Proximal row carpectomy combined with primary flexor pollicis longus tenorrhaphy: An effective treatment alternative for a rare case of flexor pollicis longus rupture due to scaphoid nonunion advanced collapse.

This study describes a case of flexor pollicis longus rupture resulting from long-term scaphoid nonunion advanced collapse. The tendon rupture mechanism was attrition due to sharp bone protuberances from the scaphoid in the carpal tunnel. Although this extremely rare complication has already been reported in the literature, our study is original in showing that proximal row carpectomy with consequent wrist shortening allowed primary tendon repair without transferring the flexor digitorum superficialis tendon of the ring finger or grafting the palmaris longus tendon, which besides using two-lev

Flexor tendon rupture of the little finger caused by calcium pyrophosphate dihydrate crystal deposition disease of the pisotriquetrum joint.

We report a case of closed rupture of the flexor tendons of the little finger caused by calcium pyrophosphate dihydrate crystal deposition disease of the pisotriquetrum joint. The patient could not flex the little finger and did not have wrist pain. Plain radiographs of the affected wrist joint showed severe arthritic changes of the pisotriquetrum joint and calcification around the joint. At operation, the pisotriquetrum joint capsule was ruptured and involved the flexor tendon of the little finger. The distal stump of the flexor tendon was transferred to the flexor tendon of the ring finger,

The management of complications following the treatment of flatfoot deformity.

Adult acquired flatfoot deformity encompasses a wide spectrum of clinical conditions. Current management approaches have emerged from a growing understanding of its manifestations, which have been learned from decades of clinical trial and error. Although surgical trends continue to evolve, many basic principles and practices have endured. Adult flatfoot deformity can arise from multiple causes, the most common of which remains posterior tibial tendon rupture with subsequent elongation of secondary supportive structures. Regardless of the cause, the fundamental goals of surgical management inc

Stem cell therapy: a promising biological strategy for tendon-bone healing after anterior cruciate ligament reconstruction.

Tendon-bone healing after anterior cruciate ligament (ACL) reconstruction is a complex process, impacting significantly on patients' prognosis. Natural tendon-bone healing usually results in fibrous scar tissue, which is of inferior quality compared to native attachment. In addition, the early formed fibrous attachment after surgery is often not reliable to support functional rehabilitation, which may lead to graft failure or unsatisfied function of the knee joint. Thus, strategies to promote tendon-bone healing are crucial for prompt and satisfactory functional recovery. Recently, a variety o

[Muscular rupture of the extensor pollicis longus].

Closed ruptures of the extensor pollicis longus tendon usually appear as a consequence of fractures of the wrist joint or the carpal bones or ensue from polyarthritic changes or result from a process of degeneration. Mechanical injury of the tendon is quite rare but can be observed after direct trauma or after operative treatment of a distal fracture of the radius. Closed traumatic ruptures of the extensor pollicis longus tendon in the absence of pathological changes are--in spite of the frequency of rotation injuries of the forearm--very rare. The patient in the case described is a 45-year-ol

Adult-acquired flatfoot deformity following posterior tibialis to dorsum transfer: a case report.

A previous study showed that flatfoot deformity does not develop after posterior tibialis to dorsum transfer in patients with peroneal nerve palsy. Their conclusion was that it is the unopposed pull of the peroneus brevis which leads to the flatfoot deformity in posterior tibial tendon dysfunction. This case report presents a patient who developed a flatfoot deformity after posterior tibialis to dorsum transfer despite nonfunctioning peroneal muscles.

[Long-Term Results of Extensor Indicis Transposition Following Rupture of the Extensor Pollicis Longus Tendon after Paediatric Forearm Fracture].

BACKGROUND: A rare but typical perioperative complication in the surgical treatment of pediatric forearm fractures is injury to the extensor pollicis longus (EPL) tendon. This article presents the long-term results after reconstruction of the EPL tendon with transposition of the extensor indicis (EI) tendon in children and adolescents after surgically treated forearm fracture. PATIENTS AND METHODS: Over a period of 15 years, 22 children up to the age of 16 were treated with EI transposition analogous to adult care. In a follow-up examination, the range of motion of the finger and thumb joints

Foreign body granulomatous reaction associated with polyethelene 'Fiberwire(®)' suture material used in Achilles tendon repair.

