Atrial, ventricular, or both cannulation sites to optimize left ventricular assistance?

The efficiency of left ventricular assist devices (LVADs) depends on the capacity of the inflow cannula to drain blood into the pump. Left atrial (LA) and left ventricular (LV) sites were compared in an animal model mimicking different hemodynamic conditions. Three calves (56.3+/-5.0 kg) were equipped with a Thoratec LVAD. A regular cardiopulmonary bypass (CPB) circuit was used as a right ventricular assist device (RVAD) (jugular vein/pulmonary artery), and preload conditions were adjusted by storage (or perfusion) of blood into (or from) the venous reservoir. LA and LV drainage, tested separa

[Evaluation of endomyocardial fibrosis by two-dimensional echocardiogram. Analysis of the severity and correlation with angiocardiography].

PURPOSE: To evaluate the capacity of two-dimensional echocardiogram (2D-Echo) to establish the severity of endomyocardial fibrosis (EMF) in comparison with angiography (Angio). METHODS: Twelve patients with EMF were prospectively studied by both: 2D-Echo and Angio. The mean age was 24.6 +/- 6.6 years (14 to 35), with 8 women and 4 men. To compare the severity by the 2 methods the obliterative process was graded according to its extension relative to the long axis of the respective, in diastole, as follows: 1 + only apical: 2 + less than 50% and 3 + greater than or equal to 50% from the apex t

Special Article - Exercise-induced right ventricular injury or arrhythmogenic cardiomyopathy (ACM): The bright side and the dark side of the moon.

There is still debate on the range of normal physiologic changes of the right ventricle or ventricular (RV) function in athletes. Genetic links to arrhythmogenic cardiomyopathy (ACM) are well-established. There is no current consensus on the importance of extensive exercise and exercise-induced injury to the RV. During the intensive exercise of endurance sports, the cardiac structures adapt to athletic load over time. Some athletes develop RV cardiomyopathy possibly caused by genetic predisposition, whilst others develop arrhythmias from the RV. Endurance sports lead to increased volume and pr

Early Left Ventricular Involvement Detected by Cardiovascular Magnetic Resonance Feature Tracking in Arrhythmogenic Right Ventricular Cardiomyopathy: The Effects of Left Ventricular Late Gadolinium Enhancement and Right Ventricular Dysfunction.

Background Left ventricular (LV) involvement is common in arrhythmogenic right ventricular cardiomyopathy (ARVC). We aim to evaluate LV involvement in ARVC patients by cardiovascular magnetic resonance feature tracking. Methods and Results Sixty-eight patients with ARVC and 30 controls were prospectively enrolled. ARVC patients were divided into 2 subgroups: the preserved LV ejection fraction (LVEF) group (LVEF ≥55%, n=27) and the reduced LVEF group (LVEF <55%, n=41). Cardiovascular magnetic resonance with late gadolinium enhancement (LGE) and cardiovascular magnetic resonance feature tracking

Haemodynamics from Induction to Cardiopulmonary Bypass Improve Prediction of Right Ventricular Failure Following Left Ventricular Assist Device.

OBJECTIVES: Right ventricular failure (RVF) remains a frequent complication following left ventricular assist device (LVAD) implantation. Conventional risk scores rely on preoperative data acquired under stable conditions, which may overlook early signs of RV decompensation that emerge under perioperative stress. This study evaluates whether perioperative haemodynamic markers prior to cardiopulmonary bypass (CPB) improve RVF prediction compared to preoperative assessment obtained by right heart catheterization (RHC), echocardiographic parameters, and scoring systems such as the European Regist

Tissue-Based Predictors of Impaired Right Ventricular Strain in Coronary Artery Disease: A Multicenter Stress Perfusion Study.

BACKGROUND: Right ventricular (RV) dysfunction is known to impact prognosis, but its determinants in coronary artery disease are poorly understood. Stress cardiac magnetic resonance (CMR) has been used to assess ischemia and infarction in relation to the left ventricle (LV); the impact of myocardial tissue properties on RV function is unknown. METHODS: Vasodilator stress CMR was performed in patients with known coronary artery disease at 7 sites between May 2005 and October 2018. Myocardial infarction was identified on late gadolinium enhancement-CMR, and infarct transmurality was graded on a

Athlete's heart: the potential for multimodality imaging to address the critical remaining questions.

Moderate exercise is a powerful therapy in the treatment and prevention of cardiac disease, but intense habitual exercise leads to cardiac adaptations for which the prognostic benefits are less clear. The athlete's heart syndrome refers to the morphological and electrical remodeling which occurs to varying extents dependent upon the sporting discipline. Its accurate differentiation from pathological entities is critical. This review describes the role multi-modality imaging serves in determining the limitations and consequences of intense exercise. Tissue characterization and imaging studies d

Association of cardiovascular magnetic resonance diastolic indices with arrhythmia in repaired Tetralogy of Fallot.

BACKGROUND: Patients with repaired Tetralogy of Fallot (rTOF) experience a high burden of long-term morbidity, particularly arrhythmias. Cardiovascular magnetic resonance (CMR) is routinely used to assess ventricular characteristics but the relationship between CMR diastolic function and arrhythmia has not been evaluated. We hypothesized in rTOF, left ventricular (LV) diastolic dysfunction on CMR would correlate with arrhythmias and mortality. METHODS: Adolescents and adults with rTOF who underwent CMR were compared to healthy controls (n = 58). Standard ventricular parameters were assessed a

Prognostic Value of Right Atrial Strain in Patients With Moderate-to-Severe Tricuspid Regurgitation.

