Human African trypanosomiasis, or sleeping sickness, is a parasitic disease endemic in sub-Saharan Africa, transmitted to humans through the bite of a tsetse fly. The first or hemolymphatic stage of the disease is associated with presence of parasites in the bloodstream, lymphatic system, and body tissues. If patients are left untreated, parasites cross the blood-brain barrier and invade the cerebrospinal fluid and the brain parenchyma, giving rise to the second or meningoencephalitic stage. Stage determination is a crucial step in guiding the choice of treatment, as drugs used for S2 are pote…

Feeding host is an important factor upon which depend the Glossina infection rate and the proportion of different species of trypanosome. Glossina feed both upon animals and humans. In order to identify species of trypanosomes present in the Komo-Mondah focus and to verify whether there is any relationship between the prevalence of sleeping sickness and the feeding habits of Glossina, we have carried out an entomological survey in this focus of Gabon. Flies were dissected and organs were analysed by PCR, while the origin of blood meals was determined by ELISA. Three species of trypanosomes wer

BACKGROUND: In Human African Trypanosomiasis, neurological symptoms dominate and cardiac involvement has been suggested. Because of increasing resistance to the available drugs for HAT, new compounds are desperately needed. Evaluation of cardiotoxicity is one parameter of drug safety, but without knowledge of the baseline heart involvement in HAT, cardiologic findings and drug-induced alterations will be difficult to interpret. The aims of the study were to assess the frequency and characteristics of electrocardiographic findings in the first stage of HAT, to compare these findings to those of…

OBJECTIVES: In human African trypanosomiasis (HAT, sleeping sickness), staging of disease and treatment follow-up relies on white cell count in the cerebrospinal fluid (CSF). As B lymphocytes (CD19 positive cells) are not found in the CSF of healthy individuals but occur in neurological disorders such as multiple sclerosis, B lymphocyte count may be useful for field diagnosis/staging and therapeutic follow-up in HAT.
METHODS: Seventy-one HAT patients were diagnosed and 50 were followed-up 6-24 months after treatment. White cell counts were used for conventional staging (stage 1, < or =5 cells…

BACKGROUND: Although the pharmaceutical industry's "neglect" of neglected tropical diseases (NTDs) has been investigated, no study evaluating media coverage of NTDs has been published. Poor media coverage exacerbates the neglect. This study aimed to investigate, describe, and analyse international media coverage of "neglected diseases" in general and three specific NTDs--African trypanosomiasis, leishmaniasis, and Chagas disease--from 1 January 2003 to 1 June 2007.
METHODS: Archives of 11 leading international, English-language media were searched. A content analysis was done, coding for medi…

The accurate identification of the causative organisms of disease is fundamental to the study of epidemiology. Hence molecular tools are now widely used to detect and distinguish pathogens, and have greatly improved our understanding of epidemiology. David Godfrey pioneered the use of molecular markers in the epidemiology of African trypanosomiasis, thus enabling the light of reliable evidence to shine on this previously problematic and controversial subject area. From the early 1970's David's group employed first isoenzyme electrophoresis and subsequently DNA-based characterization methods to…

Abstract. Because human and animal cases of African trypanosomiasis have been reported in and around the city of Kinshasa for a long time, the likelihood of local transmission was examined. A georeferenced image of the city was produced, based on a satellite image (SPOT 4). Urban, peri-urban and rural areas were delineated. All recent data on captures of Glossina fuscipes quanzensis Pires (Diptera: Glossinidae) between 1999 and 2004, as well as epidemiological data on a 1999 outbreak of human trypanosomiasis by Trypanosoma brucei gambiense in the Kisenso District, were entered in a geographica…

A study using a structured questionnaire was conducted in Busia District, Western Kenya and Kwale District, Coastal Kenya to obtain qualitative and quantitative information from 256 cattle owners about their production systems, their perceptions of the diseases encountered in their cattle, the drugs used, and other measures adopted to control trypanosomiasis in cattle. The predominant production system was mixed crop-livestock with farmers owning 2-11 local cattle on holdings between 2 and 5 ha. Approximately 15% of disease episodes in cattle were perceived to be trypanosomiasis, although the …

Human African trypanosomosis (HAT) remains a major public health problem in Subsaharan Africa. The region around the town of Bonon in middle western Côte d'Ivoire is a highly endemic HAT zone. The purpose of this study was to assess the role of travelling of infected patients in transmission of HAT. The study population included a total of 96 patients in whom HAT had been diagnosed actively or passively between 1999 and 2000. Information on each patient's residence and workplaces, i.e. water site, and farm field, was used to calculate the mean distance traveled and mean number of places visite

