Hypogeusia is an uncommon complication of uvulopalatopharyngoplasty (UPPP) and few reports in the literature have documented it. Excision of part of the soft palate and damage to the glossopharyngeal nerve or its lingual branch as a result of diathermy or surgery are the possible causes. The case of a 45-year-old male who suffered hypogeusia following UPPP is reported herein. He reported no improvement over a 6-month period. Surgical intervention and treatment of OSAHS and snoring has a considerable history, encompassing a number of operations all of which have the intention of reducing or by-passing the pharyngeal resistance that occurs during sleep. Review of the published literature presents some problems, however, due to the ethical difficulties of undertaking randomised controlled trials in surgery. Uncontrolled trials are less satisfactory due to the large 'regression to the mean' and placebo effects. However, there is a considerable body of literature available relating to surgical intervention.

Obstructive sleep apnea syndrome (OSAS) is characterized by snoring and apnea during sleep leading to decreased oxygen saturation and disturbed sleep, excessive daytime sleepiness and neuropsychological disturbances. Laser-assisted uvulopalatoplasty (LAUP) is an outpatient surgical procedure which is in use as a treatment for snoring. LAUP also has been used as a treatment for sleep-related breathing disorders, including obstructive sleep apnea. The Standards of Practice Committee of the American Academy of Sleep Medicine reviewed the available literature, and developed these practice parameters as a guide to the appropriate use of this surgery. Obstructive sleep apnoea is a frequently occurring disease that can have important consequences including disabling hypersomnolence and sleepiness as well as cardiovascular diseases like hypertension. Treatment modalities are, however, limited. The efficacy of nasal continuous positive airway pressure (CPAP) stands out, but not all patients can tolerate this treatment or be compliant with it. Efficacious alternative therapies are still remarkably few in number. The actual therapy of choice for the common obstructive sleep-apnea syndrome is the application of continuous positive airway pressure (CPAP) via a nose-mask. We present results of uvulopalatoplasty (UPP) in 22 patients with obstructive sleep apnea who had preoperative and postoperative polysomnograms. Symptomatic improvement in daytime somnolence, snoring, and quality of sleep was reported by 20 patients. After operation, ten of 22 patients had more than a 50% improvement in the apnea index and apnea/sleep percentage, and the number of apneas, apnea index, apnea/sleep percentage, apneic duration, lowest oxygen saturation and longest apneas were significantly better (P less than .05).
