Thyroglobulin levels in COVID-19-positive patients: Correlations with thyroid function tests, inflammatory markers, and glucocorticoid use.

COVID-19 often results in generalized inflammation and affects various organs and systems. Endocrine research focused on the possible sequelae of COVID-19, with special interest given to the thyroid gland. Clinical problems such as thyroid function in non-thyroidal illness (NTI), autoimmune thyroiditis, and COVID-19-related subacute thyroiditis (SAT) quickly gained wide coverage. Thyrotoxicosis of various origins leads to the release of peripheral thyroid hormones and thyroglobulin (TG), the main glycoprotein contained within the thyroid follicular lumen. In our study, we evaluated TG levels i

[Discordance of total plasma l-triiodothyronine and l-thyroxine in thyroid diseases].

Radioimmunoassay (RIA) for plasma T3 has been proposed in the diagnosis of thyroid disease. 1 171 simultaneous determination (RIA) of plasma Thyrotropin-Stimulating Hormone (TSH), triiodothyronine (T3) and thyroxine (T4) have been performed in unselected patients under suspicion of a thyroid disease. 81.8% of the cases showed a good agreement between T3 and T4 levels. The remaining 18.2% can be divided into 6 groups: 1. High T3-normal T4 : 51% of the discrepancies: Graves disease untreated (16%) or treated (4%); sporadic goiter (13%); growth failure (8%); hypothyroidism under treatment (6%); t

Development of Graves' Disease After SARS-CoV-2 mRNA Vaccination: A Case Report and Literature Review.

Background: Mounting evidence has revealed the interrelationship between thyroid and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) to explain the thyroid dysfunction and autoimmune thyroid disorders observed after coronavirus disease 2019 (COVID-19). There are limited reports of thyroid dysfunction after SARS-CoV-2 vaccination. Methods: We report a case of a 40-year-old Chinese woman who developed Graves' disease after BNT162b2 mRNA vaccine. A search of PubMed and Embase databases from 1 September 2019 to 31 August 2021 was performed using the following keywords: "COVID," "vacci

Passive Smoking and Thyroid Diseases: Association by Mendelian Randomization Study.

INTRODUCTION: Existing literature has primarily focused on the associations between active smoking and thyroid diseases, whereas the potential causal relationship between passive smoking and thyroid diseases remains insufficiently investigated. Therefore, we conducted a Mendelian randomization (MR) study to evaluate the potential associations of passive smoke exposure with seven prevalent thyroid diseases. METHODS: A two-sample MR design was employed using data from the IEU OpenGWAS Project. Five MR methods were applied to assess potential causal associations. Cochran's Q test assessed hetero

Thyroid as a target of adjuvant autoimmunity/inflammatory syndrome due to mRNA-based SARS-CoV2 vaccination: from Graves' disease to silent thyroiditis.

BACKGROUND: As COVID-19 became a pandemic, the urgent need to find an effective treatment vaccine has been a major objective. Vaccines contain adjuvants which are not exempt from adverse effects and can trigger the autoimmune/inflammatory syndrome induced by adjuvants (ASIA). There is very little information about autoimmune endocrine disease and the ASIA after the use of mRNA-based SARS-CoV2 vaccination. CASE SERIES: We report three cases and also review the literature showing that the thyroid gland can be involved in the ASIA induced by the mRNA-based SARS-CoV2 vaccination. We present the f

Clinical value of the first automated TSH receptor autoantibody assay for the diagnosis of Graves' disease (GD): an international multicentre trial.

BACKGROUND: Most recently, a new rapid and fully automated electrochemiluminescence immunoassay for the determination of TSH receptor autoantibodies (TRAb) based on the ability of TRAb to inhibit the binding of a human thyroid-stimulating monoclonal antibody (M22) has been established. OBJECTIVE: To evaluate this assay system in clinical routine based on an international multicentre trial and to compare the results with other established TRAb assays. PATIENTS AND MEASUREMENTS: Totally 508 Graves' disease (GD), 142 autoimmune thyroiditis, 107 subacute thyroiditis, 109 nonautoimmune nodular go

Clinical and diagnostic aspects of encephalopathy associated with autoimmune thyroid disease (or Hashimoto's encephalopathy).