Human African trypanosomiasis is an essentially rural disease. Occurrence in urban areas is uncommon except in cities that reproduce rural conditions conducive to the survival of glossinidae, i.e., forest and water. This is the case in neighborhoods near the zoo in Brazzaville, People's Republic of the Congo and in the residual mangrove forest in Conakry, Guinea. In Kinshasa, Democratic Republic of the Congo, an average of 39 cases were reported annually from 1970 to 1995. This figure increased to 254 in 1996 and 226 in 1997. This sharp rise led authorities to organize screening operations in …

During a mass screening of sleeping sickness conducted in 1998 and 1999, and involving 27,932 persons in Cameroon and the Central African Republic, we tested the polymerase chain reaction (PCR) on whole blood for the diagnosis of human African trypanosomiasis due to Trypanosoma brucei gambiense. The 1858 samples obtained were from 4 groups: 155 infected patients, 1432 serological suspects detected by the card agglutination test for trypanosomiasis (CATT), 222 negative controls living in the prospected area (negative with the CATT and parasitological methods), and 49 negative controls (CATT and…

A multidisciplinary work was undertaken in the agropastoral zone of Sidéradougou, Burkina Faso to try to elucidate the key factors determining the presence of tsetse flies. In this study the PCR was used to characterize trypanosomes infecting the vector (Glossina tachinoides and Glossina palpalis gambiensis) and the host, i.e. cattle. A 2-year survey involved dissecting 2211 tsetse of the two Glossina species. A total of 298 parasitologically infected tsetse were analysed by PCR. Trypanosoma vivax was the most frequently identified trypanosome followed by the savannah type of T. congolense and

Two acute phase proteins; C-reactive protein and acid alpha 1-glycoprotein and three nutritional markers; prealbumin, retinol binding protein and transferrin have been evaluated in 8 patients suffering from trypanosomiasis in meningoencephalitic state and compared to those obtained from 15 normal control subjects. Findings show a markly decrease of nutritional markers without change of sera acute phase proteins. We concluded that in meningo-encephalitic state of human african trypanosomiasis, denutrition was a major biological or clinical feature in association with lymphoid cells stimulation …

The results of a novel direct serological card agglutination test for the diagnosis of camel trypanosomosis due to Trypanosoma evansi (CATT/T. evansi) were compared with those obtained by direct detection of parasites in a study using 1,093 sera from camels raised in northern Mali. A good correlation was revealed between the percentage of positive results obtained by CATT and the presence of trypanosomes (89%), as well as a good coincidence between the percentage of positive results obtained by CATT and low haematocrit values (packed cell volume). CATT revealed a global serological prevalence

This work aims at contributing to the knowledge of trypanosomiasis epidemiology in calves of trypanotolerant breeds and at defining an appropriate treatment to improve the survival of such calves in a tsetse infested area. The first study was a parasitological survey of 100 calves from the day of birth to the age of one year. According to the results of this survey, the period from birth to three months is a "critical" moment in the life of the calves, due to a high infection rate and mortality related to trypanosomiasis. The purpose of the second study was to investigate the possible interfer…

The chemotherapy of CNS-trypanosomiasis with DL-alpha-difluoromethylornithine (Eflornithine, DFMO) with the diamidine, pentamidine (Lomidine) or diminazene aceturate (Berenil) was examined using the mouse model for CNS-trypanosomiasis. Although combined DFMO/pentamidine therapy can give complete cures, one or more doses of 100 mg/kg pentamidine was required with continuous administration of DFMO for 14-16 days, and even this treatment failed to cure the mice in a repeat experiment. Diminazene aceturate was more efficient and provided a 40 mg-kg dose was utilised, it was possible to reduce the …

The focus of African human trypanosomiasis at Douala was evaluated in 1973 for the last time. The authors report on the results of their prospective survey. The methodology was characterized by utilizing Testryp C.A.T.T. as immunological case finding field test. 11,614 people were surveyed and 5 positive CATT revealed. But such results have be weighted by the rate of people present (52.2%) and by the fact that the selected area was not fully covered. On the other side, the authors examine the operational feasibility of Testryp CATT as immunological case finding field test. Over almost 12,000 t…
