BACKGROUND: Immune dysregulation plays a pivotal role in the pathogenesis and progression of Parkinson's disease (PD). Increasing evidence suggests that immunotherapies targeting α-synuclein (α-syn) pathology may offer potential disease-modifying strategies for PD.
OBJECTIVE: This review aims to systematically summarize recent advances in α-syn-targeted immunotherapies for PD and to evaluate the translational challenges and future directions of precision immunotherapy.
METHODS: We critically reviewed the preclinical evidence and clinical trial outcomes of both active immunization strategies,

Parkinson's disease (PD) is a prevalent neurodegenerative movement disorder characterized by bradykinesia, rigidity, and resting tremor, progressing insidiously over time. Central to its pathophysiology is the degeneration of dopaminergic neurons in the substantia nigra, leading to a significant decrease in striatal dopamine (DA) levels. This dopaminergic deficit disrupts basal ganglia circuitry, impairing motor function and contributing to the core symptoms of the disease. While the etiology of PD remains incompletely understood, a combination of genetic predispositions and environmental expo

Multiple system atrophy is a rare, rapidly progressive neurodegenerative disorder within the spectrum of atypical parkinsonian syndromes. It is clinically defined by the combination of parkinsonism, cerebellar ataxia, and autonomic failure. Neuropathological findings reveal misfolded α-synuclein accumulation in oligodendrocytes, leading to multisystem neurodegeneration. Diagnosis is based on the Movement Disorder Society criteria and characteristic MRI findings but can only be definitively confirmed post mortem. Currently, no disease-modifying therapies are available; management is symptomatic

Early and accurate diagnosis of Parkinson's disease (PD) remains challenging in routine practice, as cardinal motor symptoms emerge only after substantial neurodegeneration has occurred. Blood-based biomarkers are therefore of considerable interest for identifying prodromal disease, enabling biologically stratified clinical trials, and supporting longitudinal monitoring. Among candidate markers, α-synuclein (α-Syn) is centrally relevant due to its fundamental role in PD pathogenesis. However, its peripheral measurement is complicated by marked molecular heterogeneity, uneven compartmental dist

The formation of amyloid fibrils by the intrinsically disordered protein α-synuclein in human brain neurons and extracellular tissue is a hallmark of Parkinson disease. Although the stable protein β-structure in α-synuclein fibrils has been determined at high resolution, the status of the dynamically disordered domains and their responses to in vivo confinement and small-molecule interactions remain elusive. We use electron paramagnetic resonance spectroscopy of the spin probe, TEMPOL, colocalized with α-synuclein fibrils, and temperature-controlled, ice boundary confinement to characterize th

BACKGROUND: Skin biopsy is a promising diagnostic tool for the in vivo identification of Parkinson's disease and other α-synucleinopathies. However, despite increasing adoption, methodological variability exists among different laboratories applying this technique. To move toward a formal standardized protocol, leading experts in the field convened for a meeting focused on discussing and harmonizing the main methodological differences. The outcome of this meeting was a consensus-based skin biopsy protocol, which is here presented.
RESULTS: A formal expert opinion conference was held on Novemb

Within neurons, the misfolding and aggregation of certain proteins has been identified as a common feature of many late-onset neurodegenerative diseases (NDs). These aggregate-prone proteins include tau (in both primary tauopathies and in Alzheimer's disease) and alpha-synuclein in Parkinson's disease. There is strong experimental evidence that the upregulation of intracellular clearance pathways (autophagy and ubiquitin-proteasome pathways) can clear aggregate-prone proteins in experimental models. When the flux through these pathways is increased, the levels of aggregate-prone proteins are r…

Parkinson's disease (PD) is a neurodegenerative disorder marked by substantial heterogeneity in pathological distribution, disease trajectories, and coexisting neuropathologies. Despite this biological complexity, current diagnosis and staging of PD remain largely anchored in clinical manifestations. Neuronal aggregation of misfolded α-synuclein and degeneration of the nigrostriatal dopaminergic system precede the onset of motor symptoms; this phase is termed the prodromal stage of PD. Therefore, clinically based frameworks inherently limit early detection and stratification. In contrast, Alzh

Orthostatic hypotension (OH) is a major problem in patients with Parkinson's disease (PD), necessitating careful management by neurologists. Furthermore, understanding the relationship between OH and other non-motor symptoms provides valuable insight into the pathological progression associated with α-synuclein and the clinical subtypes for PD. This study explains the physiological mechanisms of blood pressure regulation and the pathogenesis and management of OH in PD. It also discusses the relationship between OH and other non-motor symptoms, particularly cognitive decline, including verifica

Amyloid fibrils are protein polymers noncovalently assembled through β-strands arranged in a cross-β structure. Biological amyloids were considered chemically inert until we and others recently demonstrated their ability to catalyze chemical reactions in vitro. To further explore the functional repertoire of amyloids, we here probe if fibrils of α-synuclein (αS) display chemical reactivity toward DNA. We demonstrate that αS amyloids bind DNA at micromolar concentrations in vitro. Using the activity of DNA repair enzymes as proxy for damage, we unravel that DNA-amyloid interactions promote chem

Two triosmium carbonyl clusters, viz., Os3(μ-H)(μ-SC6H4-p-NO2)(CO)10 (1) and Os3(μ-H)(kO,μ-O'-2-flavone)(CO)9 (2), effectively inhibited α-synuclein aggregation, a key signature of Parkinson's disease (PD), in both wild-type and A53T-mutant α-synuclein models. Cluster 2 showed superior efficacy and a significantly better safety profile, and could also disassemble preformed aggregates.

