Novel use of biodegradable temporising matrix for non-healing wounds following non-medical grade silicone injectionables.

Silicone injections are frequently used for aesthetic purposes, demonstrating safety when administered by ethical licensed medical professionals using medical-grade silicone. However, when non-medical-grade silicone is used for injections, complications can arise, such as silicone migration, cellulitis, silicone nodules, granulomatous formations and chronic open wounds. These wounds can be disfiguring and extremely difficult to treat. In this article, we present the case of a late 30-year-old male-to-female transgender patient who underwent non-medical-grade silicone injections in Mexico 20 ye

Aesthetic Considerations for Neuromodulator Use in Transgender Patients.

BACKGROUND: The aesthetic dimension of transgender experiences involves various aspects linked to the visual representation and expression of one's gender identity. Nonsurgical cosmetic procedures, such as neuromodulators, have consequently become a viable treatment option for patients with gender dysphoria because of the ability to yield significant, although temporary, results with minimal associated risks and downtime. OBJECTIVE: To comprehensively review literature pertaining to the aesthetic considerations for use of neuromodulators in transgender and nonbinary individuals. MATERIALS AN

Incidence of breakthrough bleeding in transgender and gender-diverse individuals on long-term testosterone.

BACKGROUND: Little is known about the maintenance of amenorrhea among transgender and gender-diverse individuals with uteri who are using long-term testosterone gender-affirming hormone therapy. Emerging data describe breakthrough bleeding among adolescents on long-term testosterone therapy and among adults who are seeking a gender-affirming hysterectomy. More studies are needed to better understand breakthrough bleeding patterns among transgender and gender-diverse individuals with uteri who are using testosterone, including the frequency, timing, and etiology of bleeding and how these patter

Lived Experience of Acne and Acne Treatment in Transgender Patients.

IMPORTANCE: While acne is common in transgender and gender-diverse people and is associated with gender-affirming hormone therapy, little research has examined these factors and their impact in gender minority groups. OBJECTIVE: To examine the lived experiences of acne and acne treatment in transgender and gender-diverse participants. DESIGN, SETTING, AND PARTICIPANTS: This mixed-methods analysis was conducted at a multidisciplinary gender center at a public safety-net hospital and endocrinology and dermatology clinics at a tertiary academic center from January 4, 2021, to April 7, 2022, usi

Inequities in the distribution of adverse childhood experiences and their association with health among transgender people of color.

BACKGROUND: Racism and cisgenderism expose transgender people of color to adversity across the life course. However, little is known about the prevalence of adverse childhood experiences (ACEs) in this population or their association with health in comparison to other groups. OBJECTIVE: Guided by the structural trauma framework, we examined race/ethnicity/gender group differences in the prevalence of ACEs and their association with adult mental and physical health. PARTICIPANTS AND SETTING: 2019-2021 Behavioral Risk Factor Surveillance Survey. METHODS: Transgender participants (n = 551) wer

Children's attitudes about transgender identity disclosure and concealment.

Supportive peers are crucial for transgender children's well-being. Transgender children who live in their affirmed gender face decisions surrounding concealment and disclosure of their transgender identity. We sought to understand how cisgender (N = 115) and gender-diverse children (N = 127), and siblings of gender-diverse children (N = 63) think about transition disclosure and concealment. All groups viewed transition disclosure and concealment positively. However, gender-diverse children showed greater acceptance of transition concealment and had stronger liking of transition concealers (re

Association between gender diversity and substance use experimentation in early adolescents.

BACKGROUND: Gender diversity, encompassing gender identity beyond traditional binary frameworks, has been associated with substance use during adolescence. However, there is a paucity of studies that consider different dimensions of gender diversity. This study investigates associations between multiple dimensions of gender diversity and substance experimentation in early adolescents. METHODS: Data from 10,092 adolescents aged 12-13 years from the Adolescent Brain Cognitive Development (ABCD) study were used to assess the relationship between gender diversity and substance experimentation. Effect of testosterone replacement therapy on scar quality in gender-affirming mastectomies.

BACKGROUND: The term transgenders refer to people who sense discordance between their gender identity and the sex assigned to them at birth. Some transgenders receive hormonal therapy, which may lead to specific skin conditions. The purpose of the present study was to determine whether a causal relationship exists between hormonal therapy (specifically testosterone therapy) and unsatisfying surgical scarring, including hypertrophic and keloid scars. In addition, this study may serve as a basis for future studies, which may test means that aim to reduce such undesired effects. METHODS: In this

Reproductive health in trans and gender diverse patients: Transgender medicine: contextual trans gynecology.

IN BRIEF: This manuscript aims to promote an understanding of the special needs of transgender and gender-diverse people by putting trans gynecology into context. By definition, medical interventions for transgender people are not different from the treatment of cisgender people but cannot be singled out as gender-affirmative treatment. Treatment is dedicated to an overarching goal beyond medicine, namely to enable and maintain social functioning under the signs of the social gender. It is therefore that this narrative overview intensively deals with the requirements of trans gynecology. Also,

Bone Health in the Transgender and Gender Diverse Youth Population.