Encephalopathy associated with autoimmune thyroid disease, currently known as Hashimoto's encephalopathy, but also defined as corticosteroid-responsive encephalopathy associated with autoimmune thyroiditis, is a relatively rare condition observed in a small percentage of patients presenting with autoimmune thyroid disease. It consists of a subacute, relapsing-remitting, steroid-responsive encephalopathy characterised by protean neurologic and neuropsychiatric symptoms, diffuse electroencephalographic abnormalities and increased titres of antithyroid antibodies in serum and/or in cerebrospinal

[Determination of antibody antireceptor of TSH, experience in patients with thyroid disorders and controls].

In order to measure TSH receptor antibodies (TRAb) we tried to set up a radioreceptor assay using human thyroid membranes. Due to lack of appropriate binding activity of the material obtained, we decided to use a kit which provides solubilized porcine membrane-receptors to TSH instead of human membranes, as well as calibrators that have been standardized in a receptor assay against MRC LATS std B. With these reactives we have measured TRAb in sera from 7 normal controls (C), 54 thyrotoxic patients (43 diffuse goiters [BDH], 8 multinodular goiters [BHM] and 3 Subacute Thyroiditis [TSA]), 3 pati

Effects of thyroid hormone on erythrocyte carbonic anhydrase-I and zinc concentrations in vivo and in vitro: clinical usefulness of carbonic anhydrase-I and zinc concentrations in erythrocytes.

Red blood cell (RBC) carbonic anhydrase-I isozyme (CAI) and zinc concentrations were measured in patients with various thyroid diseases. Both concentrations were decreased in patients with hyperthyroid Graves' disease, and the RBC CAI concentration significantly (r = 0.95) correlated with the RBC zinc concentration. After treatment, the normalization of RBC CAI and zinc lagged 2 months behind normalization of plasma T4 and T3 levels. Furthermore, the highest correlation coefficients were observed between RBC CAI and zinc levels and plasma thyroid hormone levels measured 8 weeks earlier. These

Recurrent steroid-intolerant Hashimoto's encephalopathy responsive to single-dose neonatal Fc receptor antagonist: a case report and literature review.

BACKGROUND: Hashimoto’s encephalopathy (HE) is a rare, underdiagnosed, steroid-responsive autoimmune condition associated with thyroid autoimmunity. CASE PRESENTATION: An 83-year-old female patient was admitted to hospital displaying symptoms of subacute cognitive decline, dysarthria and myoclonus. She had a history of Hashimoto’s thyroiditis and diabetes. Given the patient’s age and comorbid diabetes, the potential risks associated with steroid treatment were considered excessive. She was given an 800 mg dose of efgartigimod-α, which swiftly improved her ambulatory stability and cognitive fun

Palatal-Myoclonus as a Presentation of Hashimoto Encephalopathy: an interesting case report.

OBJECTIVE: Hashimoto encephalopathy (HE) is known as a steroid- responsive encephalopathy associated with autoimmune thyroiditis or nonvascular inflammation-related autoimmune meningoencephalitis. The average age of onset of HE is approximately 50 years; and it is more common in women. The onset of HE may be acute or subacute. The course of most HE cases is relapsing and remitting, which is similar to that of vasculitis and stroke. METHODS: In this article, we present a previously healthy 32 years old;veterinarian male with palatal myoclonus, as a rare presentation of this disorder, and revie

A different look at corticosteroids.

Systemic corticosteroids have been used in the treatment of numerous medical conditions for approximately 50 years. Short-acting products such as hydrocortisone are the least potent. Prednisone and methylprednisolone, which are intermediate-acting products, are four to five times more potent than hydrocortisone. Dexamethasone is a long-acting, systemic corticosteroid; its potency is about 25 times greater than the short-acting products. Corticosteroids reduce the need for hospitalization in patients with croup and decrease morbidity and the incidence of respiratory failure in the treatment of

Evaluation of a radioreceptor assay for TSH receptor autoantibodies.

