[Robot-assisted Minimally Invasive Surgery for Mediastinal Tumors].

The subxiphoid approach in thymectomy provides better visibility around the left brachiocephalic vein than the lateral thoracic approach. Robot-assisted thoracoscopic surgery is easier to parform than video- assisted thoracoscopic surgery for surgery of the upper mediastinum, because the forceps can be moved with many joints. Robot-assisted thymectomy using the subxiphoid approach may be less traumatic and less invasive than median sternotomy. We must continue to devise surgical procedures to make oncologically curative surgery more minimally invasive.

Video-assisted resection in benign frontal tumors.

Soft and osseous tumors that develop into the frontal are the most profitable with the use of video-assisted surgery, thus avoiding also a visible scar. In the Department of Plastic Surgery at Argerich Hospital in Buenos Aires, Argentina, from 1999 to 2010, video-assisted operations were used in the treatment of 158 patients, 26 of them presented lipomas and osteomas into the frontal tissues. In all 26 patients, both local anesthesia and incisions behind the hairline were performed. Minor complications such as hematoma and transitory paresis of the frontal nerve were detected. Video-assisted t…

Surgical Management of Thoracic Multiple Exostoses.

Hereditary multiple exostoses is a rare autosomal dominant condition resulting in the development of multiple osteochondromas. We present the case of a 25-year-old woman with hereditary multiple exostoses who was referred for thoracic surgery assessment due to severe right-sided chest pain. Computed tomographic scan allowed preoperative planning for resection of the lesion. Under general anesthesia, right video-assisted thoracoscopic surgery was performed through a cosmetic retromammary incision. Complete removal of the tumor resolved symptoms, and she was discharged 2 days later. We show that

[Surgery for primary hyperparathyroidism in 1998. Apropos of 66 patients and 3 methods of approach].

STUDY AIM: The aim of this prospective study was to assess the advantages and disadvantages of cervicotomy, selective lateral approach and video-assisted surgery in the treatment of primary hyperparathyroidism (HPT 1). PATIENTS AND METHODS: During 1998, 66 patients were operated on for HPT 1 in the same center. There were 48 women and 18 men (mean age: 58 years, range: 21-84), familial HPT 1 or MEN1 excluded. The interventions were performed via classical cervicotomy (n = 32), via selective lateral approach (n = 8) and were video-assisted (n = 25). A mediastinal adenoma was removed via cervic…

Management of thoracoabdominal gunshot injuries by using minimally invasive surgery at role 2 deployed field hospitals in Ukraine.

The Russia-Ukraine war is associated with critical and severe thoracoabdominal injuries. A more specific approach to treating patients with thoracoabdominal injury should also include minimally invasive technologies. It remains unclear about the utility of using video-assisted thoracoscopic surgery (VATS) and laparoscopy in patients with thoracoabdominal injury. The aim of this study was to investigate and evaluate the utility of video-assisted thoracoscopic surgery, laparoscopy as well as magnetic tool applications for the management of severe thoracoabdominal injury in combat patients injure

Effectiveness of PET/CT and VATS for detecting and treating internal mammary lymph node metastasis: a case series.

OBJECTIVE: We occasionally observed internal mammary lymph node metastases of breast cancer in a clinical setting. However, unlike a standard treatment in axillary metastasis, surgical resection for internal mammary lymph node metastasis is not prevalent because of unclear safety and benefits. Thus, we aimed to evaluate the diagnostic ability and clinical outcomes of positron emission tomography/computed tomography and video-assisted thoracoscopic surgery. METHODS: We retrospectively investigated 34 patients with breast cancer with abnormal 18F-fluorodeoxyglucose uptake in internal mammary ly

Tubeless uniportal VATS in thoracic surgery - Indications, ERAS pathways, and outcomes: A review.

Tubeless uniportal video-assisted thoracoscopic surgery (VATS) is an innovative approach characterized by the use of non-intubated (spontaneous-breathing) anesthesia, the omission of routine postoperative chest drainage, and single-port access. This technique has gained traction in recent years for a variety of thoracic procedures. While practices reported in the literature may differ, this review primarily examines the combined non-intubated and drainless approach. This narrative review provides a comprehensive overview and critical analysis of its current clinical applications, including sym

Primary Low-Grade Fibromyxoid Sarcoma of the Left Upper Lobe Treated With Uniportal Video-Assisted Thoracoscopic Lobectomy.

Low-grade fibromyxoid sarcoma (LGFMS) is a rare tumor that usually affects young adults and arises on the proximal extremities and trunk. Primary pulmonary LGFMS is an extremely rare entity. We reported a postsurgical diagnosis of a primary lung LGFMS in a patient with single pulmonary nodule located in the left upper lobe. A uniportal thoracoscopic lobectomy with mediastinal lymph node dissection was performed. The histopathological report supposed the hypothesis of a primary lung LGFMS. The immunohistochemical investigation yielded intense and diffuse positivity of the neoplastic cells for M

Scaling up self-supervised learning for improved surgical foundation models.