BACKGROUND: Significant tricuspid regurgitation (TR) leads to right ventricular dysfunction and associated comorbidities. As the receiving chamber for regurgitant volume, the right atrium undergoes significant mechanical remodeling. This study investigated whether right atrial reservoir strain (RArS), a key marker of atrial mechanics, serves as a prognostic indicator for mortality in patients with moderate-to-severe TR. METHODS: Clinical, laboratory, and echocardiographic data from 289 consecutive patients with TR (grade ≥+3) were analyzed. RArS was evaluated by 2-dimensional speckle-tracking

Metabolic Response to Stress by the Immature Right Ventricle Exposed to Chronic Pressure Overload.

Background The right ventricle exposed to chronic pressure overload exhibits hypertrophy and decompensates when exposed to stress. We hypothesize that impaired ability to increase myocardial oxidative flux through pyruvate dehydrogenase leads to hypertrophied right ventricular (RV) dysfunction when exposed to hemodynamic stress, and pyruvate dehydrogenase stimulation can improve RV function. Methods and Results Infant male Yorkshire piglets (13.5±0.6 kg weight, n=19) were used to assess substrate fractional contribution to the citric acid cycle after sustained pulmonary artery banding (PAB). RV pressure overload: from hypertrophy to failure.

In pulmonary arterial hypertension (PAH), right ventricular (RV) adaptation is essential to overcome the chronic increases in RV pressure overload. Ultimately, RV compensatory mechanisms are not sufficient and patients succumb to RV failure. The processes underlying the transition of RV adaptation to RV failure are not well understood. In this review, we propose that important insights in RV adaptation processes can be obtained by comparing different etiologies of PAH, namely patients with PAH secondary to Eisenmenger syndrome, patients with PAH secondary to systemic sclerosis and patients whe

Heart failure etiology and risk of right heart failure in adult left ventricular assist device support: the European Registry for Patients with Mechanical Circulatory Support (EUROMACS).

Objectives: Development of right ventricular failure (RVF) after left ventricular assist device (LVAD) implantation remains a leading cause of perioperative morbidity, end-organ dysfunction and mortality. The objective of this study was to investigate whether the etiology of HF (ischemic HF versus non-ischemic HF) affects the risk of RVF within admission for LVAD implantation and during long-term follow-up. Methods: Between January 2011 and June 27, 2018, 3536 patients were prospectively enrolled into EUROMACS registry. Adult patients (>18 years) who received a first time LVAD were included. W

Frontal QRS-T angle and ventricular mechanics in congenital heart disease.

BACKGROUND: The QRS-T angle has been associated with adverse cardiovascular events and sudden cardiac deaths. We determined frontal QRS-T angle in patients with complete transposition of the great arteries (TGA) after atrial switch operation and repaired tetralogy of Fallot (TOF) and explored its relationships with ventricular mechanics. METHODS: Thirty TGA patients aged 32.3 ± 4.4 years after atrial switch operation and 47 repaired TOF patients aged 28.7 ± 6.0 years were studied. The frontal planar QRS-T angle and QRS duration were measured from 12-lead electrocardiograms. Right (RV) and lef

Modeling ventricular interaction: a multiscale approach from sarcomere mechanics to cardiovascular system hemodynamics.

Direct ventricular interaction via the interventricular septum plays an important role in ventricular hemodynamics and mechanics. A large amount of experimental data demonstrates that left and right ventricular pump mechanics influence each other and that septal geometry and motion depend on transmural pressure. We present a lumped model of ventricular mechanics consisting of three wall segments that are coupled on the basis of balance laws stating mechanical equilibrium at the intersection of the three walls. The input consists of left and right ventricular volumes and an estimate of septal w

Altered ventricular mechanics in cardiac allografts: a tissue Doppler study in 30 children without prior rejection events.

BACKGROUND: Tissue Doppler imaging (TDI), a non-invasive echocardiography technique, permits quantitative analysis of the regional distribution pattern of myocardial velocities. During normal childhood development, regional function changes markedly, including an increasing predominance of longitudinal velocities. This study analyzed the impact of heart transplantation on ventricular mechanics in growing children. METHODS: TDI was performed in 30 pediatric heart transplant recipients (7.1 +/- 6.2 years) and 32 age-matched healthy children (6.8 +/- 5.4 years). Patients had no rejection history

Correction to: Mortality Patterns in Pediatric Pulmonary Vein Stenosis: Insights Into Right Ventricular Systolic Pressure Associations.

In the article by Takajo et al, "Mortality Patterns in Pediatric Pulmonary Vein Stenosis: Insights Into Right Ventricular Systolic Pressure Associations," which published online on January 17, 2025 (J Am Heart Assoc. 2025; 2025;14:e037908. DOI: 10.1161/JAHA.124.035037) and was included in the January 21, 2025 issue of the journal, corrections were needed. Throughout the manuscript, the term "ECG" was mistakenly used instead of "echocardiographic" or "echocardiography." This error occurred during the copyediting process, where "echo" was inadvertently converted to "ECG." The specific correction

Right Heart Structure and Function in Lifelong Recreational Endurance Athletes with and without Paroxysmal Atrial Fibrillation.

BACKGROUND: Healthy young athletes adapt to the increased demands of endurance exercise with symmetric cardiac remodeling. Male veteran endurance athletes have an increased risk for atrial fibrillation (AF), and some athletes seem susceptible to changes mimicking arrhythmogenic cardiomyopathy. Intense exercise puts a disproportionate hemodynamic load on the right-sided heart chambers. Despite this, data describing right heart structure and function in older veteran athletes are scarce. The aim of this study was to investigate structural and functional characteristics of the right heart in vete
