The intricate interrelationship between obstructive sleep apnea (OSA) and orofacial pain represents a significant clinical challenge that necessitates comprehensive understanding and management. This review elucidates the bidirectional pathophysiological mechanisms underlying these comorbid conditions, wherein OSA demonstrates prevalence rates of 2-9% in adults, with marked gender dimorphism, while orofacial pain conditions affect 10-20% of individuals during their lifetime. The manuscript delineates how sleep-disordered breathing induces compensatory neuromuscular responses, manifesting as in…

PURPOSE: Obstructive sleep apnea (OSA) and rapid eye movement (REM) sleep fragmentation are associated with increased fatigue, sleepiness, anxiety and depressive symptoms. However, the clinical significance of REM-predominant OSA (REM-OSA) remains less clear.
AIM: To determine any differences in symptom-burden between REM-OSA and non-REM (NREM) OSA.
METHODS: Patients diagnosed with OSA at a University Hospital in Brussels between 25-11-2019 and 19-09-2020 were eligible. REM-OSA was defined as an apnea hypopnea index REM/NREM ratio of at least 2 with a minimum REM-time of 30 min. Severe OSA w…

PURPOSE: Obstructive sleep apnea (OSA) subjects show significant white matter injury, including myelin changes in several brain areas, potentially from impaired glial cells, contributing to increased iron levels that escalate neurodegeneration, but brain iron loads are unclear. Our aim was to examine regional brain iron load, using T2∗-relaxometry, in OSA adults before and after continuous positive airway pressure (CPAP) treatment over controls.
METHODS: We performed T2∗-weighted imaging using a 3.0-T MRI scanner on 35 OSA adults, who were followed after 3- and 9- mo CPAP treatment, and 67 co…

The early detection and treatment of sleep apnea syndrome (SAS) is becoming an important issue in medical practice, as the conventional polysomnography-based method is not only costly, but also demands high expertise from technicians. In this study, we propose a method for diagnosing SAS using noninvasive measurements of oxygen saturation (SpO2), obtained from a pulse oximeter as the input signal, using convolutional neural networks (CNNs). As a result of 10-fold cross-validation, we confirmed that the proposed method yields an 88.99% accuracy.

OSA is diagnosed and managed by a metric called the apnea-hypopnea index (AHI). The AHI quantifies the number of respiratory events (apnea or hypopnea), disregarding important information on the characteristics and physiologic consequences of respiratory events, including degrees of ventilatory deficit and associated hypoxemia, cardiac autonomic response, and cortical activity. The oversimplification of the disorder by the AHI is considered one of the reasons for divergent findings on the associations of OSA and cardiovascular disease (CVD) in observational and randomized controlled trial stud…

A 55-year-old woman presented with an incidental finding of significant respiratory variation in mean right atrial pressure during cardiac catheterization. Later, she was evaluated and diagnosed with sleep apnea. A significant increase in right atrial pressure with respiration should raise awareness for sleep apnea.

IMPORTANCE: Obstructive sleep apnea (OSA) is a common condition in older adult (aged >65 years) populations, but more mechanistic research is needed to individualize treatments. Previous evidence has suggested an association between OSA and posttraumatic stress disorder (PTSD) but is limited by possible selection bias. High-quality research on this association with a careful evaluation of possible confounders may yield important mechanistic insight into both conditions and improve treatment efforts.
OBJECTIVE: To investigate the association of current PTSD symptoms and PTSD diagnosis with OSA…

BACKGROUND: Automatic sleep staging is essential for assessing sleep quality and diagnosing sleep disorders. While previous research has achieved high classification performance, most current sleep staging networks have only been validated in healthy populations, ignoring the impact of Obstructive Sleep Apnea (OSA) on sleep stage classification. In addition, it remains challenging to effectively improve the fine-grained detection of polysomnography (PSG) and capture multi-scale transitions between sleep stages. Therefore, a more widely applicable network is needed for sleep staging.
METHODS: …

Purpose: Obstructive sleep apnea (OSA) is a prevalent condition with long waiting lists for the gold standard polysomnography (PSG) test. Screening tests are essential to identify and prioritize patients with severe OSA. Current questionnaires do not accurately detect severe cases. This study aims to develop an alternative screening tool based on anthropometric and morphological characteristics to determine the severity of OSA. Methods: The study included 149 patients with sleep apnea symptoms who did not have additional diseases. The obstructive sleep apnea morphologic scale (OSAMS) was creat…

Obstructive sleep apnea (OSA) is a common disorder that affects quality of life and is associated with comorbidities such as hypertension, atrial fibrillation, heart failure, coronary heart disease, type 2 diabetes mellitus, and stroke. OSA is characterized by a reduction or cessation of breathing during sleep, resulting in intermittent hypoxemia, autonomic fluctuation, and sleep fragmentation. The U.S. Preventive Services Task Force states that there is insufficient evidence to recommend routine screening for OSA in the absence of symptoms. OSA should be considered in patients with excessive …
