Gait kinematics during walking in children with impaired binocular vision due to amblyopia or strabismus.

Impaired binocular vision early in life from amblyopia or strabismus is related to lower scores than controls for walking on standardized tests of motor ability. However, standardized tests do not assess gait kinematics, which may provide a better understanding of motor deficits. This study investigated gait kinematics while walking in children with amblyopia or strabismus. Children ages 7-13 years with amblyogenic factors (13 anisometropia, 8 strabismus) were enrolled in the amblyopia or strabismus (AS) group (15/21 had current amblyopia). An age-similar group of 33 controls with normal visio…

Heavy eye syndrome in a paediatric patient: an unusual presentation of highly myopic restrictive strabismus.

Heavy eye syndrome is an acquired restrictive strabismus typically observed in adult patients with high axial myopia. We report an unusual paediatric case of a teenage girl with progressive myopia and a large-angle esotropia. Coronal orbital MRI confirmed the diagnosis by showing inferior slippage of the lateral recti and nasal slippage of the superior recti. Management involved a two-stage surgical approach: a bilateral Yokoyama procedure followed by bilateral medial rectus recession. This strategy achieved a successful correction of the ocular alignment and significant improvement in ocular …

The global prevalence of horizontal strabismus: A systematic review and meta-analysis with a focus on ethnic variation.

The prevalence of the 2 types of horizontal strabismus, esotropia and exotropia, varies considerably between studies. This variability has been attributed to factors such as geography/environment, research methodology, age of study subjects, and/or ethnicity. Comprehensive estimates of regional and global prevalences of esotropia and exotropia are lacking, making it difficult to recognize true patterns, trends, and etiologies. We compile prevalences and ratios of esotropia to exotropia from 315 population-based studies and 374 clinic-based studies. We analyze data to assess effects of ethnicit…

Novel Approaches to Surgical Correction of Strabismus Based on Current Understanding of Orbital Pulley Anatomy.

Recent anatomic and functional studies of the extraocular muscles have led to a new understanding that their proper function is dependent upon a complex collagenous orbital scaffolding. This structure, for which the term "pulley system" is a useful concise designation, maintains the positions of the eye muscles relative to the globe, to each other, and to the orbit. Disruption of this system results in muscle displacement and dysfunction. Novel strabismus surgical procedures designed to utilize this new anatomic knowledge may yield powerful results while lowering ophthalmic risk. Such procedur…

A Case of Highly Myopic Strabismus That Showed Temporary Large Exotropia After the Yokoyama Procedure.

Highly myopic strabismus is a progressive form of esotropia and hypotropia. It occurs when the globe axis is elongated due to high myopia, causing the posterior portion of the globe to dislocate between the superior rectus and lateral rectus muscles and prolapse from the muscle cone. This condition is often accompanied by mechanical restrictions of eye movement. If it progresses, the globe may become fixed in an inferomedial position. The most common treatment is uniting the superior and lateral rectus muscle bellies using a suture posterior to the muscle insertions, also known as the Yokoyama…

Influence of Pre- and Intraoperative Local Anaesthetic in Strabismus Surgery under General Anaesthesia in Adults.

BACKGROUND: As studies have shown a reduction in the occurrence of the oculocardiac reflex with the addition of local anaesthesia, we changed our care regime accordingly a few years ago. To promote and establish better patient care, we retrospectively analysed the files of our patients who underwent strabismus surgery from 2013 to 2021 in order to compare strabismus surgery under general anaesthesia with and without local anaesthetics in a routine clinical setting. PATIENTS AND METHODS: Data from 238 adult patients who had undergone strabismus surgery could be extracted from the files: G1: n …

Interocular suppression in primary visual cortex in strabismus: impact of staggering the presentation of stimuli to the eyes.

Diplopia (double vision) in strabismus is prevented by suppression of the image emanating from one eye. In a recent study conducted in two macaques raised with exotropia (an outward ocular deviation) but having normal acuity in each eye, simultaneous display of stimuli to each eye did not induce suppression in V1 neurons. Puzzled by this negative result, we have modified our protocol to display stimuli in a staggered sequence, rather than simultaneously. Additional recordings were made in the same two macaques, following two paradigms. In trial type 1, the receptive field in one eye was stimul…

Postoperative Behaviour after Primary Strabismus Surgery in Children: Is There an Influence of Intraoperative Topical Anaesthesia?

INTRODUCTION: The course of the recovery phase after general anaesthesia, during which consciousness and pain gradually return, is essentially determined by the choice of anaesthetic drug, and its total dose or duration of application. This phase is often complicated by more or less severe agitation, especially in children. In the present study we investigated whether the application of a topical anaesthetic to the conjunctiva at the end of a strabismus operation had a positive effect on the recovery behaviour of the child. PATIENTS/METHODS: This prospective, randomised, blinded study was con…
