The use of propafenone in the treatment of tachyarrhythmias in children.

Propafenone was given to 60 children (mean age 4.5 years) with paroxysmal re-entrant supraventricular tachycardia (rSVT: 41 cases), postoperative automatic junctional tachycardia (JET: eight cases), automatic atrial tachycardia (AT:four cases), ventricular tachycardia (VT:four cases) and atrial flutter (AF: three cases). In acute cases (29) propafenone was administered intravenously (mean dose 1.3 +/- 0.5 mg.kg-1) chronic oral treatment (mean dose of either 11 +/- 3.3 mg.kg.day-1 or 265 +/- 78 mg.m-2 x day-1) was given to 48 children, in 2-3 divided doses. Overall efficacy was 76% for acute an

Use of a dual chamber pacemaker with a novel fallback algorithm as an effective treatment for sick sinus syndrome associated with transient supraventricular tachyarrhythmia.

A dual chamber pacemaker having a fallback (FB) mode was utilized in nine patients with sick sinus syndrome (SSS) associated with transient supraventricular tachyarrhythmia (atrial fibrillation [Af] in three patients, atrial flutter [AF] in two, and paroxysmal atrial tachyarrhythmia [PAT] in four). Various degrees of abnormality of atrioventricular (AV) conduction were observed at the time of pacemaker implantation in five patients. In this pacemaker, a DDD mode was active during sinus rhythm, but the mode changed, through a given cycle of high rate tracking at an upper rate limit (URL), to VD

Bipolar transseptal radiofrequency ablation of AV node: an alternative to high energy shocks?

Atrioventricular (AV) node ablation to control ventricular response was attempted in two patients with recurrent drug refractory atrial flutter. Standard radiofrequency (RF) procedure, which delivers energy through the 4-mm tip electrode of an ablation catheter positioned at the right AV junction and a large back plate (unipolar mode), failed in both patients. As an accepted second step, high energy direct current (DC) ablation under general anesthesia was then performed in patient one. After two shocks of 200 J, complete AV block occurred, but complete recovery was noted 3 hours later. In a t

Cardiac dysrhythmia following pneumonectomy. Clinical correlates and prognostic significance.

Cardiac tachydysrhythmias occurred in 53 (22 percent) of 236 consecutive patients undergoing pneumonectomy. All patients had preoperative electrocardiograms which showed normal sinus rhythm. Patients did not receive digitalis before surgery. Atrial fibrillation was the most common dysrhythmia (64 percent; 34/53), followed by supraventricular tachycardia (23 percent; 12/53) and atrial flutter (13 percent; 7/53). No episodes of ventricular tachycardia were documented. Elevated concentrations of cardiac enzymes were associated with 12 (28 percent) of 43 tachydysrhythmias. Recurrent or persistent

Findings on ambulatory electrocardiographic monitoring in subjects older than 80 years.

Twenty-four-hour electrocardiograms were recorded in 50 subjects (44 women, 6 men) older than 80 years without cardiovascular disease and with normal standard electrocardiographic responses. During waking and sleeping periods, the mean sinus rates were, respectively, 78 +/- 3 and 64 +/- 1 beats/min; heart rate ranged from 43 to 180 beats/min over 24 hours. Supraventricular tachycardia (SVT) was present in 28% of the subjects. Nocturnal sinus arrhythmia was only noted in 12% of the patients; it was accompanied by sinus pauses of 1.8 to 2 seconds, and 1 woman had a transient pattern compatible w

Wolff-Parkinson-White syndrome and supraventricular tachycardia during infancy: management and follow-up.

The records of 90 patients with Wolff-Parkinson-White syndrome who presented with supraventricular tachycardia in the first 4 months of life were reviewed. Among these, 63% were male. Structural heart disease was present in 20%, most commonly Ebstein's anomaly. All patients presented with a regular narrow QRS tachycardia, and pre-excitation became evident only when normal sinus rhythm was established. Only one infant had atrial flutter and none had atrial fibrillation. Type A Wolff-Parkinson-White syndrome was most common (49%), with heart disease occurring in only 5% of these patients. In con

[Verapamil in the study and treatment of supraventricular tachycardias, with special reference to pre-excitation].