Exosomes are the smallest of the extracellular vesicles, essential in intercellular communication by transporting lipids, proteins, and RNA, thereby influencing the recipient cells. The exosomes from cerebrospinal fluid, central nervous system, and other body fluids have substantial modifications in their molecular composition under various disease conditions. Recent research suggests their potential use as biomarkers for early and prompt detection of neurodegenerative disorders, particularly Parkinson's disease (PD), which is characterized by the misfolded aggregates of the α-synuclein protei

Parkinson's disease (PD) is a neurodegenerative disorder with rapidly increasing prevalence, characterized by dopaminergic neuronal loss and α-synuclein aggregation. Current therapies provide only symptomatic benefit and do not alter the disease course. MicroRNAs (miRNAs), small noncoding RNAs that regulate gene expression, are increasingly recognized as key modulators of PD pathogenesis, influencing oxidative stress, mitochondrial dysfunction, neuroinflammation, and proteostasis. Therapeutic modulation of miRNAs in PD has centered on two complementary approaches: miRNA inhibitors (antagomiRs)

Parkinson's disease (PD) is a chronic, progressive neurodegenerative disorder characterized by severe motor symptoms. While the degeneration of dopaminergic neurons in the substantia nigra plays a central role, other neurotransmitter systems also contribute to PD symptoms. α-Synuclein (αSyn), normally expressed in neurons to support synaptic function and neurotransmitter release, becomes pathologically accumulated in PD, despite not being upregulated under physiological conditions. Intracellular aggregation of αSyn into Lewy bodies is a hallmark of synucleinopathies. A vital facet of both the

Parkinson's disease (PD) is a severe pathology that is caused by a progressive degeneration of dopaminergic neurons in substantia nigra pars compacta as well as other areas in the brain. These neurodegeneration processes are linked to the abrupt aggregation of α-synuclein (α-syn), a small protein that is abundant at presynaptic nerve termini, where it regulates cell vesicle trafficking. Due to the direct interactions of α-syn with cell membranes, a substantial amount of work was done over the past decade to understand the role of lipids in α-syn aggregation. However, the role of phosphatidic a

Increasing research efforts focus on exploiting antibodies to inhibit the amyloid formation of neurodegenerative proteins. Nevertheless, it is challenging to discover antibodies that inhibit this process in a specific manner. Using ribosome display, we screened for synthetic single-domain antibodies, i.e., sybodies, of the P1 region of α-synuclein (residues 36-42), a protein that forms amyloid in Parkinson's disease and multiple-system atrophy. Hits were assessed for direct binding to a P1 peptide and the inhibition of amyloid formation. We discovered a sybody, named αSP1, that inhibits amyloi

Parkinson's disease is one of the most prevalent neurodegenerative motor disorders worldwide with postural instability, bradykinesia, resting tremor and rigidity being the most common symptoms of the disease. Despite the fact that the molecular mechanisms of Parkinson's disease pathogenesis have already been well described, there is still no coherent picture of the etiopathogenesis of this disease. According to modern concepts, neurodegeneration is induced mainly by oxidative stress, neuroinflammation, dysregulation of cerebral proteostasis, apoptotic dysregulation, and impaired autophagy. Thi

Synucleinopathies are a set of devastating neurodegenerative diseases that share a pathologic accumulation of the protein α-synuclein (α-syn). This accumulation causes neuronal death resulting in irreversible dementia, deteriorating motor symptoms, and devastating cognitive decline. While the etiology of these conditions remains largely unknown, microglia, the resident immune cells of the central nervous system (CNS), have been consistently implicated in the pathogenesis of synucleinopathies. Microglia are generally believed to be neuroprotective in the early stages of α-syn accumulation and c

The neurovascular unit (NVU) is composed of vascular cells, glial cells, and neurons. As a fundamental functional module in the central nervous system, the NVU maintains homeostasis in the microenvironment and the integrity of the blood-brain barrier. Disruption of the NVU and interactions among its components are involved in the pathophysiology of synucleinopathies, which are characterized by the pathological accumulation of α-synuclein. Neuroinflammation contributes to the pathophysiology of synucleinopathies, including Parkinson's disease, multiple system atrophy, and dementia with Lewy bod

α-Synuclein accumulates in Lewy bodies, and this accumulation is a pathological hallmark of Parkinson's disease (PD). Previous studies have indicated a causal role of α-synuclein in the pathogenesis of PD. However, the molecular and cellular mechanisms of α-synuclein toxicity remain elusive. Here, we describe a novel phosphorylation site of α-synuclein at T64 and the detailed characteristics of this post-translational modification. T64 phosphorylation was enhanced in both PD models and human PD brains. T64D phosphomimetic mutation led to distinct oligomer formation, and the structure of the ol
