Electrophysiologic evaluation of syncope in patients with bifascicular block.

Thirteen patients with syncope and bifascicular block were evaluated by electrophysiologic study (EPS) including programmed stimulation. The mean age was 62 years. Six patients had coronary artery disease, three had cardiomyopathy, and four showed no evidence of organic heart disease. Holter monitoring and neurologic evaluation were nondiagnostic in all patients prior to EPS. EPS demonstrated inducible ventricular tachycardia (VT) in four patients, an HV interval greater than or equal to 70 msec in four, intra- and infra-His block with atrial pacing in one, and was nondiagnostic in four patien

Pre-excitation induced left ventricular dysfunction: A less known cause of cardiomyopathy in children.

A 7-year-old boy was diagnosed to have dilated cardiomyopathy with severe left ventricular (LV) dysfunction at 1 year of age. Although well compensated, his LV function remained impaired over the follow-up years. His electrocardiogram (ECG) was initially misinterpreted as left bundle branch block. However, on follow-up, it was recognized as a pre-excited ECG. The accessory pathway causing pre-excitation was implicated as the cause of LV dysfunction and dysynchrony as the mechanism. Radiofrequency ablation was done. His LV function and dimensions improved significantly over 6 months follow up.

Fatal heart block due to mesothelioma of the atrioventricular node.

A fit young man of 23 was symptom free until the time of his death despite a narrow complex complete heart block, with resting heart rates down to 35 beats/min, that was first diagnosed when he was 10. The clinical diagnosis remained congenital heart block. Necropsy showed extensive infiltration of the atrioventricular node and proximal bundle by mesothelioma tissue. Pacing had not been advised because of his excellent exercise tolerance.

Lyme carditis with isolated left bundle branch block and myocarditis successfully treated with oral doxycycline.

Lyme disease may present with a variety of cardiac manifestations ranging from first degree to third degree heart block. Cardiac involvement with Lyme disease may be asymptomatic, or symptomatic. Atrioventrical conduction abnormalities are the most common manifestation of Lyme carditis. Less common, are alternating right bundle branch block (RBBB) and left bundle branch block (LBBB). We present an interesting case of a young male whose main manifestation of Lyme carditis was isolated LBBB. He also had mild Lyme myocarditis. The patient was successfully treated with oral doxycycline, and his is

Reverse BBRVT in a structurally normal heart.

We present a case of bundle-branch reentry ventricular tachycardia with RBBB morphology (reverse BBRVT) in a patient with preexisting right bundle-branch block and without apparent structural heart disease. The diagnosis of BBRVT may be very challenging due to the similar wide QRS morphology during tachycardia and in sinus rhythm, rather suggesting a supraventricular origin. BBRVT should be suspected even in the absence of structural heart disease, but especially in patients with extensive underlying His-Purkinje conduction disorders. The aetiology, mechanism, diagnostic issues and treatment ar

A microcomputer system for on-line study of atrioventricular node accommodation.

An automated on-line programmable stimulator and interval measurement system was developed to study atrioventricular node (AVN) accommodation. This dedicated microcomputer system measures and stores the stimulus-to-His bundle (S-H) interval from His bundle electrogram (HBE) recordings. Interval measurements for each beat are accurate to within 500 microsecond. This user-controlled system has been used to stimulate at any rate up to 6.5 Hz and to measure intervals up to 125 ms in isolated perfused guinea pig hearts. A built-in timer-reset mechanism prevents failure of the system in the absence

Electrophysiological effects of lidocaine in acute myocardial infarction with bifascicular block or complete A-V block.

Electrophysiological effects of lidocaine were studied in 27 patients with acute myocardial infarction complicated by bifascicular block (group I: 20), and complete A-V block (group II: 7). Lidocaine was administered intravenously in bolus doses of 100 mg each at intervals of 10 min. In group I, there was no significant change in the heart rate (Before (B) = 84.85 +/- 24.19, After (A) = 87.25 +/- 20.26 beats/min) intra-atrial (PA) conduction time (B = 25 +/- 6.18, A = 27.22 +/- 6.69 ms), A-V nodal (AH) conduction (B = 111.5 +/- 56.12, A = 111.5 +/- 56.5 ms) or His bundle to ventricular (HV) ac

[Comparing the impact of left bundle branch area pacing and traditional left ventricular pacing on right heart function following dual-chamber pacemaker implantation].