Verapamil is a novel antiarrhythmic agent which appears to act as a calcium-ion antagonist, blocking calcium transport across the myocardial cell membrane. It was given intravenously, in a dose of 10 mg, to thirty-two patients suffering from paroxysmal supraventricular tachycardia, and sinus rhythm was achieved promptly in all. Identical results were obtained in a further ten patients with supraventricular tachycardias associated with the Wolff-Parkinson-White or other pre-excitation syndromes. In a separate group of eighteen patients in whom A-V junctional tachycardias were induced during int

Pulsed Field Ablation of Persistent Atrial Fibrillation With Continuous Electrocardiographic Monitoring Follow-Up: ADVANTAGE AF Phase 2.

BACKGROUND: There are sparse high-quality safety and effectiveness data for pulsed field ablation (PFA) of persistent atrial fibrillation (PerAF), in which lesions beyond pulmonary vein isolation (PVI) are often placed. In addition, no large trials have used insertable cardiac monitors for continuous rhythm monitoring after ablation in patients with PerAF or after PFA in any atrial fibrillation population. In Phase 2 of the ADVANTAGE AF study (A Prospective Single Arm Open Label Study of the FARAPULSE Pulsed Field Ablation System in Subjects With Persistent Atrial Fibrillation), patients with

Minimally invasive hybrid ablation procedure for the treatment of persistent atrial fibrillation: one year results.

BACKGROUND: The concept of a hybrid approach, combining the most effective techniques of surgical and endocardial catheter ablation has resulted in the creation of the convergent ablation procedure. This novel, pericardioscopic, hybrid approach can be an effective option for highly symptomatic patients with persistent atrial fibrillation (PSAF) and longstanding persistent atrial fibrillation (LSPAF) for whom standalone surgical or endocardial ablation procedures offer sometimes unsatisfactory outcomes. AIM: To assess the safety, efficacy and effectiveness of a hybrid epicardial and endocardia

Ultrasound-guided stellate ganglion block for refractory sympathetically mediated arrhythmia following acute myocardial infarction: a tutorial and examples.

INTRODUCTION: Sympathetic arrhythmia represents a life-threatening complication following acute myocardial infarction (AMI), which is characterized by recurrent episodes of electrical dysrhythmias that are often refractory to antiarrhythmic medications and thereby necessitate repeated electrical cardioversion or defibrillation. This case series aims to evaluate the efficacy and safety of ultrasound-guided stellate ganglion block (SGB) as a rescue therapy for refractory ventricular arrhythmias and one case of rapid atrial flutter following AMI. METHODS: We retrospectively analyzed five AMI pat

Outcome of catheter ablation in the very elderly-insights from a large matched analysis.

BACKGROUND: Ablation emerged as first line therapy in the treatment of various arrhythmias. Nevertheless, in older patients (pts), decision is often made pro drug treatment as more complications and less benefit are suspected. HYPOTHESIS: We hypothesized that different kind of ablations can be performed safely regardless of the pts age. METHODS: We enrolled all pts aged >80 years (yrs) who underwent ablation for three different arrhythmias (atrial flutter [AFL], atrioventricular nodal re-entry tachycardia [AVNRT], ventricular tachycardia [VT]) between August 2002 and December 2018. A Novel SCN5A Mutation Associated with Drug Induced Brugada Type ECG.

BACKGROUND: Class IC antiarrhythmic agents may induce acquired forms of Brugada Syndrome. We have identified a novel mutation in SCN5A, the gene that encodes the α-subunit of the human cardiac sodium channel (hNav1.5), in a patient who exhibited Brugada- type ECG changes during pharmacotherapy of atrial arrhythmias. OBJECTIVE: To assess whether the novel mutation p.V1328M can cause drug induced Brugada Syndrome. METHODS: Administration of pilsicainide, a class IC antiarrhythmic agent, caused Brugada- type ST elevation in a 66-year-old Japanese male who presented with paroxysmal atrial fibril
