A review of 56 cases of chronic closed lock treated with temporomandibular joint arthroscopy.

PURPOSE: The aims of this prospective clinical study were to look at the features that constitute chronic closed lock of the temporomandibular joint (TMJ) and to assess the effectiveness of TMJ arthroscopic lavage and lysis in the management of this condition. PATIENTS AND MATERIALS: Sixty joints in 56 patients who presented with mandibular hypomobility suggestive of chronic closed lock were prospectively examined and treated with TMJ arthroscopic lavage and lysis during a 3-year period from 1996 to 1999. RESULTS: Eighty-seven percent (49 of 56) of patients were found to have chronic closed

First case of bilateral coronoid hyperplasia in monozygotic twin sisters-a new aetiological perspective?

BACKGROUND: Mandibular coronoid hyperplasia is a rare condition associated with gradual reduction in mouth opening. Its aetiology is unknown but increased temporalis activity, endocrine stimulus, trauma and familial causes have been proposed. CASE REPORT: In this article, we present a case of bilateral coronoid hyperplasia presenting with limited mouth opening in 16-year-old Caucasian twin sisters. DISCUSSION: This report gives a new perspective to the aetiology of coronoid hyperplasia since it describes its occurrence in monozygotic twins and hence provides a strong argument in support of a

Carotid artery rupture and cervicofacial actinomycosis.

Cervicofacial actinomycosis is an uncommon, progressive infection caused by bacilli of the Actinomyces genus. Actinomyces are common commensal saprophytes in the oral cavity which may have medical importance as facultative pathogens. Subsequent to local injuries to the oral mucosa, they may penetrate the deep tissues and be responsible for suppurative or granulomatous infections. We herein report a case of a 65-year-old man who underwent surgery followed by chemotherapy and radiotherapy for a tonsillar carcinoma. An ulcerous lesion in the base of the tongue developed and spread to the carotid

A cephalometric analysis of patients with recessive dystrophic epidermolysis bullosa.

Patients diagnosed with Recessive Dystrophic Epidermolysis Bullosa (RDEB) suffer from severe growth inhibition due to reduced food intake as a result of severe oropharyngeal and esophageal blistering. This investigation examined the patterns of facial growth in a group of 42 children with RDEB for whom lateral skull radiographs were available. The differences between RDEB patients and patients with normal cephalometric values were also assessed. Lateral skull radiographs were digitized and a number of orthodontic indices were compared to the published normal values. The RDEB patients examined

Myositis ossificans traumatica of the temporal muscle: a case report.

Myositis ossificans traumatica (MOT) is a pathological condition characterized by extraskeletal formation of bony tissue, induced by violent or repeated trauma. Reports of this pathology occurring in the region of the head and neck are rare, and even more so in the muscles of mastication. We present the case of patient with MOT of the temporal muscle, the etiology of which seems to be related to traumatic manipulations during dental treatment. A review of the literature is presented and the surgical approach, which resolved this case is discussed.

Generalized Tetanus in an Unvaccinated Adolescent: A Case Report on Successful Management in a Resource-Limited Setting.

BACKGROUND: Tetanus is a life-threatening neurological disease caused by Clostridium tetani. Among its clinical forms, cephalic tetanus is the rarest, representing only 1-3% of cases. It is typically characterized by cranial nerve involvement, rapid progression to generalized tetanus, and a high risk of mortality. CASE PRESENTATION: We describe the case of a 17-year-old male with no prior tetanus vaccination who presented with trismus, painful muscle spasms, and opisthotonos following a traumatic head injury. Laboratory findings showed leukocytosis, electrolyte disturbances, and raised inflam…

RESULTS: No significant differences in postoperative VAS pain, swelling, and mouth opening (trismus) measures were observed between the two groups. However, the chronotherapy group had reduced overall postoperative complications compared to the control group. Advanced therapeutic strategies and drug delivery approaches for management of oral submucosal fibrosis: a comprehensive review.

Oral Submucous Fibrosis (OSF) is a chronic, progressive, and potentially malignant oral mucosal disorder that leads to development of trismus, burning sensations, mucosal blanching, and impaired oral function. Traditional treatments of OSF, including corticosteroids, hyaluronidase, antioxidants, and surgery, are limited due to poor drug delivery, systemic toxicity, and patient non-compliance. In this current scenario, the development of an advanced drug delivery system can be a potential strategy for OSF therapy, and emerging drug delivery strategies offer promising solutions by enabling local…

Comparative effects of hydrogen peroxide (Nanosil) and chlorhexidine mouthwashes on healing and pain after impacted mandibular third molar surgery: a randomized clinical trial.

OBJECTIVE: This study compares the effects of Nanosil® mouthwash, a multi-component formulation containing hydrogen peroxide, silver ions, eugenol, and menthol and chlorhexidine on healing, pain, and complications following impacted mandibular third molar surgery. Impacted mandibular third molars extraction, often accompanied by complications like pain, dry socket, swelling, trismus, and infection. Mouthwashes are used to reduce infection risk and promote healing by eliminating oral microorganisms. METHODOLOGY: This double-blind, randomized clinical trial involved 62 patients (aged 18-50 year…

Dental pulp stem cells as a novel antifibrotic therapy for oral submucous fibrosis: An in vitro study.

BACKGROUND: Oral submucous fibrosis (OSMF) is a chronic, potentially malignant disorder associated with areca nut consumption. It is characterized by progressive fibrosis, trismus, and a significant risk of malignancy, with limited treatment options primarily offering symptomatic relief. Dental pulp stem cells (DPSCs), a type of mesenchymal stem cells (MSCs), have shown potential for modulating fibrotic conditions through their immunomodulatory and regenerative properties. This study evaluates the antifibrotic potential of DPSCs on OSMF fibroblasts in an in vitro model. METHODS: DPSCs were is…

Intranasal Dexmedetomidine on Post-operative Pain and Swelling after Third Molar Impaction Surgery - A Comparative Study.

INTRODUCTION: Surgical removal of impacted mandibular third molars often results in post-operative discomfort, including pain, swelling and trismus. Dexmedetomidine, an alpha-2 adrenergic agonist, offers analgesic and sedative benefits with minimal respiratory depression. Its intranasal (IN) use in oral surgery remains underexplored. This study evaluates the efficacy and safety of intranasal dexmedetomidine (IN DEX) in managing post-operative pain, swelling, sedation and haemodynamic parameters in third molar surgery. MATERIALS AND METHODS: A randomised, double-blind, split-mouth controlled t…
