[Morphologic changes in chronic active superficial Campylobacter pylori-positive antrum gastritis after treatment with bismuth].

The authors investigated the effect of one month bismuth treatment (Bismuthi citrici 120 mg per capsule), 4 x 120 mg/day, on morphological changes of the gastric mucosa along with the effect on Campylobacter pylori (CP) in a group of 23 probands with histologically verified active superficial CP positive antrum gastritis. The probands suffered only from functional dyspepsia, to eliminate the action of other disease of the digestive tract on chronic gastritis. Complete eradication of CP occurred in 65.2% and disappearance of granulocytic infiltration as a manifestation of activity in 73.9%. The

Spontaneous resolution of acid gastric injury.

While alkali ingestion nearly always injures the esophagus, acid usually spares the esophagus and damages the gastric outlet. Most reported cases of acid gastric injury have required resection. We report a case of acid ingestion managed with parenteral nutrition and histamine blockers in which healing is documented without operation.

[Posttraumatic real and false Mendelson's syndromes].

Six observations of post-traumatic acute respiratory insufficiency of alveolar origin are reported briefly. Three of them have been attributed to a Mendelson's syndrome. This authors showed that diagnosing these lung diseases by acid inhalation is particularly difficult in casualties. It is a diagnosis by elimination which no certainty can prop. These post-traumatic Mendelson's syndrome are serious and difficult to prevent. In the three cases reported two accidents occurred during or immediately after an intubation. The systematic use of an efficient anti-acid could be recommended in all serio

Autoimmune atrophic gastritis with hypergastrinemia.

Elevation in fasting serum gastrin levels was found in three patients being evaluated for persistent upper abdominal pain without radiographic evidence of peptic ulcer disease. Fiberoptic endoscopy of the upper gastrointestinal tract in each patient revealed characteristic changes of chronic atrophic gastritis. Gastric biopsies showed diffuse chronic inflammation in the lamina propria, a decrease in the number of parietal cells, and "intestinalization" of gastric mucosa. Total achlorhydria was demonstrated after a maximal histalog stimulus; however, serum levels of vitamin B12 and Schilling te

Knowledge gaps in the management of refractory reflux-like symptoms: Healthcare provider survey.

BACKGROUND: Refractory reflux-like symptoms have a substantial impact on patients and healthcare providers. The aim of the survey was to qualitatively assess the needs and attitudes of practicing clinicians around the management of refractory reflux symptoms and refractory gastroesophageal reflux disease (rGERD). METHODS: An International Working Group for the Classification of Oesophagitis (IWGCO) steering committee invited clinicians to complete an online survey including 17 questions. KEY RESULTS: Of the 113 clinicians who completed the survey, 70% were GIs, 20% were primary care physicia

Pathophysiology and modern treatment of ulcer disease.

This is an overview of the pathophysiological abnormalities of gastroduodenal (GD) ulcers [duodenal ulcer (DU), gastric ulcer (GU) and Dragstedt ulcers (combined DU and GD)], as well as the effects of the different treatments (surgical, medicinal and physiological) described since the introduction of stomach resections. The intention is to demonstrate whether the peptic ulcer diseases are a homogeneous entity with a characteristic pathophysiology or whether they represent the final expression of many heterogeneous causes including impairment of upper gastrointestinal motility. The review also

Gastroesophageal reflux disease--state of the art.

Gastroesophageal reflux disease (GERD) is defined as chronic symptoms or mucosal damage produced by the abnormal reflux of gastric contents into the esophagus; heartburn, its most common manifestation, occurs in 7% to 10% of the U.S. population on a daily basis. In addition, many so-called extraesophageal or atypical symptoms, including chronic cough, laryngitis and other otolaryngologic conditions, asthma, and unexplained chest pain, can be associated with GERD, but these patients appear to have a decreased frequency of heartburn, making the diagnosis of GERD difficult. All patients can be su

Selection of drugs to treat gastro-oesophageal reflux disease: the role of drug interactions.

Gastro-oesophageal reflux disease is probably the most common acid-peptic disease in Western countries, and the successful treatment of mild … medications that may influence the efficacy of antireflux therapy. Important drug interactions with antacids include the prevention of the absorption of antibacterials such as tetracycline, azithromycin and quinolones. H2 antagonists, proton pump inhibitors and prokinetic agents undergo metabolism by the cytochrome P450 (CYP) system present in the liver an …

Chemical structure of bismuth compounds determines their gastric ulcer healing efficacy and anti-Helicobacter pylori activity.

The recognition of the role of Helicobacter pylori in the pathogenesis of peptic ulcer disease has led to renewed interest in bismuth pharmacology since bismuth compounds have both anti-Helicobacter pylori and ulcer healing properties. The precise chemical structure of current bismuth compounds is not known. This has hindered the development of new and potentially more efficacious formulations. We have created two new compounds, 2-chloro-1,3-dithia-2-bismolane (CDTB) and 1,2-[bis(1,3-dithia-2-bismolane)thio]ethane (BTBT), with known structure. In a rat model of gastric ulceration, BTBT was com

Histamine H2-receptor antagonists in the treatment of duodenal ulcers.

A double-blind trial of histamine H2-receptor antagonists and placebo was carried out in 46 patients with endoscopically proven duodenal ulceration. H2-receptor antagonists (metiamide and cimetidine) produced a significantly greater degree of completely healed ulcers and fewer complete failures than placebo. The results of the trial indicate that H2-receptor antagonists promote the healing of ulcers. The action of these compounds would appear to be related to the inhibition of acid secretion. Neither the duration of the illness, rate of recurrence of the ulcers of level of gastric acid secreti
