Angina bullosa hemorrhagica: a case report.

BACKGROUND: Angina bullosa hemorrhagica (ABH) is a rare, benign, and underdiagnosed oral mucosal condition characterized by the sudden appea … ON: This case underscores the importance of raising awareness about ABH to prevent misdiagnosis and unnecessary procedures. ABH is a self-limiting, recurrent oral lesion that can occur in atypical sites like the tongue, even without systemic risk factors. Diagnosis is primarily clinical, and recognizing its self-limiting nature is key for effective patient management.

Hydrocele of the Canal of Nuck: A Rare Etiology of Inguino-Labial Swelling in a Young Female: A Case Report.

BACKGROUND: Hydrocele of the canal of Nuck is a rare congenital condition in females that often mimics other groin masses, making accurate diagnosis challenging. Limited awareness, particularly among non-surgical clinicians, often leads to unnecessary investigations or delayed treatment. Greater recognition of this entity is crucial for timely diagnosis and appropriate management. CASE PRESENTATION: We report the case of a 6-year-old girl who presented with a painless swelling in the right inguinal region, persisting for six months. On examination, a firm, non-tender mass measuring 3 × 3 cm w

Antibiotic prophylaxis in clean and clean-contaminated plastic surgery: A critical review.

Antibiotic prophylaxis is frequently used in plastic surgery procedures, despite the generally low rates of infection and few guidelines on the practice. The increasing prevalence of bacterial resistance to antibiotics calls for a reduction in unnecessary antibiotic use. The aim of this review was to create an updated summary of the available data on the effectiveness of antibiotic prophylaxis in reducing the postoperative infection in clean and clean-contaminated plastic surgery. A systematic literature search was performed on the databases Medline, Web of Science, and Scopus, limited to arti

[ELDERLY HOSPITALIZED FEBRILE PATIENTS WITH A SUSPECTED URINARY TRACT INFECTION: DIAGNOSTIC AND THERAPEUTIC APPROACH].

Since the urinary tract is thought to be one of the common sources of fever in hospitalized geriatric patients, urine analysis and urine cultures are routinely ordered in patients with and without urinary tract symptoms. The widespread lack of understanding of the uncertainties in the diagnosis and treatment of a symptomatic urinary tract infection (UTI) leads to unnecessary laboratory testing, and inappropriate antibiotic therapy. We present evidence for the following proposal that on the one hand will limit urine cultures and unnecessary antibiotic therapy without compromising patient safety

Incidental findings at MRI-enterography in patients with suspected or known Crohn's disease.

AIM: To determine the frequency and clinical impact of incidental findings detected with magnetic resonance imaging (MRI)-enterography in patients with suspected or known Crohn's disease (CD). METHODS: Incidental findings were defined as unexpected lesions outside the small intestine, not previously known or suspected at the time of referral, and not related to inflammatory bowel disease. Through a systematic review of medical charts we analyzed the clinical impact of incidental findings, and compared the MRI findings with subsequent diagnostic procedures. RESULTS: A total of 283 patients we

Unresolved legal and ethical issues in research of adults with severe traumatic brain injury: analysis of an ongoing protocol.

This paper synthesizes federal and state laws and bioethics literature with observations from an ongoing research protocol to identify, define, and clarify the unresolved legal and ethical issues regarding research involving adults with traumatic brain injury (TBI). Solutions that protect rights and minimize unnecessary impediments to valuable clinical and scientific inquiry are also illustrated using the same protocol. Research was performed at intensive care, inpatient rehabilitation, and long-term acute chronic hospitals. Our research protocol identified five areas of law impacting adults w

The impact of concurrent Hashimoto thyroiditis on thyroid nodule cytopathology assessed by ultrasound-guided fine-needle aspiration cytology.

Objective: Thyroid nodules are highly prevalent and a common clinical problem worldwide. How to identify the nature of a nodule is a major concern of clinicians. Fine needle aspiration cytology (FNAC) has an established role and is well-utilized in nodule management. However, the unsatisfactory nondiagnostic and indeterminate rates limit its usage and lead to some unnecessary surgery. Hashimoto thyroiditis (HT) is prevalently found concurrent with thyroid nodules. Whether HT can influence the accuracy of cytopathological diagnosis of nodules is still controversial. Methods: We collected medica

Group B streptococcal transmission rates as determined by PCR.

Background Group B Streptococcus (GBS) is a common cause of neonatal sepsis. GBS colonization of the newborn gastrointestinal tract (GIT) may be a critical precursor for late-onset infection. Assessment of the rate of neonatal GBS intestinal colonization has generally relied upon culture-based methods. We used polymerase chain reaction (PCR) and culture to determine the rate of GBS transmission to neonates. We hypothesized that PCR may enhance the detection of neonatal GBS colonization of the GIT, and that the rate will be higher when evaluated with PCR as compared to culture. Methods This was

An unusual cause of a large fibrinous pericardial effusion.