A commercial radioreceptor assay for TSH receptor autoantibodies (TRAb), based on solubilized porcine receptor and purified radio-iodinated bovine TSH, was tested in 264 subjects with a variety of thyroid disorders. The sensitivity of the assay for the detection of hyperthyroid Graves' disease was 91%. The assay specificity for Graves' disease was 95%. With the exception of one patient with Hashimoto's disease and one patient with de Quervain's subacute thyroiditis no subjects other than Graves' patients had detectable TRAb. Thus purely blocking TSH receptor autoantibodies were not detected wi

Benign thyroid disease: what is the role of nuclear medicine?

Nuclear medicine is directly involved in both the diagnosis and treatment of benign thyroid disease, which requires an understanding of the pathophysiology and management of thyroid disorders in addition to expertise in nuclear methodology. Thyroid uptake and imaging, the principal nuclear tests in thyroid disease, may be used as follows: (1) Differential diagnosis of hyperthyroidism: A very low thyroid uptake suggests destructive ("subacute") thyroiditis, a self-limited disorder, whereas a normal or elevated uptake is consistent with toxic nodular goiter and Graves' disease. Subacute combined degeneration of the spinal cord with concomitant autoimmune disease: report of 2 cases.

The etiology of subacute combined degeneration (SCD) of the spinal cord is closely associated with vitamin B12 (VitB12) deficiency. The clinical manifestations of SCD are complex and vary substantially. Due to some SCD patients with atypical manifestations and concomitant autoimmune disorders, the probability of misdiagnosis and missed diagnosis is still relatively high in the early stage. We report the cases of two patients who were missed or misdiagnosed at another hospital because of the normal initial VitB12 level and partial overlap of clinical manifestations, finally diagnosed as SCD wit

Thyrotoxic periodic paralysis in long standing graves' disease: an unusual presentation with normokalemia.

CONTEXT: Thyrotoxic periodic paralysis (TPP) is a potentially life-threatening complication of hyperthyroidism that is underdiagnosed and frequently missed. It is relatively common in Asian men with Graves' disease. TPP attacks are frequently associated with hypokalemia. CASE REPORT: Here we report a non-Asian female patient with Graves' disease, who presented with flaccid paralysis associated with an episode of subacute thyroiditis (SAT). Interestingly, she was found to have low normal potassium levels in the serum during the initial attack despite which she continues to require low dose pot

Hashimoto encephalopathy presenting as progressive myoclonus epilepsy syndrome.

We report an 11 year old girl with multi-focal spontaneous myoclonus, generalized seizures and behavioural changes. She was found to have sub-clinical hypothyroidism and elevated anti-thyroid peroxidase antibodies. A diagnosis of Hashimoto encephalopathy (or Steroid Responsive Encephalopathy with Autoimmune Thyroiditis) was made in view of consistent clinical and laboratory features. The patient showed and maintained significant improvement with valproate, clonazepam and long term prednisolone. Other findings included mild cerebral cortical grey matter atrophy on brain magnetic resonance imagi

Serum calcitonin in hyperthyroidism.

The mineral content of the radius was found to be lower in thyrotoxic than in euthyroid women, particularly in the case of elevated serum triiodothyronine levels. On examining the hormones influencing bone metabolism, the basal serum calcitonin level of thyrotoxic patients was identical with that of the controls. The Ca and calcitonin responses to i.v. Ca loading (3.64 mg CaCl2/kg/3 min) were identical in the euthyroid and hyperthyroid subjects. The results suggest that the calcitonin reserves in Graves' disease are normal and that the bone abnormalities typical of this disease are unrelated t

Serum concentrations of Thymidine kinase 1 measured using a novel antibody-based assay in patients with Hodgkin Lymphoma.

OBJECTIVE: To gain an insight into risk factors for hypothyroidism after subacute thyroiditis (SAT), we examined the correlation between initial laboratory and ultrasonographic findings and sequential thyroid dysfunction among treatment modalities. PATIENTS: We reviewed retrospectively the medical records of 252 patients (26 men and 226 women) with SAT who consecutively visited our thyroid clinic at Kuma Hospital for at least 6 months from 1996 through 2004. RESULTS: Throughout the course, 135 patients (53.6%) developed transient or permanent hypothyroidism. Levels of TSH were most often ele
