Mechanisms and management of paroxysmal supraventricular tachycardia.

Paroxysmal supraventricular tachycardia (PSVT) is defined as a regular rapid heart beat, which initiates and terminates suddenly. PSVT may have a variety of electrophysiologic mechanisms, including atrial tachycardia, atrioventricular (AV) nodal reentry, and tachycardia involving an accessory AV connection. Atrial tachycardia may be reentrant or may be caused by abnormal automaticity or triggered activity. AV nodal reentry, in which the reentrant circuit is confined to the AV node or the region around the AV node, is the most common type of PSVT in adults. Orthodromic supraventricular tachycar

[Atriofascicular connection with slow conduction and longitudinal dissociation. Electrophysiologic evidence and clinical implications].

We report on a patient with paroxysmal left bundle branch block-like tachycardia with electrophysiologic findings suggestive of nodoventricular pathway (ventricular pre-excitation dependent on slowing of AV conduction, and accessory pathway with exclusive anterograde conduction). There was no pre-excitation during sinus rhythm but it was brought on by intravenous verapamil. Atrioventricular node conduction curves showed no signs of duality. Diagnosis of an atriofascicular pathway with decremental properties was based on the following findings: 1) absence of AV dissociation during reciprocating…

Ventriculoatrial intervals: diagnostic use in paroxysmal supraventricular tachycardia.

Reciprocating tachycardias due to reentry either within the atrioventricular (AV) node or using an accessory AV pathway are a common cause of paroxysmal supraventricular tachycardia in humans. Unfortunately, although of potential therapeutic value, differentiation of these forms of reciprocating tachycardia may be difficult and require detailed electrophysiologic study. To develop diagnostic criteria that permit exclusion of participation of an accessory AV pathway in reciprocating tachycardia without extensive laboratory testing, results of electrophysiologic studies were examined in 50 patie

Wide QRS Tachycardia in WPW: When Antidromic AVRT Imitates VT.

Atrioventricular Re-entry Tachycardia (AVRT) occurs in patients with accessory pathways (AP) via a re-entry circuit between the AV node and the AP. Anterograde conduction through the AP causes antidromic AVRT with wide QRS complexes, which may mimic ventricular tachycardia (VT). We report a 59-year-old male with ischemic cardiomyopathy (EF 35%) and recurrent wide QRS tachycardia who was referred for ICD implantation. Electrophysiology study identified a left posterolateral AP, and radiofrequency ablation successfully eliminated pre-excitation. ICD implantation was subsequently performed withou

A Case of Junctional Ectopic Tachycardia and Complete Atrioventricular Block After Transcatheter Aortic Valve Replacement.

Junctional ectopic tachycardia (JET), a tachyarrhythmia originating from the atrioventricular (AV) node and/or bundle of His, is commonly observed in pediatric patients following congenital heart surgery. JET is characterized by a heart rate above the 95th percentile for age, whereas rates below this threshold are referred to as accelerated junctional rhythm (AJR). Although AJR with a potential risk of developing AV block has been reported following transcatheter aortic valve replacement (TAVR), no cases of JET following TAVR have been documented. We report a case of JET and complete AV block

Conduction System Pacing Following a Double Switch Operation for Congenitally Corrected Transposition of the Great Arteries.

The use of conduction system pacing (CSP) in adults with congenital heart disease (CHD) is growing, however data remain limited. In patients with congenitally corrected transposition of the great arteries following the double switch operation, existing CSP tools and techniques require modification to allow for the anterior displacement of the atrioventricular node and proximal conduction system in addition to navigating the tortuous route of the atrial redirection. We report the successful use of CSP focusing on the technique of delivery tool modification to allow stability on the basal septum…

An Unusual Case of Atrial Wall Cardiac Sarcoidosis Detected on Fluorine-18 Fluorodeoxyglucose Positron Emission Tomography-Computed Tomography Scan.

Isolated cardiac sarcoidosis (ICS) accounts for 5%-10% of patients with sarcoidosis. It can involve atrioventricular node causing heart block, as well as the basal septum, papillary muscles, focal regions in the free wall, and the myocardium being more commonly involved. The diagnosis is achieved on magnetic resonance imaging (MRI) and endomyocardial biopsy. Recently, Fluorine-18 fluorodeoxyglucose positron emission tomography and computed tomography (F-18 FDG PET) has been incorporated in the diagnosis as well as management algorithm. We describe an interesting case of ICS detected on F-18 FD…

[Lung Cancer Accompanied by Sarcoidosis with Mediastinal and Bilateral Hilar Lymphadenopathy;Report of a Case].

A 64-year-old woman with complete atrioventricular block caused by sarcoidosis was emergently placed a pacemaker. A 10 mm nodule in the left upper lobe of the lung and the mediastinal and bilateral hilar lymphadenopathy was detected through chest computed tomography. To establish the diagnosis, resection of the tumor and #4L was performed. By intraoperative pathology, the nodule was diagnosed as an adenocarcinoma and #4L was found to be a granuloma without metastasis of carcinoma. Subsequently, left upper lobectomy and lymph node dissection (ND2a-2) was conducted. Pathological stage was stageⅠ

Wide QRS complex supraventricular tachycardia with negative precordial concordance: Electrocardiographic clues for Mahaim pathway with Ebstein anomaly.

