A 40-year-old woman was admitted because of progressive dyspnea on exertion. Magnetic resonance imaging revealed an atrial septal defect and an abnormal membranous structure separating the left atrium into proximal and distal chambers. Surgical correction was performed by a minimally invasive surgical approach, i.e., port-access heart surgery, under the diagnosis of cor triatriatum (subtype of Lucas Schmidt IIIA1 or IB1) complicated with atrial septal defect. The postoperative course was uneventful, and she was discharged 12 days after surgery. This case is the first report of a surgical corre

The presence of an azygos lobe (AL) may make it difficult or even impossible to use video-assisted thoracic sympathectomy if the surgeon is unprepared to deal with this anatomical anomaly. Seven hyperhidrosis patients with an AL from a total of 1,876 individuals who underwent video-assisted thoracic sympathectomy are presented. The surgical technique and clinical evolution are described. For these 7 patients, the surgical intervention could be performed using thoracoscopy. Only 1 patient required postoperative pleural drainage. Another patient presented atelectasis in part of the AL, which was

A 21-year-old adult with previously diagnosed Marfan syndrome (MS) was admitted to our hospital complaining of chest pains that resulted from a complete left spontaneous pneumothorax (SP). A chest roentgenogram showed a 70% pneumothorax on the left side. Because for 8 days chest tube drainage did not ameliorate the air leak, and also because a computed tomography (CT) scan showed many cystic opacities at the apical lung, a bullectomy was performed under video-assisted thoracic surgery (VATS). The patient's family history revealed that his father and two brothers had MS. Three family members ha

The resection of pulmonary metastases can prolong the survival of selected patients and its therapeutic value is now accepted. The criteria for eligibility have also evolved. We reviewed the recent literature on pulmonary metastasectomy for various epithelial primary tumors and tried to establish better prognostic indicators for its surgical application. In addition to the welldefined requisites for pulmonary metastasectomy, other requirements include the absence of mediastinal lymph node involvement, a limited number of pulmonary metastatic lesions, a long disease-free interval, small metasta

OBJECTIVE: To investigate the perioperative management and video-assisted thoracoscopic (VATS) extended thymectomy for myasthenia gravis (MG).
METHODS: A total of 102 patients who received extended thymectomy for MG from June 2000 to August 2007 were divided into 2 groups by operative approach: 51 in the VATS group and 51 in the full median sternotomy group. The clinical features, including operation time, operation blood loss, post-operative drainage, incidence of crisis, hospital stay, and the 1-year total effective rate after the operation were compared. Complications were treated exactly

Pectus deformities arise from overgrowth of the cartilagenous portion of the ribs. Traditional resective/open procedures have been almost entirely replaced by video assisted retrosternal bar placement with excellent outcomes. This review considers the comprehensive evaluation and care of a pectus patinet as well as technical details regarding conduct of the surgery.

BACKGROUND: Hyperhidrosis is pathological perspiration in palmar, plantar or axillary surfaces. Video-assisted thoracic surgery (VATS) is currently the most commonly used therapy for hyperhidrosis. Blockage of sympathetic ganglia is achieved by segmental resection, transection and/or cauterization, and clipping of the chain. We aimed to compare the efficacy of these methods with respect to patient satisfaction, recurrence of symptoms and complications.
METHODS: Eighty male patients with a mean age of 22.02 +/- 2.61 years undergoing bilateral thoracoscopic sympathectomy or sympathetic blockage

The treatment possibilities of broncho-pleural fistula (BPF) caused by necrotizing pneumonia (NP) are discussed based on the experiences of a case and on the basis of the literature. In case of pleural fluid caused by NP the importance of thoracic drainage and--in selected cases--video assisted thoracoscopy (VATS) are emphasized. If BPF develops and the oxygen saturation in the blood can not be kept on acceptable level because of the large volume of the lost air through the fistula bronchoscopic occlusion of the bronchus leading to the BPF is recommended. In conservative therapy resistant case

Fetal pleural effusion, a nonspecific accumulation of fluid in the pleural space, is an uncommon anomaly which can be associated with aneuploidy and a range of other structural malformations or genetic syndromes. Spontaneous resolution is not rare and confers a good prognosis. Perinatal outcome is better for those fetuses without hydrops than those presenting with hydrops. A detailed review of the literature indicates that, for fetuses with persistent effusions, in utero intervention (repeated thoracocentesis, intrauterine shunting and pleurodesis) may improve the chances of survival.

A video assisted thoracoscopic surgery (VATS) thymectomy is a viable alternative to conventional open thymectomy in myasthenia gravis (MG). A previous operation in the same anatomical region is generally considered a relative contraindication to any minimally invasive approach in the same area. Few cases of VATS on previously operated chests have been reported. We report a case of a VATS thymectomy in a patient who had undergone two previous sternotomies for cardiac disease.