The commonest cause of a large fibrinous pericardial effusion in sub-Saharan Africa is tuberculosis. There are, however, limited resources available for making a definitive diagnosis of tuberculous pericarditis. The diagnosis is largely based on clinical criteria. There is a risk of misdiagnosing less-common causes of large fibrinous pericardial effusions. We present a patient who had a pericardial angiosarcoma that was initially thought to be a tuberculous pericardial effusion, and discuss the challenges in making a definitive diagnosis of tuberculosis.

Drivers of unnecessary diagnostic imaging for uncomplicated low back pain among family physicians: a theory-informed qualitative study.

BACKGROUND: Most major society guidelines recommend against diagnostic imaging for uncomplicated low back pain (LBP), referring to LBP that is not accompanied by symptoms that suggest a serious diagnosis, as it does not benefit patients. Despite this, diagnostic imaging for uncomplicated LBP remains common and contributes to overdiagnosis, overtreatment, and increased healthcare costs. AIM: The purpose of this qualitative study was to use a framework to rigorously understand the factors influencing family physicians' decisions to order diagnostic imaging for uncomplicated LBP in our context.

Reducing Unnecessary Implantable Cardioverter-Defibrillator Therapy With ENHANCED Programming: Long-Term Outcomes of the ENHANCED-ICD Study.

BACKGROUND: Advances in implantable cardioverter-defibrillator (ICD) programming strategies have achieved significant reductions in inappropriate shocks. However, further refinement is needed to minimize appropriate but unnecessary therapies. The ENHANCED-ICD study initially demonstrated the short-term safety and efficacy of programming a number of intervals to detect (NID) of 60/80 over a median follow-up of 1.3 years. A decade later, this study presents the long-term impacts of this programming strategy. OBJECTIVE: To assess the long-term impact of programming NID 60/80 for ventricular tach

Simulation-based training reduces unnecessary ultrasound referrals for developmental dysplasia of the hip in newborns.

BACKGROUND: Developmental dysplasia of the hip is a common newborn condition characterized by malformation or instability of the hip joint. Identifying dysplasia of the hip early is imperative for avoiding long-term consequences. In the United States, dysplasia of the hip screening of newborns uses the Ortolani and Barlow maneuvers. However, evidence suggests that inadequate training in these techniques may lead to unnecessary imaging referrals for further assessment. We investigated the impact of a dysplasia of the hip simulator (MiHip) and its associated training program on ultrasound referr

The Overall Poor Specificity of MRCP in the Preoperative Evaluation of the Jaundiced Patient Will Increase the Incidence of Nontherapeutic ERCP.

Laparoscopic cholecystectomy remains one of the most common surgical operations. Common bile duct stones (CBDS) are estimated to be present in 10%-20% of individuals with symptomatic gallstones. Preoperative magnetic resonance cholangiopancreatography (MRCP) and intraoperative cholangiography (IOC) remain the most common methods of evaluation, with subsequent endoscopic retrograde cholangiopancreatography (ERCP) for stone extraction if positive for CBDS. We examined our experience with preoperative MRCP versus IOC for the management of the jaundiced patient with cholelithiasis. This is a retro

Thoracolumbar Evaluation in the Low-Risk Trauma Patient: A Pilot Study Towards Development of a Clinical Decision Rule to Avoid Unnecessary Imaging in the Emergency Department.

BACKGROUND: Thoracolumbar (TL) injury is a common finding in the severely injured multi-trauma patient. However, the incidence and pattern of TL injury in patients with milder trauma is unclear. OBJECTIVE: The aim of this study was to collect and analyze evidence for the development of a clinical decision rule (CDR) to evaluate the TL spine in patients with non-severe blunt trauma and avoid dedicated imaging in low-risk cases. METHODS: Adult patients with blunt trauma who presented to a major academic center (May 2016 to October 2017) and received dedicated imaging of the TL spine were inclu

Are postoperative standing radiographs relevant before hospital discharge in adolescent idiopathic scoliosis?

PURPOSE: Postoperative standing radiographs are usually performed before hospital discharge after AIS fusion. However, patients are often still painful and have not recovered yet their physiological balance. The aim of this study was therefore to evaluate the relevance of such early radiographs and more specifically investigate whether postoperative alignment could be analyzed. METHODS: All consecutive AIS patients operated between January 2015 and December 2015 were included. All patients underwent biplanar stereoradiographs before hospital discharge, at 4 months postoperative and at last fo
