Multimodal rehabilitation for faecal incontinence: experience of an Italian centre devoted to faecal disorder rehabilitation.

BACKGROUND: Sphincter exercises and biofeedback therapy have been used to treat faecal incontinence but results have been unpredictable and standards of treatment have not yet been established. The aim of this study was to retrospectively evaluate the effects of a new multimodal rehabilitation model on faecal incontinence. METHODS: All of the rehabilitative procedures are guided by manometric data. Primary study outcome criteria were the determination of changes or deterioration in incontinence, failure to achieve full continence and/or presence of faecal urgency. The clinical outcome was des

[Deglutition disorders in the elderly. Management].

GENERAL PRINCIPLES: The management of dysphagia requires multi-disciplinary interventions, implying various procedures, the choice of which depends on the results of the global prior assessment of the patient. General measures for oral hygiene, dental care and the organization of conditions in which meals are taken are essential, particularly for dependent patients. A change in food texture or in viscosity of liquids is a strategy commonly used. However, such changes must depend on objective bases and not unnecessarily penalize patients or expose them to further risks of complications. COMPENS

Parallel man-machine training in development of EEG-based cursor control.

A new parallel man-machine training approach to brain-computer interface (BCI) succeeded through a unique application of machine learning methods. The BCI system could train users to control an animated cursor on the computer screen by voluntary electroencephalogram (EEG) modulation. Our BCI system requires only two to four electrodes, and has a relatively short training time for both the user and the machine. Moving the cursor in one dimension, our subjects were able to hit 100% of randomly selected targets, while in two dimensions, accuracies of approximately 63% and 76% was achieved with ou

How does biofeedback reduce clinical symptoms and do memories and beliefs have biological consequences? Toward a model of mind-body healing.

Changes in the magnitude and direction of physiological measures (EMG, EEG, temperature, etc.) are not strongly related to the reduction of clinical symptoms in biofeedback therapy. Previously, nonspecified perceptual, cognitive, and emotional factors related to threat perception (Wickramasekera, 1979, 1988, 1998) may account for the bulk of the variance in the reduction of clinical symptoms. The mean magnitude of these previously nonspecified or placebo factors is closer to 70% when both the therapist and patient believe in the efficacy of the therapy. This powerful placebo effect is hypothes

The practical aspects of diagnosing and managing children with attention deficit hyperactivity disorder.

AD/HD is a behaviorally defined disorder with specific behavioral criteria. The most recent definitions decrease heterogeneity by defining subtypes although the current treatments tend to be more generic. The main well-established treatments are stimulant medication and behavior modification, which are most effective when used together.

Anal reeducation for postoperative fecal incontinence in congenital diseases of the rectum and anus.

From October 1993 to March 1996, 14 patients with anorectal disease were referred to an anal reeducation clinic. Initial evaluation allowed the authors to identify three classes of defects: lack of proprioception in the sphincters, use of synergistic muscles (gluteal) to compensate for anal dysfunction, and inversion of command by contraction, rather than relaxation, of abdominal muscles. Patients were treated by electrostimulation through an anal probe as well as biofeedback therapy coupled with home exercises. This therapy resulted in rapid correction of the abnormal motor commands and erron

[Hinman syndrome. Pronounced renal failure as a sequela of pseudo-neurogenic disorder of bladder emptying].

The case of an eight year old girl is presented who was seen because of secondary enuresis and recurrent urinary tract infection. Detailed examinations revealed urinary retention and sustained renal insufficiency (Creatinine clearance: 11 ml/min x 1.73 m2) due to a pseudo-neurogenic bladder. After improvement of renal function by continuous bladder drainage, bladder dysfunction was successfully treated by medication (Phenoxybenzamine, Baclofen), conventional physical therapy and biofeedback.

[Sleep induction].

Physiological sleep induction is a fragile phenomenon which may be altered by environmental factors and various somatic and psychiatric disorders. Therefore sleep induction disorder is a common component of different types of insomnia, including sleep-onset insomnia and to a lesser degree multiple awakenings insomnia and early morning awakening insomnia. The treatment of sleep induction disorders is difficult and requires a precise analysis of the disorder and its aetiological factors. Specific treatment of aetiological factors is more likely to be efficacious in the long term than pharmacolog

Trachea-noise biofeedback in asthma: a comparison of the effect of trachea-noise biofeedback, a bronchodilator, and no treatment on the rate of recovery from exercise- and eucapnic hyperventilation-induced asthma.

We review some of the evidence that supports the existence of psychosomatic triggers to bronchospasm in asthmatics, and hypothesize that it may also be possible to consciously reverse bronchospasm using trachea-noise biofeedback. We precipitated significant levels of bronchospasm in 16 asthmatics using exercise or eucapnic-hyperventilation challenges on five occasions, and administered four different treatments and a no-treatment control. The treatments were trachea-noise biofeedback (TNBF), wrong-information TNBF, an inhaled adrenergic bronchodilator, and a placebo inhaler, all given double b

A microcomputer-based wearable biofeedback device to improve transfer of treatment in parkinsonian dysarthria.

Achieving transfer of treatment outside the clinic is a problem for almost all individuals with speech disorders but has been particularly difficult in the treatment of patients with Parkinsonism. Given this situation and the frequency of vocal intensity problems in this population, we developed a wearable biofeedback device that could provide a patient with information about speech intensity outside the clinic. Auditory-perceptual and acoustic analyses were performed on audiotaped samples of reading and spontaneous speech recorded in the clinic pre- and posttreatment and at 10- and 20-week fo

Electromyographic biofeedback: behavioral treatment of neuromuscular disorders.

