BACKGROUND: Surgery for congenital cataracts in early infancy usually includes a primary posterior capsulectomy and an anterior vitrectomy. Initially, most of these infants have aphakia after surgery. Over time, remaining equatorial lens epithelial cells produce new cortical fibers, resulting in a ring of cortex trapped between the lens equator and the fused anterior and posterior capsulectomy edges. A potential space is maintained between the anterior and posterior capsular leaflets. We describe a technique for placing a secondary intraocular lens (IOL) within the capsular bag.

We describe a case of 34-year-old male with post penetrating keratoplasty glaucoma, post trabeculectomy with aphakia in the only seeing eye, in which a modified surgical technique of inserting Ahmed glaucoma valve (AGV) tube in vitreous cavity was done to reduce the risks associated with pars plana incision during pars plana vitrectomy (PPV). A hybrid 20-25 gauge PPV was done concurrently, implant fixed to sclera, and tube inserted through the 25 gauge sclerotomy port in supero-temporal quadrant. Visual acuity and intraocular pressure remained stable during 1-year follow-up.

INTRODUCTION: Glaucoma of the aphakic patient is often difficult to treat. Classical surgical management is limited by poor quality conjunctiva and by the presence of the vitreous gel opposite the fistula. We present a new surgical technique which uses a portion of the trabeculum as a drain placed in the anterior chamber.

The authors presented the data concerning pathogenesis, diagnostic principles and methods of treatment of pupillary block in the eyes with intraocular lenses. 3 cases with pupillary block in the eyes with anterior chamber lenses have been presented. The fellow normal-appearing eyes of 23 patients with unilateral aphakic bullous keratopathy were examined by wide-field specular microscopy and pachymetry. All patients had: pseudophakic or aphakic bullous keratopathy in one eye; unoperated fellow eye on no medication; no other ocular disease; and no endothelial guttata seen by slitlamp biomicroscopy or specular microscopy. Three of 23 patients had endothelial cell counts less than 1,000 cells/mm2. There was no correlation between cell density and corneal thickness, but a negative correlation existed between age and endothelial cell density. P

Since 1977 intraocular lenses of 4 different types have been implanted in a total of 963 eyes at Frankfurt University Eye Clinic: the 4-loop lens (Binkhorst), the 2-loop lens (Binkhorst) and the posterior chamber lenses designed by Shearing and Simcoe. Implantation of a posterior chamber lens after extracapsular cataract extraction produced very good results with no severe complications.

Glaucoma secondary to angle closure is a not infrequent complication of cataract extraction. The incidence may be reduced by preventing or accurately treating those conditions causing shallowing of the anterior chamber in the postoperative period. Once established, however, management follows the principles used in managing chronic simple glaucoma.