Ebstein's anomaly is often accompanied by either Wolff-Parkinson-White syndrome or atriofascicular Mahaim. These bypass tracts give rise to antidromic atrioventricular (AV) re-entrant tachycardias, in which the bypass tract serves as the anterograde limb of the circuit and the AV node as the retrograde limb of the reentrant circuit. Since the antidromic AV reentrant tachycardia over a Mahaim fibre has a typically left bundle braunch block (LBBB) morphology, it is easy to make a misdiagnosis of supraventricular tachycardia with functional LBBB or even of ventricular tachycardia particularly in

Changing QRS Morphology in a Patient with Wolff-Parkinson-White Syndrome.

A case of Wolff-Parkinson-White syndrome with supraventricular tachycardia is presented in which 2 atrioventricular (AV) reentry circuits are identified. The first circuit used the AV node in the antegrade limb and a left lateral accessory pathway in the retrograde limb. The second circuit used an atriofascicular accessory pathway as the antegrade limb and the left lateral accessory pathway as the retrograde limb. The characteristics of these tachycardias are described, along with diagnostic maneuvers used in the electrophysiology laboratory. Mapping and ablation of both of these accessory pat

Evaluation and management of the adult patient with transposition of the great arteries following atrial-level (Senning or Mustard) repair.

Atrial-level repair for transposition of the great arteries is one of the greatest achievements of modern cardiovascular medicine, transforming a once fatal congenital heart condition into one with excellent long-term survival. Although atrial-level repair has been supplanted by the arterial switch, there remains a population of patients who underwent atrial-level repair as children and now require ongoing care as adults. Survival slowly continues to decline in this population mainly owing to systemic right ventricular dysfunction and sudden death. Other problems include sinus node dysfunction

Double ventricular response caused by longitudinal dissociation in the his bundle.

Electrocardiograms were taken from a 36-year-old woman with normal sinus rhythm in which 4 types of QRS complexes and 2 types of fusion QRS complexes were found in configuration. To interpret this arrhythmia, we proposed 3 unique mechanisms: a) double ventricular response through the fast pathway mainly directing to the right bundle branch and the slow pathway mainly directing to the left bundle branch, b) longitudinal dissociation in the His bundle, and c) transverse conduction in the lower edge of the dissociation. Although functional longitudinal dissociation in the atrioventricular node ha…

Proarrhythmic effects of adenosine: one decade of clinical data.

In 1989, adenosine was introduced into the American clinical setting as an antiarrhythmic drug for the acute management of reentrant supraventricular tachycardia involving the atrioventricular node. During this decade of use, evidence for proarrhythmic effects of the drug have been documented. In addition to the mostly benign transient episodes of atrial fibrillation, several cases of life-threatening ventricular arrhythmias induced by adenosine have been reported. This article summarizes the proarrhythmic effects of adenosine as they were reported in the literature as well as data from the ma…

[Diagnostic use of adenosine triphosphate in cardiac arrhythmias].

Adenosine triphosphate (ATP) has been used for more than 50 years as a major medication in our therapeutic arsenal for the termination of supraventricular tachycardia crises for which the mechanism is re-entry implicating the atrioventricular node. In the present article other applications of ATP as a diagnostic agent for several cardiac arrhythmias are described: differential diagnosis of narrow or wide QRS tachycardia, diagnosis of a latent accessory pathway (intermittent or hidden), diagnosis of intranodal duality, and non-invasive evaluation of the results of the ablation of the slow pathw…

Forensic significance of conduction system abnormalities as a precise cause of accidental death.

We examined the pathology findings of the cardiac conduction system in 42 human autopsies who were considered to have died an accidental death. Abnormalities in serial sections of the conduction system were found in seven and of these three had an accessory pathway in the conduction system. In one of these there was mild Ebstein's anomaly (ME), two had fibromuscular dysplasia of the atrioventricular node artery with mitral valve prolapse, and two others had abnormal routes showing fragmentation of the bundle of His with ME. A full investigation of the cardiac conduction system can therefore be…

Clinical, pathological and molecular genetic findings in a case of neonatal Marfan syndrome.

An infant with neonatal Marfan syndrome is described who presented with arachnodactyly, distinctive dysmorphic features and prolapse of both atrioventricular valves and dilatation of both the aortic and pulmonary root. She died in cardiac failure shortly after pacemaker implantation, due to dysrhythmia and severe mitral insufficiency. At autopsy, apart from myxomatous changes of the valves and dilated aortic and pulmonary roots, an aneurysm of the sinus of Valsalva of the pulmonary valve and abnormal myxomatous connective tissue surrounding the AV node were also found. Incidence, clinical characteristics, electrophysiological characteristics and outcomes of patients with baseline PR prolongation undergoing radiofrequency ablation for Atrioventricular nodal reentrant tachycardia.

AIMS AND OBJECTIVES: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common supraventricular tachycardia (SVT). Prolonged PR interval(>200 ms) on baseline electrocardiogram (ECG) is uncommon in such patients. The aim of the current study was to evaluate the incidence, clinical, electrophysiological characteristics, and outcomes of patients with baseline prolongation of PR interval undergoing radio-frequency ablation (RFA) for AVNRT. METHODS: Over 10 years, out of the total number of 1435 patients with diagnosed AVNRT, 16 patients had prolonged PR intervals at baseline. All un
