Listening to the multidisciplinary care team: exploring the pediatric palliative care needs in advanced chronic kidney disease.

BACKGROUND: Pediatric palliative care (PPC) aims to improve the quality of life for children with life-limiting conditions, such as advanced chronic kidney disease (CKD), from the time of diagnosis. However, PPC is not commonly integrated into routine pediatric nephrology care. This study explores the perspectives and experiences of healthcare providers (HCPs) to better understand the experiences and specific barriers related to PPC integration for children and adolescents with advanced CKD. METHODS: We conducted a qualitative study with 23 HCPs, including nurses, psychologists, social worker

Diagnosis and management of pyruvate kinase deficiency: international expert guidelines.

Pyruvate kinase (PK) deficiency is the most common cause of chronic congenital non-spherocytic haemolytic anaemia worldwide, with an estimated prevalence of one in 100 000 to one in 300 000 people. PK deficiency results in chronic haemolytic anaemia, with wide ranging and serious consequences affecting health, quality of life, and mortality. The goal of the International Guidelines for the Diagnosis and Management of Pyruvate Kinase Deficiency was to develop evidence-based guidelines for the clinical care of patients with PK deficiency. These clinical guidelines were developed by use of GRADE

How do healthcare providers construe patient complexity? A qualitative study of multimorbidity in HIV outpatient clinical practice.

INTRODUCTION: Patient complexity is an increasingly used concept in clinical practice, policy debates and medical research. Yet the literature lacks a clear definition of its meaning and drivers from the health provider's perspective. This shortcoming is problematic for clinical practice and medical education in the light of a rising number of multimorbid patients and the need for future healthcare providers that are adequately trained in treating complex patients. OBJECTIVES: To develop an empirically grounded framework of healthcare providers' perceptions of patient complexity and to charac

Team-Based Improvements in Pediatric Massive Transfusion Practice.

INTRODUCTION: Massive transfusion protocols (MTP) reduce morbidity and mortality in patients with life-threatening hemorrhage. However, in pediatric settings, MTP activations are rare, leading to inconsistent transfusion practices and underuse of adjunct therapies. We hypothesized that implementing a multidisciplinary MTP response team at our institution would be associated with improved adherence to best practice blood product ratios during pediatric MTP. METHODS: We conducted a retrospective cohort study of pediatric patients who underwent MTP activation at our tertiary pediatric hospital b

Healthcare provider knowledge regarding oral contraception effectiveness for women with a history of bariatric malabsorptive procedures.

BACKGROUND: Clinical practice guidelines that recommend women avoid pregnancy immediately after bariatric surgery, highlighting the need for effective contraception. However, women with a history of malabsorptive bariatric procedures should generally not use oral contraceptives, as these procedures may decrease oral contraceptive effectiveness. OBJECTIVES: To identify provider characteristics associated with knowledge of combined oral contraceptive (COC) effectiveness. SETTING: United States. METHODS: We analyzed weighted survey data collected from national samples of public-sector health c

Translating clinical trials into clinical practice: a survey assessing the potential impact of the Pediatric Heart Network Infant Single Ventricle Trial.

BACKGROUND: A few studies have evaluated the impact of clinical trial results on practice in paediatric cardiology. The Infant Single Ventricle (ISV) Trial results published in 2010 did not support routine use of the angiotensin-converting enzyme inhibitor enalapril in infants with single-ventricle physiology. We sought to assess the influence of these findings on clinical practice. METHODS: A web-based survey was distributed via e-mail to over 2000 paediatric cardiologists, intensivists, cardiothoracic surgeons, and cardiac advance practice nurses during three distribution periods. The resul

Australian best practice recommendations for transjugular intrahepatic portosystemic shunt (TIPS) in portal hypertension: a consensus statement.

BACKGROUND: Transjugular intrahepatic portosystemic shunt (TIPS) is one of the preferred interventional radiology techniques for reducing clinically significant portal pressures in patients with cirrhosis and complications from portal hypertension when pharmacological therapy or endoscopic interventions have failed or been insufficient. Recent advances in TIPS procedural techniques and stent technology, along with emerging indications for TIPS, warrant a review of current practices and establishment of consensus recommendations in Australia, where TIPS remains underused. This TIPS consensus st

A Treat-to-Target Approach in IBD: Contemporary Real-World Perspectives from an International Survey.