Electromyographic biofeedback is becoming widely used to help patients regain voluntary control of specific muscles affected by neuromuscular disorders. Electromyographic feedback training has been employed in the rehabilitation of patients affected by poliomyelitis, cerebrovascular accident, torticollis, nerve injury, temporomandibular joint syndrome, bruxism, and other disorders. While EMG biofeedback appears to be a promising treatment technique, the research literature on its effectiveness consists mainly of uncontrolled case reports and clinical trials. It is concluded that new studies wi

Laryngopharyngeal Symptoms and Laryngopharyngeal Reflux Disease: Interdisciplinary Considerations and Management.

BACKGROUND: The San Diego Consensus for Laryngopharyngeal Symptoms (LPS) and Laryngopharyngeal Reflux Disease (LRPD) describes a broad-based multidisciplinary management paradigm that focuses on mechanisms underlying symptoms to improve treatment outcomes. PURPOSE: This review expands on the San Diego Consensus framework to discuss multidisciplinary management of LPS and LPRD. LPS manifest as persistent or disproportionate symptoms despite minimal or inconsistent evidence of reflux exposure. Emerging evidence suggests that hyperresponsiveness, hypervigilance, and symptom-specific anxiety are

Barriers to the management of obstructed defaecation according to colorectal surgeons.

AIM: Obstructed defaecation (OD) has a high prevalence and high disease impact; however, patients often experience suboptimal management. This problem reflects the complex pathophysiology of OD as well as health service delivery factors. This study aimed to identify the factors that act as a barrier to effective management of OD as perceived by specialist colorectal surgeons treating this disorder. METHOD: A postal questionnaire was administered to a bi-national sampling of colorectal specialists in Australia and New Zealand who were registered with their specialty society. Questions addresse

Psychological interventions for treating foot ulcers, and preventing their recurrence, in people with diabetes.

BACKGROUND: Diabetic foot ulceration (DFU) can be defined as a full-thickness wound below the ankle and is a major complication of diabetes mellitus. Despite best practice, many wounds fail to heal, and when they do, the risk of recurrence of DFU remains high. Beliefs about personal control, or influence, on ulceration are associated with better engagement with self-care in DFU. Psychological interventions aim to reduce levels of psychological distress and empower people to engage in self-care, and there is some evidence to suggest that they can impact positively on the rate of wound healing.

Patient-specific breath-hold reproducibility in thoracic and abdominal radiotherapy: comparison of auditory and visual biofeedback.

BACKGROUND AND PURPOSE: This study aimed to compare patient-specific reproducibility and comfort of breath-holding using auditory and visual biofeedback guidance in a respiratory monitoring system. MATERIALS AND METHODS: A respiratory monitoring device that tracks abdominal and chest wall motion was used to provide auditory and visual feedback. Forty patients with thoracic or abdominal tumors underwent three computed tomography scans under each guidance method. Breath-hold reproducibility was assessed by measuring the distance between an anatomical landmark and the tumor. Patient preferences

Effect of 4 Weeks Resonance Frequency Breathing on Glucose Metabolism and Autonomic Tone in Healthy Adults.

BACKGRUOUND: The autonomic nervous system plays a crucial role in the brain's communication with metabolically important peripheral organs, modulating insulin sensitivity and secretion. Increased sympathetic tone is a common feature in prediabetes and diabetes. The parasympathetic nervous system activity might be improvable through resonance frequency breathing (RFB) with heart rate variability biofeedback (HRV-BF) training. METHODS: We here investigated the effect of a 4-week mobile RFB-HRV-BF intervention on glucose metabolism and HRV of 30 healthy adults (17 females; mean age 25.77±3.64 ye

Additional benefits of pelvic floor proprioceptive training combined with conventional therapy in the treatment of female stress urinary incontinence.

OBJECTIVES: Stress urinary incontinence (SUI) is a common condition among women that severely impairs quality of life. Pelvic floor proprioceptive training (PFPT) has attracted increasing attention for its potential to enhance pelvic floor muscle function and alleviate SUI symptoms. This study aims to observe and compare the clinical efficacy of PFPT combined with electroacupuncture, electrical stimulation, and biofeedback therapy versus conventional therapy consisting of electroacupuncture, electrical stimulation, and biofeedback alone in women with SUI, and to explore the role of PFPT in imp

Identifying behaviour change techniques within precision health interventions that use continuous glucose monitoring: a secondary analysis of a scoping review.

BACKGROUND: Continuous glucose monitoring (CGM) is increasingly being used within precision health interventions to motivate behaviour change. However, there is considerable variability and complexity in the design of behaviour change interventions that incorporate CGM-based biofeedback, making it challenging to disentangle the intervention components that are driving intervention effects. The objective of this review is to identify the behaviour change techniques and mechanisms of action commonly implemented alongside CGM-based biofeedback. METHODS: We conducted secondary analyses of a scopi

Comparing the effectiveness of robotic plantarflexion resistance and biofeedback between overground and treadmill walking.

Individuals with diminished walking performance caused by neuromuscular impairments often lack plantar flexion muscle activity. Robotic devices have been developed to address these issues and increase walking performance. While these devices have shown promise in their ability to increase musculature engagement of the lower limbs when used on a treadmill, most have not been developed or validated for overground walking and community use. Overground walking may limit the effectiveness of robotic devices due to differences in gait characteristics between walking terrains and reduced user engagem
