Interplay of microbe and major histocompatibility complex: a family study.

We describe a 24-year-old man who developed reactive arthritis (ReA) after a dysenteric illness caused by Salmonella hadar. Serologic tests suggested recent exposure of family members to Salmonella. All members of the family were HLA-B27 positive, but no other family member developed acute ReA, although 2 of them had clinical evidence of previously existing B27 associated arthritis.

[Clinico-immunologic heterogeneity of chronic arthritis in children].

The authors present the results of immunological examination of 29 children with reactive arthritis, 26 patients with different forms of juvenile rheumatoid arthritis and 25 practically healthy children. Considerable differences between the groups in the subpopulation composition of T- and B-lymphocytes were revealed. Direct relationship between the absolute value of OCT4/OCT8 ratio and the clinical severity of the process was noted.

The potential value of fibrinogen to albumin ratio (FAR) in the assessment of inflammation in spondyloarthritis.

BACKGROUND: Fibrinogen to albumin ratio (FAR) is a newly investigated indicator for inflammation. The study aimed to explore the potential ability of FAR in assessing the severity of inflammation in spondyloarthritis. METHODS: The clinical data of 196 spondyloarthritis (SpA) patients, 66 osteoarthritis (OA) patients, and 81 healthy controls (HC) were collected in this retrospective study. The SpA group included 69 psoriatic arthritis patients, 47 reactive arthritis patients and 80 ankylosing spondylitis patients. The radiology of rheumatoid arthritis.

Rheumatoid arthritis is a connective tissue disorder of unknown etiology that can involve any synovial-lined joint in the body. The most frequently involved joints are the small joints of the hands and feet, the wrists, knees and elbows, and the glenohumeral and acromioclavicular joints. The radiographic hallmarks of rheumatoid arthritis are swelling of the soft tissue, osteoporosis, narrowing of the joint spaces and marginal erosions. The unique combination of osteoporosis, marginal erosions and relatively minimal reactive bone formation help distinguish rheumatoid arthritis from other inflam

Chronic arthritis in adolescence.

Musculoskeletal complaints, and joint pain in particular, are common symptoms among adolescents presenting to primary care providers. In this article, the distinction between arthralgia and arthritis is described, and important inflammatory arthropathies seen in teenagers are addressed. The current nomenclature of juvenile idiopathic arthritis is used to address each of the major clinical entities that account for chronic arthritis in teens, including oligoarthritis, polyarthritis, systemic arthritis, psoriatic arthritis, and spondyloarthropathy. Diagnostic criteria, important comorbidities, a

"Bypass arthritis" after gastroileostomy.

We describe a patient who underwent inadvertent gastroileostomy, and subsequently developed malabsorption, granulomatous hepatitis, arthritis, and reactivation of tuberculosis accompanied by circulating immune complexes and lowered C1q. Surgical correction resulted in marked improvement, with disappearance of arthritis, return to normal of complement levels, and absence of demonstrable circulating immune complexes. Arthritis has not been documented in the spectrum of abnormalities described after gastroileostomy.

Mycoplasmal arthritis in man.

Naturally occurring mycoplasmal arthritis in various animals species and the ability to induce mycoplasmal arthritis experimentally have been the main reasons for searching for evidence of mycoplasmal infection in the joints of patients with rheumatoid arthritis. However, reliable reports of the isolation of mycoplasmas from synovial fluids and tissue of such patients do not exist, and measurements of specific antibodies have not proved rewarding. There is some evidence that infection by Mycoplasma pneumoniae occasionally has an arthritic sequela, although the microorganism has not been isolat

Serum amyloid P component levels are not decreased in patients with systemic lupus erythematosus and do not rise during an acute phase reaction.

BACKGROUND: Serum amyloid P component (SAP) and acute phase proteins like C-reactive protein contribute to the clearance of apoptotic cells. This response is diminished in systemic lupus erythematosus (SLE). OBJECTIVES: To analyse SAP concentrations in SLE in relation to disease activity, and investigate whether SAP reacts like an acute phase protein. METHODS: SAP was measured in 40 patients with SLE during active and inactive disease and compared with healthy controls and patients with rheumatoid arthritis and Wegener's granulomatosis. Normal SAP values were determined in 120 healthy contro

Juvenile Spondyloarthritis: A Distinct Form of Juvenile Arthritis.

