In a minority of late-onset Group B streptococcal (GBS) cases in neonates, facial or buccal cellulitis has been described. We report a case of sepsis with GBS, in which an atypical cellulitis in the inguinal area was seen as presenting symptom.

Streptococcal infection is a rarely recognized cause of nonrheumatic perimyocarditis. We report a case of a young patient who developed acute perimyocarditis as manifested by diffuse electrocardiogram changes, and markedly elevated cardiac enzymes, concurrently with streptococcal tonsillitis. Despite the dramatic presentation, the patient recovered uneventfully. We conclude that streptococci can involve the heart also directly by a bacterial component or toxin, and not only through a delayed immunologic mechanism as in rheumatic fever. Two young adults had clinical and roentgenographic evidence of explosive pleuritis that was caused by group A beta-hemolytic streptococci. Persistent high fever and intense pleuritic pain following severe pharyngitis should suggest streptococcal pleural infection and prompt careful roentgenographic investigation. These cases show that group A beta-hemolytic streptococcal infection can cause explosive pleuritis in the absence of apparent bronchopneumonia.

Group B streptococcus is not often isolated by gynaecologists when a clinical diagnosis of cervicitis is made. I report on a patient suffering from cervicitis and urethritis caused by this organism, whose symptoms completely resolved after she was treated with phenoxymethylpenicillin.

A 20-year-old man had a penile ulceration from which a pure, heavy growth of group B streptococcus was cultured. Cutaneous infection with this organism is unusual, and it is most often reported in postpartum women, diabetic patients, and immunocompromised individuals. Vaginal colonization with group B streptococcus is common. Since traumatic erosions or ulcerations of the penis are not uncommon, it is likely that a certain number of unrecognized infections of this type occur.

Group B streptococcal infections, although well studied in neonates, have only recently been appreciated as important infectious agents in adults. Seven cases of Group B streptococcal pneumonia were verified by transtracheal aspiration, blood and sputum cultures, or multiple stab cultures at autopsy. The infections were largely nosocomial and, ultimately, fatal in all seven patients.