PURPOSE OF REVIEW: The purpose of this review is to summarize the scientific evidence on bone health in transgender and gender diverse (TGD) youth. RECENT FINDINGS: Gender-affirming medical therapies may be introduced during a key window of skeletal development in TGD adolescents. Before treatment, low bone density for age is more prevalent than expected in TGD youth. Bone mineral density Z-scores decrease with gonadotropin-releasing hormone agonists and differentially respond to subsequent estradiol or testosterone. Risk factors for low bone density in this population include low body mass i

Severe ectopic Cushing syndrome in a transgender man with a metastatic gastrinoma and an adrenal tumor-A case report and review of the literature.

A 38-year-old transgender man with advanced metastatic functional pancreatic neuroendocrine neoplasm (PanNEN) gastrinoma was admitted to the Department of Endocrinology due to severe ACTH-dependent hypercortisolemia. An ectopic production of ACTH by PanNEN was suspected. The patient qualified for bilateral adrenalectomy after preoperative treatment with metyrapone. Finally, the patient underwent resection of the left adrenal gland with the tumor only, which surprisingly resulted in a significant decrease in ACTH and cortisol levels, leading to clinical improvement. Pathology report revealed an

Gender-Affirming Surgery and Cancer: Considerations for Radiation Oncologists for Pelvic Radiation in Transfeminine Patients.

Access to gender-affirming surgery is increasing for many transgender and nonbinary people in the United States, and radiation oncologists must be equipped to care for patients who have undergone such surgery in the region of their planned radiation treatment field. There are no guidelines for radiation treatment planning after gender-affirming surgery, and most oncologists do not receive training in the unique needs of transgender people with cancer. We review common gender-affirming genitopelvic surgeries for transfeminine people, including vaginoplasty, labiaplasty, and orchiectomy, and sum

Strategies to increase HIV testing among men who have sex with men and transgender women: an integrative review.

BACKGROUND: Men who have sex with men (MSM) and transgender women (TGW) are disproportionately affected by HIV, with much higher incidence and prevalence rates than in the general population in different countries. There are several barriers to testing among MSM and TGW, such as low risk perception, anticipation of HIV-related stigma, discrimination of sexual orientation, in addition to difficulties related to care and access to health services. Therefore, analyzing the available evidence of the effectiveness of strategies for scaling up HIV testing among key populations is essential to point

Community Health Care Utilization Patterns in Transgender Individuals Who Have Been Incarcerated.

Transgender individuals experience higher risk of incarceration and chronic disease than cisgender individuals. Both transgender and previously incarcerated individuals can face barriers to accessing health care in the community. This study aims to investigate differences in health care utilization in the community between transgender and cisgender individuals who have been incarcerated. We evaluated emergency department (ED), urgent care, and primary care visit data for individuals incarcerated in Santa Clara County jails over a 3-year period. Among the study population, 0.57% were identified

Mental health of transgender people in prison: a systematic review and meta-analysis.

This systematic review aims to explore the condition of Transgender Inmates (TGI) in correctional facilities (CF), focusing on their mental health, their access to Gender-affirming Treatments (GAT), and their placement within the CF. Medline, Embase, Scopus, PsychInfo, and CINAHL were searched for studies on TGI in CF. Pooled Odds Ratios (ORs) with 95% confidence interval (95%CI) were estimated through inverse variance models with random-effects. A narrative synthesis of each study's findings was also performed since only a few studies included a control group. Thirteen studies were selected a

Building an Academic Transgender Medicine Center of Excellence: The 5-Year Johns Hopkins Experience.

Gender-affirming care for transgender and gender diverse (TGD) individuals is a multidisciplinary endeavor that requires organized efforts of many specialized practitioners. TGD individuals experience many health care barriers, including the scarcity of multidisciplinary teams formed to coordinate and deliver complex care in an efficient and affirming way. The Johns Hopkins Center for Transgender Health was founded in 2017 with the mission of decreasing health disparities and improving the health of the TGD community. The authors present their experience building the center around a service li

Transgender People's Experiences Sharing Information With Clinicians: A Focus Group-Based Qualitative Study.

PURPOSE: Investigating transgender people's experiences sharing health information in clinical encounters may yield insights for family medicine clinicians. METHODS: This was a qualitative study using a community-based participatory research approach and interpretive description methodology. Seven qualitative focus groups were conducted with 30 transgender adults living in North America. We used purposive sampling to ensure diversity. The focus groups were transcribed verbatim, and 2 investigators independently reviewed and coded each transcript, then they mutually reviewed the transcripts, r

Transidentities and autism spectrum disorder: A systematic review.

Transidentity and autism frequently co-occur. Previous reviews have focused mainly on frequencies. Here, we conducted a systematic review to condense all the studies and themes on this co-occurrence and to offer a global view. We followed the PRISMA method and selected 77 articles (including 59 clinical studies) in April 2022. We found 5 main themes (sex ratio, theories, sexual orientation, clinical and social consequences, and care implications) in addition to frequencies. Many theories have been proposed to explain the co-occurrence. One posits that social difficulties related to autism woul