Juvenile spondyloarthritis (SpA) is a distinct form of juvenile arthritis characterized by male predominance and adolescent onset. Clinical manifestations include lower extremity and sacroiliac joint arthritis, enthesitis, and subclinical gastrointestinal inflammation. Juvenile SpA is an immune-mediated inflammatory disease long recognized as associated with HLA-B27, which may be related to the microbial environment as suggested by its coexistence with reactive arthritis and psoriasis. Treatment of peripheral arthritis includes nonsteroidal anti-inflammatory drugs, joint injections, and diseas

Leflunomide as a cause of collagenous colitis: an entity to consider.

Leflunomide belongs in the group of disease-modifying anti-rheumatic drugs (DMARDs) used in the treatment of psoriatic, rheumatoid, and reactive arthritis. Approximately 20 % of patients will experience some adverse event, mainly weight loss, abdominal pain, and diarrhea. We describe the clinical, endoscopic, and histological findings in a patient with psoriatic arthritis (PA) who developed severe chronic diarrhea after drug use.

Renal affection in patients with ankylosing spondylitis and psoriatic arthritis.

In a retrospective study of 148 patients with well-defined ankylosing spondylitis (AS), psoriatic arthritis (PSA) or reactive arthritis (ReA) an 11% prevalence of idiopathic hematuria, proteinuria, or cylinduria was found in the former two groups. None of the patients with ReA had unexplained pathological urinary findings. Such findings were associated with raised ESR and presence of peripheral arthritis in AS and with the duration of disease in PSA. No patient lacking sacroiliitis showed pathological urinary findings. We believe that such findings may reflect nephropathy associated with AS an

A syndrome of seronegative enthesopathy and arthropathy in children.

Thirty-nine children with a syndrome of seronegative enthesopathy and arthropathy were evaluated. The group included 25 patients with no apparent underlying primary disease and 13 with either ankylosing spondylitis, inflammatory bowel disease, reactive arthritis, or Reiter's syndrome. Significant distinguishing characteristics of the group included male predominance, late age at onset, positive family histories of arthritis, oligoarthropathy, axial skeleton involvement, and the presence of the B27 histocompatibility antigen. This syndrome is distinguishable from other childhood rheumatic disor

Sir Benjamin Collins Brodie (1783-1862) - a pioneer rheumatologist.

AIM: Sir Benjamin Collins Brodie (1783-1862) was a London surgeon who investigated joint disease by observation and morbid anatomy in the first half of the 19th century. His descriptions of inflammatory joint disease have been referred to in the past, but only in a fragmentary way. This study aimed to provide more detail than previously, allowing for a new appreciation of his contributions. METHODS: The authors used the first (1818), third (1834) and fifth (1850) editions of his book, Pathological and Surgical Observations on the Diseases of Joints, to provide his description of reactive arth

A Systematic Approach to the Evaluation of a Limping Child.

A limp is defined as a deviation from a normal age-appropriate gait pattern resulting in an uneven, jerky, or laborious gait. It can be caused by pain, weakness, or deformity as a result of a variety of conditions. Transient synovitis is the most common diagnosis. Other causes of acute limp include contusion, foreign body in the foot, fracture, osteomyelitis, septic arthritis, reactive arthritis, and Lyme arthritis. Causes of chronic limp include rheumatic disease, dermatomyositis, acute rheumatic fever, inflammatory bowel disease, and systemic lupus erythematosus. Evaluation of a limping chil…

Keratan sulphate in rheumatoid arthritis, osteoarthritis, and inflammatory diseases.

Serum concentrations of antigenic keratan sulphate determined by an enzyme linked immunosorbent assay (ELISA) with a monoclonal antibody were studied in patients with rheumatoid arthritis (RA), osteoarthritis, ankylosing spondylitis, other inflammatory diseases, and a large control group of women without arthritis. Mean keratan sulphate concentrations were low in 117 women with RA compared with 227 female control subjects matched for age drawn from a community survey. There were significant correlations between serum keratan sulphate concentrations in patients with RA and serum C reactive prot
