Differentiated treatment of secondary cataract following extracapsular cataract operation.

161 eyes with Nd:YAG laser posterior capsulotomy after in the bag IOL implantation 1-3 years before, were actively followed over a period of 24 +/- 6 months. No CME nor retinal detachment nor induced glaucoma was found. Therefore a YAG capsulotomy in eyes with completely in the bag implanted IOL seems to assure the same level of security than capsular polishing in eyes with sulcus implanted lenses.

Pediatric aphakic contact lens wear: rates of successful wear.

Two hundred thirty-eight consecutive patients with pediatric cataracts were fitted with a variety of aphakic contact lenses after cataract extraction. Thirty-nine patients did not return for follow up. Of the remaining 199 patients, 40 discontinued contact lens wear. None of the 78 patients with unilateral or bilateral acquired cataracts discontinued contact lens wear due to problems wearing their lenses, although nine discontinued contact lens wear due to poor vision or difficulty maintaining amblyopia treatment. Twenty-two of 84 patients with unilateral congenital cataracts discontinued lens

Secondary implantation of flexible open loop anterior chamber IOLs.

Secondary implantation of flexible, open loop anterior chamber IOLs was performed on 20 aphakic eyes between January 1990 and May 1992. In 3 cases anterior vitrectomy was necessary. No other complications were encountered during surgery. The patients were followed up for 4 months postoperatively. Seventeen eyes achieved a corrected visual acuity of 5/5 or better. Fourteen eyes achieved the same visual acuity as preoperatively, and 5 ended with an improved visual outcome. In one case the visual acuity became worse than preoperatively due to cystoid macular edema. No other serious postoperative

Trabeculectomy at the inferior limbus.

Fourteen eyes (12 with open-angle glaucoma and 2 with secondary glaucoma) of 14 patients were operated by trabeculectomy at the inferior limbus. The surgical technique is described and results of 6-12 months follow-up are reported. Complication rate was reasonably low. Early postoperative IOP higher than 30 mmHg occurred in one eye where a blood clot and sodium hyaluronate obstructed filtration of aqueous. Postoperative mean IOP was 16.3 mmHg (6-29 mmHg) in 13 eyes at 3 months, 17.1 mmHg (13-26 mmHg) in 12 eyes at 6 months and 20.1 mmHg (11-31 mmHg) in 12 eyes at 12-16 months. Three eyes had f

Secondary lens implantation: incidence, indications and complications.

A retrospective study of 79 secondary intraocular lens implants performed during 1984-87 in a large teaching hospital is reported. The period of follow-up ranged from 6 months to 3.9 years (mean 16.6 months). The principal indication for surgery was contact lens intolerance. Thirty-four per cent of the patients had previously undergone cataract surgery with implantation of the other eye. Eighty-three per cent of the patients achieved a visual acuity within one Snellen line of best-corrected preoperative visual acuity. A further 2.5% lost two lines of Snellen chart acuity, and 14.5% had a subst

Extended wear hydrogel and daily wear hard contact lenses for aphakia. Success and complications compared in a longitudinal study.

The success and complications of extended wear hydrogel (EWSCL) and daily wear hard contact lens (DWHCL) correction for aphakia were evaluated retrospectively in two comparable groups of unilaterally aphakic patients (64 with EWSCL, 86 with DWHCL). At the end of the follow-up period, 55% of patients in each group were still using contact lenses. Suppurative keratitis occurred in two (3%) patients using EWSCL and none with DWHCL. The relative risk to all patients of a complication developing severe enough to result in a loss of wearing time was 9.8 times higher for the EWSCL users compared with

Epidemiology of retinal detachment.

A survey of rhegmatogenous retinal detachment during 1976 was conducted on the population of Iowa. The annual incidences (per 100,000 populations) of four types of detachment were determined: nontraumatic phakic (6.1), traumatic phakic (1.0), nontraumatic aphakic (4.9), and traumatic aphakic (0.4). The incidences of nontraumatic phakic detachment for men and women were similar, although there was a preponderance of women that reflected their greater representation in the general population. The proportions of men were significantly higher in traumatic detachments compared with nontraumatic det

Correcting low vision in aphakic children.

There are many children who have subnormal vision in spite of successful cataract surgery and aphakic spectacle lenses. There are children who have lens opacities for whom the state of aphakia would not be an improvement. Careful evaluation of functional vision, careful refraction, and the prescription of high adds or other optical devices are part of complete ophthalmological care for every one of these children. The dioptric strength of the reading add is determined by the functional reading vision and the calculation of the reciprocal of the visual acuity. It is also important to realize th

The need for intraocular lens implantation in children.

The intraocular lens offers an additional experimental parameter to be considered for the visual rehabilitation of children with traumatic or unilateral infantile cataracts. The disadvantages of aphakic spectacles or contact lens and amblyopia have committed many of these eyes to the status of visual cripples. In this series of 41 patients, with a maximum of 24 months follow-up, it has been concluded that the IOL may be successfully implanted.

Long-term Effect of Intraocular Lens vs Contact Lens Correction on Visual Acuity After Cataract Surgery During Infancy: A Randomized Clinical Trial.

IMPORTANCE: Although intraocular lenses (IOLs) are often implanted in children, little is known whether primary IOL implantation or aphakia and contact lens correction results in better long-term visual outcomes after unilateral cataract surgery during infancy. OBJECTIVE: To compare long-term visual outcomes with contact lens vs IOL correction following unilateral cataract surgery during infancy. DESIGN, SETTING, AND PARTICIPANTS: This multicenter randomized clinical trial enrolled 114 infants with a unilateral congenital cataract who underwent cataract surgery with or without primary IOL im
