Fluvoxamine attenuates panic induced by 35% CO2 challenge.

BACKGROUND: The authors investigated whether 6 weeks of treatment with fluvoxamine would decrease the anxiogenic response to the 35% CO2 challenge in 11 patients with DSM-III-R panic disorder with agoraphobia. METHOD: The patients underwent a 35% CO2 challenge at baseline and again after 6 weeks of fluvoxamine treatment. RESULTS: The anxiogenic effect of CO2 was significantly (p < .05) reduced during fluvoxamine treatment. CONCLUSION: The results suggest a relationship between the anxiogenic effect of CO2 and the therapeutic effect of fluvoxamine.

The classification of anxiety disorders in current and future classifications.

Comments are made on the main categories by which specific states of morbid anxiety are classified in the ICD-9, ICD-10, DSM-III and DSM-III-R. The latest classifications, ICD-10 and DSM-III-R, contain many more such categories than ICD-9, and although they differ in the priority given to panic versus agoraphobia, the content is very largely similar. ICD-10 contains several categories of special interest to mental health workers in developing countries and primary health care.

From symmetry to asymmetry.

An introduction is given on Matte Blanco's theory about symmetrical and asymmetrical thought processes and their correspondence with primary and secondary thought processes. The system unconscious is seen in terms of Matte Blanco's principles of symmetry and generalization and their implication in the absence of time, space and the differentiation between the whole and its parts. Clinical material from two cases of agoraphobia are shown as examples of a disturbance or absence of the concept of space in the unconscious and its translation into a symptomatology. The result of psychoanalysis of b…

Alprazolam treatment for panic disorders.

Office records of 27 private patients who were treated with alprazolam for panic disorders were evaluated by a second psychiatrist. Alprazolam treatment led to a complete remission of panic attacks in 85% of the patients; panics ceased within an average of 6 days, at a mean dose of 2.2 mg/day. In addition, alprazolam was found to bring about a complete absence of phobic avoidance behavior in 21 of the 23 agoraphobic patients in the group. This is the first study to demonstrate the efficacy of medication alone as a treatment for agoraphobia.

The treatment of pathological panic states with propranolol.

The authors report on the effects of propranolol, a beta-adrenergic blocking agent, on 10 patients with pathological panic states. Propranolol was effective in treating acute pathological panic, but modest doses of the drug administered for brief periods of time did not alleviate chronic panic attacks associated with agoraphobia. The drug suppressed panic associated with depressive syndromes but did not affect the depression and had no clear effect on anticipatory anxiety. The authors suggest that further study of these findings may clarify other clinical problems.

Synthesis and evaluation of analgesic, behavioral effects and chronic toxicity of the new 3,5-diaminopyrazole and its precursor the thiocyanoacetamide.

This study aimed to explore the analgesic, antioxidant, behavioral and toxicological effects of 3,5-diaminopyrazole and thiocyanoacetamide. Caffeine was used as reference drug whose effects are known after oral treatment with an efficient dose (10mg/kg/day) for 30days. The preliminary bioassays indicated that both compounds at this dose have strong antioxidant capacities and present highly analgesic effects. The behavioral study showed an activation of the rat memory by thiocyanoacetamide. This molecule caused a phobia state to open areas in the elevated plus maze and specifically agoraphobia …

Assessment of automatic associations with bodily sensations and agoraphobic situations in panic disorder.

BACKGROUND AND OBJECTIVES: One of the central assumptions of cognitive models of Panic Disorder (PD) is that automatic panic-related associa … ior to treatment. It included the following stimuli as targets: panic-related bodily sensations and agoraphobia-related situations, and as attributes: pleasant versus unpleasant, fear-related words. RESULTS: Contrary to our expectations, panic patients did not show stronger negative than positive automatic associations for either panic-related symptoms or ag …

Separation anxiety disorder across the lifespan: DSM-5 lifts age restriction on diagnosis.

DSM-5 has lifted the age criterion in the definition of separation anxiety disorder thereby overturning the long-standing convention of restricting the diagnosis to childhood. Previously, adults with separation anxiety symptoms were assigned to other conventional categories such as panic disorder, agoraphobia or generalized anxiety disorder. Over the past two decades, an evolving body of research has identified separation anxiety disorder in adulthood, with 20-40% of adult psychiatric outpatients being assigned that diagnosis. In the US, the lifetime prevalence of the disorder in adulthood is …

Examining determinants of early and late age at onset in panic disorder: an admixture analysis.

Past research demonstrated that age at onset might account for different clinical and etiological characteristics in panic disorder (PD). Ho … s with early onset PD were younger and more likely to be female. Early onset PD was associated with agoraphobia, higher frequency of childhood trauma and life events, and higher rates of suicide attempts as compared to late onset PD. Multivariate logistic regression analysis demonstrated that only current age, childhood trauma and agoraphobia remained signifi …

Association of polymorphisms in P2RX7 and CaMKKb with anxiety disorders.

BACKGROUND: There is considerable evidence that genetic factors play an important role in the pathophysiology of affective disorders includi … ion for multiple testing. We found, however, a prominent association between severity of panic- and agoraphobia symptoms and an exonic SNP (rs3817190) in the CaMKKb gene and a trend for association with an exonic SNP in P2RX7 (rs1718119) with severity scores in the panic- and agoraphobia scale. CONCLUSION: The locus 12q24.31 seems to be an important genetic …

The effectiveness of anxiety treatment on alcohol-dependent patients with a comorbid phobic disorder: a randomized controlled trial.

OBJECTIVE: Evidence has emerged which indicates that the post-treatment relapse rate for alcohol-dependent patients with a comorbid anxiety disorder is higher than for alcohol-dependent patients without a comorbid anxiety disorder. The question raised by this evidence is whether the relapse rate in these dually diagnosed patients could be reduced if they were given additional treatment for the comorbid anxiety disorder. We attempted to answer this question by conducting a trial among patients with a double diagnosis of alcohol dependence and agoraphobia or social phobia. METHOD: We conducted …

Specific side effects of long-term imipramine management of panic disorder.

In a recent study, the authors suggested that tachycardia, dry mouth, and sweating continued to burden patients with panic disorder with agoraphobia who have shown marked and stable response to 6 months of imipramine treatment at the fixed, weight-adjusted dose of 2.25 mg/kg/day. Although sexual dysfunction and weight gain were not a significant burden in that study, they are important problems in long-term treatment with antidepressant drugs. In the present study, in the context of a randomized, double-blind, placebo-controlled, 1-year discontinuation and maintenance study of 53 patients with…
