: To explore the clinical effect of sellar floor bone flap with a pedicled nasoseptal flap in endoscopic transnasal pituitary adenoma surgery for skull base reconstruction.Method: This was a retrospective clinical analysis of 30 patients with pituitary adenoma operated by the same neurosurgical team from June 2015 to June 2018. All patients were diagnosed with pituitary adenoma by pituitary magnetic resonance imaging, and the authors confirmed that the sellar floor bone was intact using sphenoid sinus computed tomography. All patients underwent an endoscopic transnasal approach, and t

BACKGROUND: Isolated synostosis of the frontosphenoidal suture is very rare and difficult to diagnose. Little has been reported on the clinical presentation and fetal development of this suture.
OBJECTIVE: To understand the development of the frontosphenoidal suture and the outcome of its synostosis.
MATERIALS AND METHODS: We studied the normal fetal development of the frontosphenoidal suture in dry human skulls and the clinical features of four patients with isolated synostosis of the frontosphenoidal suture.
RESULTS: The frontosphenoidal suture develops relatively late during the second t

The transzygomatic approach has been utilised to facilitate neurosurgical access to the skull base for a number of years. Advocates of the technique claim the additional access gained provides wider exposure of the neurosurgical pathology and improved visualisation of adjacent vital neurovascular structures. The aim of this study was to photographically demonstrate the technique and to highlight the anatomical areas to which access can be improved. A morphological cadaver study was undertaken. Specific intracranial structures in the vicinity of the skull base were targeted and exposure was com

Two cases of extradural solitary plasmacytoma (SIP) with infiltration of the dura, destruction of the skull base, multiple cranial nerve palsies and proptosis are presented. The cases were treated aggressively with surgery and radiotherapy and showed no signs of progressing to multiple myeloma after 15 months and four and a half years respectively.

The authors report seven patients with fractures of the sella turcica. Four of them developed Diabetes Insipidus as a consequence of their traumata. The importance of computed tomography as a diagnostic procedure is paramount in order to make early diagnosis of this potential life-threatening condition possible.

A 10-year-old male with history of Beals syndrome presented with hearing loss and was found to have middle and inner ear dysplasia and left temporal encephalocele on imaging. Beals syndrome is a rare autosomal dominant connective tissue disorder caused by a mutation in the fibrillin-2 gene. Skeletal manifestations of Beals have been reported, including anomalies of the long bones, calvarium, and spine. External ear abnormalities with "crumpled ear" deformity are seen in the majority of patients. This is the first case to report imaging findings of the middle and inner ear in a patient with Bea

AIM: This study chronicles skull base and face development in nonsyndromic unilateral coronal synostosis (UCS) during infancy, to characterize the mechanistic progression of facial dysmorphology.
METHODS: Computed tomography scans from 51 subjects were reviewed (26 UCS, 25 controls) and data were reconstructed. Patients were stratified into 5 age groups. A series of measurements were taken from the reconstructions.
RESULTS: All patients had a unilaterally fused coronal suture at the time of analysis. Asymmetry of the sphenoid wings was present across all age groups. The sphenoid wing ipsilat

Chondromyxoid fibroma is an unusual benign tumor of cartilaginous derivation. We describe a rare example of chondromyxoid fibroma of the frontal-sphenoid junction with orbital infiltration in a 35-year-old Hispanic woman who presented with frontal headaches. Gross total excision was performed. The excised mass was composed of neoplastic cells with chondrocytic features within a myxoid matrix. Bony infiltration was present without infiltration of dura mater or brain tissue. The lack of mitotic activity, low cell density, lack of nuclear pleomorphism, and a fused lobular architectural pattern in
