Interaction of click intensity and cochlear hearing loss on auditory brain stem response wave V latency.

Auditory brain stem responses (ABRs) to 95, 80, 60, 40, and 30 dB nHL clicks were retrospectively studied from 103 patients (194 ears) with various degrees of cochlear impairment. Hearing loss and sample size were balanced across gender. Results indicate that the slope of the wave V latency versus 4000 Hz hearing loss function doubles as click intensity is decreased from 80 (0.01 msec/dB HL) to 60 nHL (0.02 msec/dB HL). Overall results indicate a slope increase of 0.0004 msec for each decibel decrease in click intensity from 95 to 30 dB nHL. Intersubject variability increased with increased he…

Inconsistencies in the correlation between loss of brain stem auditory evoked response waves and postoperative deafness.

This case underscores the difficulty of predicting postoperative hearing status from brain stem auditory evoked response (BAER) monitoring when wave I is preserved and all later waves are lost. During an operation involving the base of the skull, sudden and irreversible loss of all BAER waves beyond wave I occurred unilaterally. Wave I was preserved, with reduced amplitude and minimal latency shift. There was no permanent postoperative hearing sensitivity loss or speech discrimination loss.

Vector analysis of brain-stem auditory evoked potentials in patients with multiple sclerosis and subtentorial tumors.

Vector analysis of BAEPs was done in 10 patients with posterior fossa tumors and 14 patients with MS. Latency abnormalities were found in both groups without significant differences. However, deviations of wave V vectors from its normal orientation were observed in tumor cases, correlated with tumor size and latency increase. It is concluded that vector deviations may indicate distortion of auditory pathways in the brain-stem.

[Millard-Gubler syndrome in a young man after hemorrhage from arteriovenous hemangioma to the brain stem].

A young man is described in whom in full health sudden violent headaches and loss of consciousness developed. After admission to hospital a complex of clinical signs was observed indicating brain stem lesion. Computerized tomography of the head and vertebral arteriography demonstrated arteriovenous haemangioma on the left side of the brain stem. The malformation was supplied by the anterior inferior cerebellar artery. The patient did not consent to operation, after several months of rehabilitation his clinical condition improved greatly.

Trismus in postoperative, posttraumatic and other brain stem lesions caused by paradoxical activity of masticatory muscles.

In fifteen cases of brain stem lesions caused by basal tumours, operations, injuries, arteriosclerosis or multiple sclerosis, a paradoxical activity of jaw closing muscles during jaw opening was observed. This inverse activity was the cause of severe trismus in 8 cases. It is the manifestation of a faulty programming of jaw movements in the trigeminal motor area in brain stem. This distinct new brain stem syndrome can only be detected by EMG investigation. EMG findings, pathological background, outcome and treatment possibilities are discussed in this paper.

[Involvement of serotoninergic systems in analgesia induced by electrical stimulation of brain stem areas (author's transl)].

Numerous studies in the rat have shown that powerful analgesia can be induced by electrical stimulation of the periaqueductal grey matter (PGM). From an extensive mapping (300 sites of stimulation) performed on unrestrained cats, we have demonstrated that the points from which analgesia can be obtained are essentially located in the raphe nuclei: dorsal raphe nucleus (DRN) located in the ventral part of the PGM, superior part of the nucleus central superior (CS) and nucleus central inferior (mainly the raphe magnus: RM). The most powerful effects were induced by stimulation sites located in th

Brain stem auditory evoked potentials are related to interaural time discrimination in patients with multiple sclerosis.

In a combined psychophysical-electrophysiological study on 29 patients with multiple sclerosis (MS), a compromised ability to make interaural time discriminations was nearly always found to be associated with 'abnormal' brain stem potentials evoked by clicks to at least one ear. However, no obvious relationships were found between evoked brain stem potentials and several other auditory behavioral measures such as interaural intensity discrimination, pure-tone thresholds and speech discrimination.

Time course of 5,6-dihydroxytryptamine neurotoxic effects on fowl diencephalon and upper brain stem monoaminergic pathways.

In adult fowls the time-course of the neurotoxic effects of 5,6-DHT on indolaminergic neurons (in comparison with controls and with animals pre-treated with mebanazine, a MAO-inhibitor) was studied. It was shown that 5,6-DHT given into the third cerebral ventricle produced a rather selective degeneration of indolaminergic neurons which was most dramatic 4 and 7 days after the injection. Injured axons characterized by a pile-up of strongly yellow fluorescent material were seen in areas bordering the third ventricle, and in the posterior, medium and anterior hypothalamus, preoptic area, median e

[Intracerebral transplantation of human umbilical cord-derived mesenchymal stem cells in neonatal rat model of hypoxic-ischemic brain damage: protective effect to injured brain].

OBJECTIVE: To study the brain protection and the possible mechanism of human umbilical cord-derived mesenchymal stem cells (hUC-MSCs) in neonatal rat model of hypoxic-ischemic brain damage (HIBD). METHODS: Successfully establishing a neonatal rat model of HIBD, hUC-MSCs labeled with BrdU were transplanted into the lateral ventricle 24 hours after HIBD. The number of apoptotic cells and the expression of Caspase-3 were detected by TUNEL and Western blot respectively at 24 and 48 hours after transplantation. The neurological functions of HIBD rats were evaluated by Longa score, and the survival

Mesenchymal Stem Cell (MSC)-based therapies for neonatal lung and brain injury - one size fits all?

