Surgical Outcomes for Drop Body Syndrome in Adult Spinal Deformity.

STUDY DESIGN: A multicenter retrospective case series of patients treated surgically for adult spinal deformity (ASD). OBJECTIVE: The aim of this study was to compare clinical outcomes between propensity score matched ASD patients with or without drop body syndrome (DBS). SUMMARY OF BACKGROUND DATA: DBS is an extreme primary sagittal-plane deformity often seen in Asian countries. Although the importance of sagittal alignment is widely recognized, surgical outcomes for deformities purely in the sagittal plane are poorly understood. METHODS: This study included 243 consecutive patients (age 6

Cluster analysis describes constellations of cardiac anomalies presenting in spinal anomaly patients.

BACKGROUND: Cardiac anomalies are prevalent in patients with bony spinal anomalies. Prior studies evaluating incidences of bony congenital anomalies of the spine are limited. The Kids' Inpatient Database (KID) yields national discharge estimates of rare pediatric conditions like congenital disorders. This study utilized cluster analysis to study patterns of concurrent vertebral anomalies, anal atresia, cardiac malformations, trachea-esophageal fistula, renal dysplasia, and limb anomalies (VACTERL anomalies) co-occurring in patients with spinal congenital anomalies. METHODS: Retrospective revi

Complication Rates and Maintenance of Correction After 3-Column Osteotomy in the Elderly: Report of 55 Patients With 2-Year Follow-up.

BACKGROUND: Three-column osteotomies (3CO) provide substantial correction for adult spinal deformity (ASD), but carry risks of complications and revisions. The risk-benefit balance of 3CO in the elderly remains unclear. OBJECTIVE: To evaluate sagittal alignment and complications after 3CO in a population over 70-yr old. METHODS: Radiographic retrospective review of consecutive patients over 70 yr ("Elderly") undergoing 3CO for ASD. Demographic, operative, and outcomes data were collected. Full-spine radiographs were analyzed at baseline and 2-yr postoperatively. Results were compared to a gr

Characterization of two ENU-induced mutations affecting mouse skeletal morphology.

Using the N-ethyl-N-nitrosourea (ENU) mutagenesis screen, we have identified two skeletal morphology mutants, Skm1 and Skm2. Positional cloning and candidate gene sequencing localized the causative point mutations within the genes coding for natriuretic peptide receptor C (NPR-C) and filamin b (FLNB), respectively. Mice that carry a mutation in Npr3 exhibit a skeletal overgrowth phenotype, resulting in an elongated body and kyphosis. Skm2 mice, carrying a mutation in Flnb, present with scoliosis and lordosis. These mutant mice will serve as useful models for the study of vertebral malformation

Reliability and validity of classification of senile postural deformity in mass examinations.

Nakada (1988) divided senile postural deformities into four types by visual observation: an extended type, an S-shaped type, a flexed type, and a hand-on-the-lap type. The purpose of this study was to investigate the inter-rater reliability and the discriminant validity of assessing the elderly spinal posture using a posture-measuring device developed by us and dividing postural deformities into the four types of Nakada's classification. Seventy-seven elderly persons (52 women and 25 men) who lived independently participated in the study. The average age of the subjects was 73 years (range, 65

Advanced Spine Imaging: Technical Updates and Practice Recommendations.

Advanced spine imaging has historically lagged behind brain and neck imaging for implementation into clinical practice. This is largely due to unique technical challenges including rectangular FOV, anatomic curvature, small cross-section, multiple tissue interfaces, physiologic motion, and surgical hardware. For common indications such as low back pain, advanced imaging may not be necessary for routine care. However, for complex problem-solving, improved anatomic and functional spine assessment can have profound impacts on patient diagnosis, interventional planning, and outcomes. Advanced tech

Spina Bifida Occulta of the Sacrum as a Contributing Factor to Progressive Scoliosis.

Spina bifida occulta (SBO) refers to a split in the spine that is concealed beneath the skin. The biomechanical impact of complete sacral SBO has often been underappreciated. This case highlights the potential effects of complete sacral SBO on the spine. A 12-year-old girl presented with truncal asymmetry and intermittent neck and lumbar pain lasting for two years. Initially, she was diagnosed with mild scoliosis during a school health screening and was placed under medical observation, but her symptoms worsened over time. Imaging studies revealed an inverted cervical lordosis along with a 25-

From genetics to epigenetics to unravel the etiology of adolescent idiopathic scoliosis.

Scoliosis is defined as the three-dimensional (3D) structural deformity of the spine with a radiological lateral Cobb angle (a measure of spinal curvature) of ≥10° that can be caused by congenital, developmental or degenerative problems. However, those cases whose etiology is still unknown, and affect healthy children and adolescents during growth, are the commonest form of spinal deformity, known as adolescent idiopathic scoliosis (AIS). In AIS management, early diagnosis and the accurate prediction of curve progression are most important because they can decrease negative long-term effects o

Neuromuscular Scoliosis: Current Concepts.

Scoliosis is a common deformity in neuromuscular disorders. This spinal deformity usually presents at an early age, rapidly progresses during growth, and continues to progress even after skeletal maturity. Neuromuscular scoliosis classically involves the entire thoracolumbar spine, often extending to the pelvis and causing pelvic obliquity. Physical examination can be challenging, but it is important to assess the flexibility of the spinal curve and associated joint contractures. Upright anteroposterior and lateral radiographs are the preferred imaging. When formulating a treatment plan, it is

Prospective multicenter assessment of perioperative and minimum 2-year postoperative complication rates associated with adult spinal deformity surgery.

OBJECTIVE Although multiple reports have documented significant benefit from surgical treatment of adult spinal deformity (ASD), these procedures can have high complication rates. Previously reported complications rates associated with ASD surgery are limited by retrospective design, single-surgeon or single-center cohorts, lack of rigorous data on complications, and/or limited follow-up. Accurate definition of complications associated with ASD surgery is important and may serve as a resource for patient counseling and efforts to improve the safety of patient care. The authors conducted a stud
