In the near future, the electronic health record (EHR) is likely to include patients' genetic information, in addition to other clinical information and administrative data. This warrants some preparatory thought. Presently, in many jurisdictions, adolescents can self-access their EHR, as an acknowledgment of their relative autonomy in areas concerning their health. The foreseeable routine integration of genetic information into the EHR could result, inter alia, in adolescents' inadvertent exposure to misattributed parentage findings emerging from genetic investigations, creating clinical chal

The medical humanities developed in the second half of the 20th century in response to challenges of mechanisation and dehumanisation in medicine. Humanities disciplines like literature and art and social sciences like anthropology and sociology served to ameliorate reductionistic tendencies in medicine and ensured that medical trainees developed a deeper understanding of patient experiences and perspectives. The medical humanities bequeathed to medicine a capacity for self-critique and highlighted ways in which medicine could provide more humane care to patients. One challenge, however, that

In this piece we examine three forms of coercive or otherwise involuntary care that can occur with patient consent. To do so, we examine: (1) uninformed consent, (2) contingency-based consent and (3) constrained-market consent, amongst female sterilization patients. While there is broad recognition that "coercion" in sterilization care can manifest beyond instances of overt force and clarity on what constitutes coercion within clinical care, this has not translated to accountability. The current practice of identifying coercion through discrete civil cases may facilitate a narrow understanding

Current conceptions of special responsibilities often adopt a narrow, individualistic lens that fails to consider the broader socio-relational context. In response to this gap, we propose a concept of relational responsibility that emphasises the interconnectedness of individuals and the wider societal context in which they exist. We posit that assigning relational responsibilities should not solely hinge on the voluntary nature of one's relationships, but rather on the intrinsic value of these connections, as determined by individuals who hold pertinent roles within those relationships or who

The triad of clinical signs, (extensive bilateral retinal haemorrhages, subdural haematoma, and encephalopathy) is regarded by some expert witnesses as pathognomonic proof that an infant was deliberately shaken and head injured (shaken baby syndrome / abusive head injury). However, that view is controversial since scientific evidence does not support the diagnostic accuracy of the triad. In contrast to previous cases, a Victorian Supreme Court jury found an accused not guilty of homicide of a one-month-old infant afflicted with the triad. Prosecution witnesses were heavily criticized for faili

This commentary shares my experiences of overcoming bias and negativity to lead an active, fulfilling, and meaningful life as someone with Down syndrome. With my parents' input and support, I dealt with various issues and challenges posed by societal misconceptions and outdated ideas. This commentary is enriched by narratives written by 12 parents of children with Down syndrome. Many of the authors have faced discrimination from healthcare professionals, educators, and policymakers. The stories discuss bias, stereotypes, negativity, and outdated ideas and information, showing the need for bett

This commentary discusses themes drawn from twelve narratives by parents of people with Down syndrome. While each parent described unique aspects of their children and of their family's experience, a common thread throughout was making room for the different ways in which their children experienced and navigated the world. They described their children's differences sometimes being honored and sometimes being devalued, and in their narratives consistently advocate for ways in which we can hold space for those differences. Healthcare institutions, schools, and family life are all sites where th

Prescribing antidepressants and benzodiazepines for patients with emotional distress is a common practice in primary healthcare that raises certain ethical questions. This paper has three aims. First, to describe the motivations that lead general practitioners to prescribe antidepressants and benzodiazepines in these cases. Second, to reflect on the ethical implications of such prescriptions based on the four principles of biomedical ethics defined by Beauchamp and Childress (autonomy, nonmaleficence, beneficence, and justice). Finally, to propose some recommendations for the mitigation of the

Racial classifications have a complex and troubled past in the social and scientific history of humankind. They are the result of racism and have been used to devalue and degrade non-White people. Although the concept of race has acquired a social component and the genetic similarity of people based on race has been proven, the category is increasingly used in pharmacogenetic studies to create biased study populations under the guise of personalized medicine. The heart failure drug BiDil, which was only approved for Black people in 2005, gained particular notoriety. Its race-specific approval