Repair of acute Achilles tendon rupture is a common procedure. Histology revealed a granulomatous reaction with giant cell response surrounding sections of the suture. Both the silicone coating of Fiberwire(®) and polyethylene core have the potential to cause a severe granulomatous reaction. We would advise ca …

Hemorrhagic ganglion of the tibialis anterior tendon: report of an unusual variant.

While clinicians have encountered heme-laden ganglion cysts in the past, to our knowledge the condition has never been described in the literature as a distinct pathological entity, namely, the hemorrhagic ganglion. Because hemorrhage may alter the appearance of an aspirate obtained from a ganglion cyst, and hemosiderin deposition within a ganglion cyst may alter the signal characteristics observed on a magnetic resonance image, such that both of these diagnostic methods may suggest the presence of a solid mass tumor rather than a ganglion cyst, it is important for clinicians and surgeons to b

Cerebral palsy. Evaluation and management of equinus and equinovarus deformities.

Walking function may be enhanced by correcting equinus and equinovarus deformities in CP. Nonoperative management should be used in young children. Muscle balancing procedures, such as gastrocnemius aponeurosis lengthening, Achilles' tendon lengthening, and SPLATT, are particularly useful in correcting these deformities.

[Transposition of scapular muscles in the treatment of injuries of the rotator cuff of the shoulder (RCS)].

An experience of 13 cases of short rotator muscles advancement is reported. These procedures were found to be available in treatment of rotator cuff avulsions with retraction, when anatomical reinsertion or shoe-lace suture after Mclaughlin appeared to be impossible. The authors had performed advancement of the supraspinatus alone in 10 cases and simultaneous advancement of supra-, infraspinatus and teres minor in 3 cases. Only one poor result was obtained that can't but confirm a most high efficacy of the procedures described.

Optimization of Subscapularis Tendon Repair in Stemless Shoulder Arthroplasty Using a Resorbable Pressure-Dissipating Onlay Suture Disk Implant.

Successful subscapularis repair in stemless shoulder arthroplasty is crucial to reduce complications and improve postoperative function. As stemless shoulder arthroplasty continues to grow in popularity, several subscapularis tendon repair techniques are being developed, with a current trend toward knotless devices and double-row anchor-based constructs. In this article, we present our technique for repair of a subscapularis tendon peel using a suture-capture construct that aids in compression of the tendon onto its footprint and then gradually releases the tension as the capture resorbs and t

Operative management of foot and ankle equinovarus associated with focal dystonia.

Six surgical procedures, consisting of tendon transfers and releases, were performed in five patients with idiopathic focal dystonia involving the lower extremity. All patients were female. Surgical management was performed to correct clinically significant foot abnormalities. The goal of each procedure was functional improvement and obtaining a plantigrade foot. The SPLATT (split anterior tibial tendon transfer) procedure was performed in each foot with a flexible equinovarus foot abnormality. Follow up at a mean of 27.2 months (range, 8-40 months) yielded satisfactory clinical results withou…

The mechanical integrity of the arch of the foot depends on static and dynamic factors. The posterior tibial tendon in particular is situate … ndon should be attempted. As retraction of the tendon ends or degeneration prevents primary repair, tendon transfer may substitute for the posterior tibial tendon. When a fixed flatfoot has developed, realignment by arthrodesis is advised. Triple arthrodesis may be appropriate to correct hindfoot valgus as well as forefoot abduction.

Relationship Between Structure and Age in Healthy Achilles Tendons.

Age is an important factor to consider with Achilles tendon injury, as variability in tendon structure during developmental growth and aging influence lower limb function and mobility. However, the overlap in structural alterations with aging and Achilles tendon injury makes it unclear which structural changes are related to age separate from tendon pathology. The objective of this study was to determine the relationship between structure and age in healthy Achilles tendons. Healthy Achilles tendons from 389 children and adults (8-79 years) were included in this retrospective analysis. Achille

Foreign Body Reaction Following Achilles Tendon Reconstruction With the Ligament Advanced Reconstructive System: Patient Outcomes and Clinical Course.

The Ligament Advanced Reinforcement System (LARS) is a common choice for ligament reconstruction in the lower limb due to its good functional and quality of life (QoL)-related outcomes. It is commonly used for Achilles tendon repair following a rupture. While it facilitates tissue ingrowth and boasts good biocompatibility, we report on multiple cases whereby foreign body reactions have led to the growth of granulomas requiring surgical excision and Flexor Hallucis Longus (FHL) transfer. Following these cases, patients have been shown to have excellent functional and QoL-related outcomes using