Foundation models have revolutionized computer vision by achieving vastly superior performance across diverse tasks through large-scale pretraining on extensive datasets. However, their application in surgical computer vision has been limited. This study addresses this gap by introducing SurgeNetXL, a novel surgical foundation model that sets a new benchmark in surgical computer vision. Trained on the largest reported surgical dataset to date, comprising over 4.7 million video frames, SurgeNetXL achieves consistent top-tier performance across six datasets spanning four surgical procedures and

Expert Consensus on Subxiphoid and Subcostal Arch Thoracoscopic Resection for the Treatment of Thymoma.

Surgery is the primary treatment for thymoma. Although subxiphoid and subcostal arch thoracoscopic thymoma surgery is widely used, there is currently a lack of consensus regarding its use, nor have standards been established. Based on the surgical experience of many domestic thoracic surgery centers, the Department of Thoracic Surgery of Tangdu Hospital of Air Force Medical University has formulated this expert consensus regarding key clinical issues related to thoracoscopic thymoma surgery, including preoperative evaluation, surgical indications, preoperative preparation, surgical details, pe

A meta-analysis of efficacy and safety of parietal pleurectomy versus pleural abrasion in treating spontaneous pneumothorax.

BACKGROUND: Spontaneous pneumothorax is a common benign disease. Its treatment aims to restore the collapsed lungs, relieve symptoms, and prevent recurrence. Currently, video-assisted thoracoscopic bullectomy combined with parietal pleural fixation has become the mainstream treatment method. However, the choice between parietal pleurectomy and pleural abrasion remains a subject of continuous debate. Consequently, we conducted a meta-analysis to assess the differences in perioperative efficacy and complications between the two surgical methods combined with bullectomy. METHODS: Relevant literat

Thoracoscopic S1 segmentectomy, right upper lobe: alternative posterior approach.

Minimally invasive pulmonary segmentectomy allows adequate oncological treatment in selected cases while preserving lung parenchyma and minimizing perioperative morbidity and length of hospital stay. Although several variations of minimally invasive pulmonary segmentectomy have been described, a fully thoracoscopic multiport approach that allows direct access to the segmental structures, is straightforward and is versatile enough to allow adaptation in case of unexpected intraoperative findings (such as conversion to lobectomy in the case of positive margins) is preferable. The S1 (apical) seg

A Novel Technique to Optimize Uniportal Thoracoscopic Right Middle Lobectomy.

The study aimed to describe a novel port design and simple surgical strategy for uniportal thoracoscopic right middle lobectomy. In this approach, a 3- to 4-cm incision was created at the sixth intercostal space posterior to the scapular line. The surgery was performed by serial division of the anterior oblique fissure, vein, bronchus, artery, and horizontal fissure. Based on our preliminary experience, this approach could provide an appropriate direction and angle for dissection and stapling, solving the challenge of conventional uniportal right middle lobectomy.

Video-Assisted Subcutaneous Destruction of the Sinus Tract with Vessel-Loop Drainage as Minimally-Invasive Surgical Treatment for Pilonidal Sinus Disease.

The video-assisted subcutaneous destruction of the sinus tract (VADST) is a novel, minimally-invasive technique for the treatment of pilonidal sinus disease (PSD). This is an advancement of the previously described subcutaneous destruction of the sinus tract and the removal of hairs as well as the long-term vessel-loop drainage of the wound channel (DST). Although the first results of this operation seemed to be promising, some of my colleagues felt that the blind approach to this procedure could be its potential "weak point". As a result of this critique, the procedure was enhanced with the s

Surgical approaches to the thymus in patients with myasthenia gravis.

Myasthenia gravis is an autoimmune disorder of neuromuscular transmission affecting 2 out of every 100,000 people. Neurologists and surgeons still debate what role surgery should play in its management. Many patients who might benefit from thymectomy are denied the opportunity because of misconceptions, ignorance, or trepidation. By offering effective methods of less invasive thymectomy to these patients, a significant number of patients and treating neurologists previously unwilling to consider surgery may realize the benefits of this established, proven treatment alternative. The surgical ap

Robot-Assisted Uterus-Sparing Repair of a Vesicouterine Fistula 26 Years After Cesarean Section: A Case Report and Review of Surgical Techniques.

INTRODUCTION AND HYPOTHESIS: This video illustrates the surgical repair of a vesicouterine fistula (VUF) via a transperitoneal, robot-assisted, uterus-sparing approach. METHODS: In this video, we present a VUF that occurred after a cesarean section. The patient presented 22 years later with persistent hematuria. She underwent a uterus-sparing, robot-assisted repair via excision of the fistula tract. The uterus and bladder were closed in multiple layers with omental interposition. RESULTS: The patient tolerated the procedure well. A follow-up cystography confirmed the absence of defects or fi