MSC-based therapies have emerged as a highly promising approach for two of the most devastating consequences of perinatal insults such as extreme prematurity and perinatal asphyxia: bronchopulmonary dysplasia (BPD) and neonatal brain injury. Both conditions share a common pathophysiological basis of inflammation, oxidative stress, and disrupted progenitor cell function in critical windows of organ development. Exogenous MSC act primarily through their secretome, releasing extracellular vesicles, growth factors, and immunomodulatory mediators that attenuate injury and support organ development.

Psychological stress drives aging-like hematopoietic stem cell dysfunction through a brain-gut-bone marrow axis.

Chronic stress influences hematopoietic stem cells (HSCs). However, how psychological stress regulates HSC function remains incompletely understood. Here, we show that psychological stress impairs HSC self-renewal and lymphoid differentiation, inducing aging-like phenotypes. Stress suppresses neuronal activity in the medial prefrontal cortex (mPFC) and periaqueductal gray (PAG), leading to HSC dysfunction, whereas chemogenetic activation of these regions restores HSC function. Psychological stress or chemogenetic inhibition of the mPFC and PAG reduces the abundance of L. reuteri in the gut mic

Mesenchymal stem cell-derived secretome in traumatic brain injury: Stage-specific paracrine mechanisms and translational challenges.

Traumatic brain injury (TBI) is a stage-dependent disorder that evolves from acute neuroinflammation, blood-brain barrier (BBB) disruption, oxidative stress, excitotoxicity, and apoptosis to subacute neurovascular remodeling and chronic impairment of neuroregeneration and circuit repair. Current clinical management improves survival by stabilizing physiological status and limiting secondary injury, but it rarely rebuilds damaged neural networks or restores long-term neurological function. Mesenchymal stem cells (MSCs) have therefore attracted attention as regenerative candidates, with increasi

Ultrasound Activated Piezoelectric Dural Patches to Drive Endogenous Neural Stem Cell-Mediated Repair Traumatic Brain Injury.

Endogenous neuronal differentiation of neural stem cells (NSCs) is a promising route to restore function after traumatic brain injury (TBI), but direct transplantation of exogenous NSCs faces practical and immunological barriers and yields limited neuronal maturation. Here, a clinically relevant strategy is reported that converts a dura mater into an active piezoelectric patch to noninvasively drive endogenous NSC neurogenesis. Electrospun poly(L‑lactic acid) (PLLA) patches were subjected to surface confinement crystallization on metal substrates, producing a metastable α' crystal structure an

Efficacy and safety of exosomes from Wharton's Jelly-derived mesenchymal stem cells in traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) is a significant public health issue, leading to long-term neurological impairments. Current treatments offer limited recovery, particularly in restoring lost functions. Mesenchymal stem cell-derived exosomes (MSCdE) have shown potential for promoting neuroprotection and regeneration. This study evaluates the safety and efficacy of MSCdE therapy in TBI patients. AIM: To evaluate the safety and efficacy of MSCdE therapy in TBI patients. METHODS: Five patients (mean age 27.00 ± 4.06 years) with TBI from combat injuries were treated with six rounds of MS

Neuroimmune Microenvironment Reprogramming via Immuno-piezoelectric Transducers for Synergistic Stem Cell Therapy in Traumatic Brain Injury.

Secondary traumatic brain injury (TBI) induces a pro-inflammatory microenvironment that hampers neural stem cells (NSCs) therapy and tissue regeneration. To address this challenge, an immuno-piezoelectric transducer have been developed to create an anti-inflammatory immune microenvironment, deliver wireless electrical stimulation, and facilitate multimodal NSCs therapy. The immuno-piezoelectric transducer drives the polarization of microglia towards the anti-inflammatory M2 phenotype and secretion of anti-inflammatory cytokines. This modulation significantly reduces the inflammatory response,

Translational Potential of Stem Cell-based Therapies in the Treatment of Neonatal Hypoxic-ischemic Brain Injury.

Neonatal hypoxia-ischemia is a leading cause of neonatal death in both developed and developing countries. The consequences of hypoxic-ischemic injury affect the rest of the child's life, often resulting in intellectual or motor disability that persists into adulthood. To date, therapeutic hypothermia (TH) appears to be the only available intervention aimed at limiting brain injury and is recognized as the "gold standard" in neonatal intensive care. The basic mechanisms of neuroprotection achieved by temporal cooling involve the reduction of free radical activity, suppression of the inflammato

18F-FDG PET/CT findings in fatal Balamuthia Mandrillaris encephalitis in brain stem: A case report.

We presented a case of a 66-year-old female whose initial symptom was headache without obvious inducement. The patient's condition progressed rapidly to a semi-coma state after symptomatic treatment. The 18F-FDG PET/CT scan revealed circular FDG hypermetabolism and central metabolic defect of the pons and left frontal lobe lesions. The combination of clinical findings, MRI, and Metagenomic next-generation sequencing (NGS) of cerebrospinal fluid led to the diagnosis of Balamuthia mandrillaris encephalitis. The patient died 5 days after discharge.

Mesenchymal stem cell-loaded hydrogel to inhibit inflammatory reaction in surgical brain injury via mitochondria transfer.

Neurosurgical procedures are the key therapeutic interventions for the cerebral hemorrhage and brain tumors. However, neurosurgical procedures inevitably cause surgical brain injury (SBI), which will induce hemorrhage and inflammation. Gelatin Sponges are still the primary hemostatic materials used in clinical, but their anti-inflammatory efficacy is poor. Herein, we developed a cross-linked gelatin hydrogel (GelMA) to load mesenchymal stem cells (MSC) and directly implant them to the SBI site. Upon contacting the SBI site, the GelMA showed better clotting performance than Gelatin Sponges. Mor