In Dobbs v. Jackson, the Supreme Court found that 'the Constitution does not confer a right to abortion.' Rather, individual states must determine whether a right to abortion exists. Following Dobbs, state abortion laws have diverged significantly. This has generated confusion over what the law permits. Consequently, some pregnant women reportedly have not received timely treatment for life-threatening conditions. Clear guidance on abortion policy is essential, therefore, since continued confusion risks lives. Sweeping calls to improve patient access to abortion will not provide clear guidance

Community Advisory Boards (CABs), as a form of community engagement, provide an important mechanism to ensure that research aligns with community needs and priorities by representing broader community interests and guiding research accordingly. Tuberculosis (TB) and multidrug-resistant tuberculosis (MDR-TB) remain significant public health concerns, particularly due to resistance to key first-line anti-TB medications, and individuals affected by these conditions often face stigma and discrimination that hinder timely diagnosis and treatment adherence. Addressing these challenges requires commu

In medical care, the obtaining of informed consent is taken to be required prior to treatment in order to ensure that patients sufficiently understand the potential risks and benefits of a given medical procedure. In this paper, I begin by looking at the history of informed consent and consider how the norms and laws in medicine have evolved away from benevolent paternalism and toward a blanket obligation to obtain informed consent. In so doing, I consider what values might be taken to underpin such a requirement. After dismissing some unsatisfactory answers, I offer a positive view as to the

There are ethical dilemmas faced by clinicians when responding to using unregistered medical devices, such as innovative internet technologies for managing type 1 diabetes mellitus. This chronic disease significantly impacts patients' health, requiring intensive daily activities like blood glucose monitoring, insulin injections, and specific dietary recommendations. Recent technological advances, including continuous glucose monitors and insulin pumps, have been shown to improve glycemic control. INTRODUCTION: Deep brain stimulation (DBS) is approved under a humanitarian device exemption to manage treatment-resistant obsessive-compulsive disorder (TR-OCD) in adults. It is possible that DBS may be trialed or used clinically off-label in children and adolescents with TR-OCD in the future. DBS is already used to manage treatment-resistant childhood dystonia. Evidence suggests it is a safe and effective intervention for certain types of dystonia. Important questions remain unanswered about the use of DBS in children and adolescents with TR-OCD, including whether mental health clinicians wo

In British Columbia, Canada, many physicians providing care to individuals with high-risk opioid use disorder adopted safer supply (SS) opioid prescribing in the spring of 2020 with the goal of facilitating public health measures for COVID-19. This prescribing practice continued after measures were lifted. This study aimed to explore prescribers' perspectives following several years of local experience in prescribing SS opioids, primarily in the form of hydromorphone tablets, and to apply ethical concepts to explore current challenges and ongoing sources of provider distress. Prenatal ultrasound is a non-invasive diagnostic examination. Despite the recognized diagnostic value, this technology raises complex ethical questions. The aim of this study is to provide a comprehensive analysis that coherently maps the ethical challenges raised by prenatal ultrasound examination, both 2D and 3D. We performed a systematic literature review. Six databases were systematically searched. The results highlight how concerns related to beneficence, informed consent, and autonomy are mainly related to routine use of prenatal ultrasound in the clinical context, while considerations l

Human organs available for transplant are in short supply. One way to increase the supply of organs consists in legalizing a live donor market. Such a market is, however, controversial. This article is about an objection to live donor organ markets made by Simon Rippon. Rippon's objection is that the presence of a market option creates new social and legal pressures that harm the poor. Legalizing the option of selling your organs transforms into a harmful, and morally indefensible, social, and legal pressure to sell on the financially desperate. This article defends the conclusion that Rippon'

The principle of non-directiveness remains an important tenet in genetics. However, the concept has encountered growing criticism over the last two decades. There is an ongoing discussion about its appropriateness for specific situations in genetics, especially in light of recent significant advancements in genetic medicine. Despite the debate surrounding non-directiveness, there is a notable lack of up-to-date international research empirically investigating the issue from the perspective of those who actually do genetic counselling. Addressing this gap, our article delves into the viewpoints