Background/Objectives: The management of inflammatory bowel disease (IBD) varies due to differences in healthcare systems, treatment costs, access to diagnostics, and diverse clinical practices between specialists. Despite the frequent advocacy of a treat-to-target (T2T) approach, there is insufficient clarity on how clinicians implement T2T in real-world settings. We aim to conduct a large, global survey among IBD experts to identify current practices in management. Methods: A prospective, cross-sectional study was conducted using a 16-item survey divided into two sections-for ulcerative coli

Using web-based training to improve accuracy of blood pressure measurement among health care professionals: A randomized trial.

Accurate blood pressure measurement is crucial for proper screening, diagnosis, and monitoring of high blood pressure. However, providers are not aware of proper blood pressure measurement skills, do not master all the appropriate skills, or miss key steps in the process, leading to inconsistent or inaccurate readings. Training in blood pressure measurement for most providers is usually limited to a one-time brief demonstration during professional education coursework. The American Medical Association and the American Heart Association developed a 30-minute e-Learning module designed to refres

Increasing immunization coverage. American Academy of Pediatrics Committee on Community Health Services. American Academy of Pediatrics Committee on Practice and Ambulatory Medicine.

Despite many recent advances in vaccine delivery, the goal for universal immunization set in 1977 has not been reached. In 2001, only 77.2% of US toddlers 19 to 35 months of age had received their basic immunization series of 4 doses of diphtheria and tetanus toxoids and acellular pertussis (DTaP) vaccine, 3 doses of inactivated poliovirus vaccine, 1 dose of measles-mumps-rubella (MMR) vaccine, and 3 doses of Haemophilus influenzae type b (Hib) vaccine. Children who are members of a racial or ethnic minority, who are poor, or who live in inner-city or rural areas have lower immunization rates

Periprocedural Role of Nurses During Interventional Endoscopic Procedures Under Deep Sedation.

BACKGROUND: Most of endoscopic procedures, either diagnostic or therapeutic, are nowadays performed under sedation, used as a standard practice in most of the centers. Consequently, the number and complexity of endoscopic procedures has increased as sedation diminishes anxiety and discomfort for patients, also improving the quality of endoscopic examinations, and outcomes in therapeutic endoscopy. Compared to standard diagnostic upper or lower GI endoscopy, endoscopic ultrasound (EUS) and endoscopic retrograde cholangiopancreatography (ERCP) are often longer and more complicated procedures, th

The Best Practice Spotlight Organization® Model: An integrated guideline implementation methodology.

Sustained integration of evidence-based clinical practice guidelines into routine practice and education remains inconsistent across health systems. While implementation science offers robust theoretical explanations of determinants of success, published reports often under-specify how theory is operationalized through governance, knowledge mobilization, and measurement systems that support accountability and learning over time. This limits interpretability, replication, and cumulative learning across contexts. This paper presents the Best Practice Spotlight Organization® (BPSO®) Model, an int

Communicating treatment options to older patients with advanced kidney disease: a conversation analysis study.

BACKGROUND: Choosing to have dialysis or conservative kidney management is often challenging for older people with advanced kidney disease. While we know that clinical communication has a major impact on patients' treatment decision-making, little is known about how this occurs in practice. The OSCAR study (Optimising Staff-Patient Communication in Advanced Renal disease) aimed to identify how clinicians present kidney failure treatment options in consultations with older patients and the implications of this for patient engagement. METHODS: An observational, multi-method study design was ado

Nurse-led family integrated care and time to first parent-infant holding in very preterm infants (< 32 weeks): a multi-site pre-post quasi-experimental study.

BACKGROUND: Preterm birth often leads to prolonged neonatal intensive care unit (NICU) stays and delays early parent-infant bonding. While Family Integrated Care (FICare) addresses these issues, the specific impact of a nurse-led FICare model on expediting first parent-infant holding in very preterm infants remains underexplored. AIM: This study evaluated a nurse-led FICare intervention to reduce the time to first parent-infant holding for very preterm infants (gestational age < 32 weeks) in the NICU. METHODS: Multi-site, nonrandomized pre-post (nonequivalent groups) Type 2 implementation-ef

Patient Transfers and Risk of Back Injury: Protocol for a Prospective Cohort Study With Technical Measurements of Exposure.

BACKGROUND: More than one third of nurses experience musculoskeletal pain several times during a normal work week. Consistent use of assistive devices during patient transfers is associated with a lower risk of occupational back injuries and low back pain (LBP). While uncertainties exist regarding which type of assistive devices most efficiently prevent LBP, exposure assessments using technological advancements allow for quantification of muscle load and body positions during common work tasks. OBJECTIVE: The main objectives of this study are (1) to quantify low back and neck/shoulder muscle
