Exposure to MRI-related magnetic fields and vertigo in MRI workers.

OBJECTIVES: Vertigo has been reported by people working around magnetic resonance imaging (MRI) scanners and was found to increase with increasing strength of scanner magnets. This suggests an association with exposure to static magnetic fields (SMF) and/or motion-induced time-varying magnetic fields (TVMF). This study assessed the association between various metrics of shift-long exposure to SMF and TVMF and self-reported vertigo among MRI workers. METHODS: We analysed 358 shifts from 234 employees at 14 MRI facilities in the Netherlands. Participants used logbooks to report vertigo experien…

Vertigo with a Vestibular Dysfunction in Children During Respiratory Tract Infections.

Sudden balance disorders with violent vegetative symptoms (nausea and vomiting) pose a diagnostic and therapeutic problem. In children vertigo/dizziness with symptoms of vestibular dysfunction is rare, but as vascular etiology is unlikely in children such symptoms arouse concern. This article presents two cases of this type of vertigo. The patients were two boys (6 and 9 years old). They came down with similar symptoms: sudden dizziness, disabled walking, nausea and vomiting, spontaneous nystagmus, and a positive Romberg test. The onset of the balance disorder was preceded by respiratory infec…

Vestibular paroxysmia in children: a treatable cause of short vertigo attacks.

Vestibular paroxysmia due to neurovascular compression is a syndrome consisting of frequent short episodes of vertigo in adults that can be easily treated. Here we describe the initial presentation and follow-up of three children (one female, 12y; two males, 8y and 9y) who experienced typical, brief, vertiginous attacks several times a day. Nystagmus was observed during the episodes. Cranial magnetic resonance imaging revealed arterial compression of the eighth cranial nerve. The attacks ceased after administration of low-dose carbamazepine (2-4mg/kg daily). Vestibular paroxysmia must be consi…

Importance of spontaneous nystagmus detection in the differential diagnosis of acute vertigo.

Vertigo is a common cause of emergency department attendance. Detection of spontaneous nystagmus may be a useful sign in distinguishing vestibular neuritis from other vestibular diagnoses. We aimed to assess the contribution of spontaneous nystagmus in the diagnosis of acute vertigo. We enrolled consecutive consenting patients arriving at a single emergency department with acute vertigo. There was no declared protocol for the emergency department staff. A standardized history and examination was conducted by the investigators. Observation for spontaneous nystagmus, its response to visual fixat…

[Vertigo and ear discomfort caused by a bone defect in os temporalis].

Superior semicircular canal dehiscence is a clinical condition where the bone tissue overlying the superior semicircular canal is absent. The symptoms can be nonspecific and include vertigo, oscillopsia and conductive hearing loss. An audiogram will reveal an air-bone-gap in the lower frequencies in an otherwise healthy ear. Diagnosis is confirmed by performing muscular evoked myogenic potentials examination and temporal bone computed tomography. A case of right side superior semicircular canal dehiscence in a 52-year-old woman and the diagnostic course is described.

[Vertigo and dizziness. Diagnostic algorithm from the perspective of emergency medicine].

Vertigo and dizziness are common symptoms in the acute care setting and have a wide diagnostic range. The most deleterious diagnosis is vertebrobasilar disease with brain infarction in the posterior fossa. Therefore, every patient with acute vestibular syndrome needs to be evaluated by a structured interview and a systematic physical examination for red flag symptoms which indicate vertebrobasilar infarction. Routinely, the physical examination should cover the head impulse (Halmagyi) test, test for nystagmus as well as the test of skew and in cases of benign paroxysmal positional vertigo, by …

The epidemiology of vertigo, dizziness, and unsteadiness and its links to co-morbidities.

Vertigo, dizziness, and unsteadiness (VDU) are common symptoms traditionally considered to result from different kinds of vestibular and non-vestibular dysfunctions. The epidemiology of each symptom and how they relate to each other and to migraine, agoraphobia, motion sickness susceptibility (MSS), vaso-vagal episodes (VVE), and anxiety-depression was the object of this population-based study in north-eastern France. A self-administered questionnaire was returned by 2987 adults (age span 18-86 years, 1471 women). The 1-year prevalence for vertigo was 48.3%, for unsteadiness 39.1%, and for diz…

Utility of head CT in the evaluation of vertigo/dizziness in the emergency department.

Acute dizziness (including vertigo) is a common reason to visit the emergency room, and imaging with head CT is often performed initially to exclude a central cause. In this study, consecutive patients presenting with dizziness and undergoing head CT were retrospectively reviewed to determine diagnostic yield. Four hundred forty-eight consecutive head CTs in a representative sample of dizzy emergency room (ER) patients, including patients with other neurological symptoms, were reviewed to identify an acute or subacute cause for acute dizziness along with the frequency and modalities used in fo…

Suppression of visually induced vertigo: effects of attentional position and modality.

INTRODUCTION: Visually induced vertigo (VIV) is a serious problem because humans rely on vision more than any other sensory system to receive information from their surrounding environment. We investigated whether attention to simultaneous visual or auditory tasks would reduce VIV. METHODS: Subjects were four women and one man who were highly susceptible to VIV and four women who had low susceptibility to VIV. An animation display was prepared to test the effect of overlapping visual task stimuli when the position of the stimuli on the screen was changed. The effect of modality was tested by …