To support an infectious disease study, we developed a surgical procedure to collect serial thymic biopsies in rhesus monkeys (Macaca mulatta). In many instances in which thymic tissue is required from living animals, open surgical approaches (thoracotomies) are used, which result in greater postoperative pain and longer recovery periods than those associated with thoracoscopic procedures. Our intent was to develop a surgical procedure that allowed serial biopsy of the thymus with minimal surgical morbidity. We modified a previously published experimental method of thoracoscopic total thymecto

The management of pleural infection remains highly challenging. Rational selection of appropriate local treatment strategies is crucial for successful management. Local treatment decisions are primarily guided by clinical features, pleural fluid biochemical parameters, and imaging characteristics, with pleural fluid pH below 7.2 directly indicating the need for pleural drainage. When multiloculated pleural effusion develops, simple drainage frequently fails, necessitating advanced interventions including intrapleural fibrinolytic therapy (IPFT), medical thoracoscopy (MT), or surgical intervent

BACKGROUND: Situs inversus totalis (SIT) is a rare congenital anomaly characterized by the complete reversal of the major thoracic and abdominal visceral organs from their normal positions. The coexistence of SIT and lung cancer is exceedingly uncommon, posing significant challenges to surgeons due to increased surgical complexity. We report the case of a 65-year-old male was hospitalized upon the discovery of pulmonary lesions. Computed tomography (CT) revealed ground-glass nodules in the upper lobe of the left lung, as well as multiple small nodules throughout the remaining lung. The preoper

AIM: Pulmonary sequestration is a rare congenital anomaly of the lung characterized by nonfunctioning pulmonary tissue that lacks normal communication with the tracheobronchial tree and receives blood supply from the systemic rather than the pulmonary circulation. Many patients remain asymptomatic for years, resulting in delayed diagnosis until adulthood. We report a case of adult intralobar sequestration (ILS) in which early diagnosis was obscured by bronchiectasis. This case aims to increase clinical awareness and provide insights into clinical strategies to reduce missed diagnoses.

BACKGROUND: Catamenial pneumothorax (CP), the most common manifestation of thoracic endometriosis syndrome, is a rare form of primary spontaneous pneumothorax that occurs in women of reproductive age. Although CP is usually treated surgically or with hormonal therapy, there is no standard treatment and recurrence is common. We performed video-assisted thoracoscopic surgery (VATS) for five patients with CP from 2014 to 2023.
METHODS: Our surgical basic procedure (BP) was as follows. VATS was performed by using one port site for the camera and a 5-cm mini-thoracotomy. Perforated holes or bluebe

Thymectomy is performed for the treatment of myasthenia gravis (MG) and the removal of thymic lesions. Minimally invasive techniques such as video-assisted thoracoscopic surgery (VATS) have now become a standard of care for thymectomies. We present the case of a woman in her early 50s with non-thymomatous MG and left atrial isomerism. As part of left atrial isomerism, she had bronchial isomerism with no right middle lobe, hemiazygous vein continuation of the inferior vena cava, bilateral superior vena cavae and no left brachiocephalic vein. A bilateral VATS approach provided a safe and effecti

Thoracoscopic sympathectomy can be used to treat primary hyperhidrosis (PH). Nonetheless, there is a paucity of literature addressing the postoperative complications associated with this procedure. We report a case of a 21-year-old male patient who developed prolonged bilateral pleural effusion after undergoing thoracoscopic sympathectomy for PH. This case aims to raise awareness of this rare complication and discuss effective management strategies for it.

BACKGROUND Hydatid disease remains a major clinical concern, particularly in regions where it is endemic. This parasitic infection is caused by Echinococcus species. The liver and lungs are the most affected organs. Although the lungs are the most commonly affected intrathoracic organ, extrapulmonary intrathoracic hydatid disease is uncommon. However, primary mediastinal hydatid disease is a rare entity, and a primary hydatid cyst in the thymus is extremely rare. Although video-assisted thoracoscopic surgery (VATS) plays an important role in the field of thoracic surgery, its role in treating

RATIONALE: Giant pulmonary hydatid cysts in children are rare, with nonspecific symptoms often delaying diagnosis. Preservation of lung parenchyma during surgery is essential. This is the first reported case of such a cyst in a Palestinian pediatric patient.
PATIENT CONCERNS: A 5-year-old girl presented with progressive left flank pain, dyspnea, and fever; initially misdiagnosed as gastroenteritis.
DIAGNOSES: Chest X-ray and computed tomography revealed a hydatid cyst occupying >90% of the left lower lung lobe with pleural effusion.
INTERVENTIONS: Urgent lateral thoracotomy combined with vi