Objective: To compare the effects of left bundle branch area pacing (LBBaP) versus traditional right ventricular pacing (RVP) on left ventricular function in patients after dual-chamber pacemaker implantation. Methods: A retrospective cohort study was conducted on patients who underwent dual-chamber pacemaker implantation from March 2017 to April 2021 in Beijing Anzhen Hospital. The patients were divided into the LBBaP group and RVP group based on the placement of the ventricular lead. Follow-up was conducted until March 2022, comparing baseline and follow-up echocardiographic parameters, paci

Optimization of the atrioventricular delay in conduction system pacing.

INTRODUCTION: In patients receiving conduction system pacing (CSP), it is not well established how to program the sensed atrioventricular delay (sAVD), with respect to the type of capture obtained (selective, nonselective His-bundle [HB] capture or left bundle branch [LBB] capture). The aim of this study was to acutely assess the effectiveness of an electrophysiology (EP)-guided method for sAVD optimization by comparing it with the echocardiogram-guided optimization. METHODS AND RESULTS: Consecutive patients undergoing HB or LBB pacing were enrolled. The EP-guided sAVD was defined as the sAVD

How to ablate the septo-pulmonary bundle: a case-based review of percutaneous ablation strategies to achieve roof line block.

Electrical conduction through cardiac muscle fibres separated from the main myocardial wall by layers of interposed adipose tissue are notoriously difficult to target by endocardial ablation alone. They are a recognised important cause for procedural failure due to the difficulties of delivering sufficient energy via the endocardial radiofrequency catheter to reach the outer epicardial layer without risking adverse events of the otherwise thin walled atria. Left atrial ablations for atrial fibrillation (AF) and tachycardia are commonly affected by the presence of several epicardial structures,

New-onset atrial fibrillation following left bundle branch area pacing vs. right ventricular pacing: a two-centre prospective cohort study.

AIMS: To investigate whether left bundle branch area pacing (LBBAP) can reduce the risk of new-onset atrial fibrillation (AF) compared with right ventricular pacing (RVP). METHODS AND RESULTS: Patients with indications for dual-chamber pacemaker implant and no history of AF were prospectively enrolled if they underwent successful LBBAP or RVP. The primary endpoint was time to the first occurrence of AF detected by pacemaker programming or surface electrocardiogram. Follow-up at clinic visit was performed and multivariate Cox regression models were applied to evaluate the effect of LBBAP on ne

Impact on right ventricular performance in patients undergoing permanent pacemaker implantation: Left bundle branch pacing versus right ventricular septum pacing.

BACKGROUND: The novel method of left bundle branch pacing (LBBP) has been reported to achieve better electrical and mechanical synchrony in the left ventricle than conventional right ventricular pacing (RVP). However, its effects on right ventricle (RV) performance are still unknown. METHODS: Consecutive patients undergoing dual-chamber pacemaker (PM) implantation for sick sinus syndrome (SSS) with normal cardiac function and a narrow QRS complex were recruited for the study. The pacing characteristics and echocardiogram parameters were measured to evaluate RV function, interventricular and R

Pleomorphic ventricular tachycardia originating from Purkinje fiber network of left anterior fascicle.

A 55-year-old woman with recurrent syncope and palpitation experienced polymorphic ventricular tachycardia (VT) and more than 3 monomorphic VTs with a right bundle branch block configuration as inferior, middle, and superior axis. During the pleomorphic VT, the diastolic potential (dp) was recorded at the anterolateral left ventricle. Changes in the QRS morphology were associated with the time between dp and onset of QRS complex (dp-V interval), and prolongation of dp-V interval terminated the VT. In addition, the delayed potentials were seen during sinus rhythm around this area. Delivery of r

Frequency dependent shortening of conduction time through the reentrant pathway during transient entrainment of ventricular tachycardia.

In a patient with nonischemic ventricular tachycardia (VT), VT was entrained and the conduction time from the pacing site to the entrained local electrogram showed a rate dependent shortening and its degree affected by the pacing site. The QRS complex, which was entrained by the last pacing stimulus, was constant and identical to that of VT and no rate dependent facilitated conduction was observed when the heart was paced at similar paced cycle lengths during sinus rhythm. As the mechanism of the shortening of the conduction time through the reentrant circuit, a shift of the entrance seems mos

Anterolateral muscle bundle of the left ventricle in atrioventricular septal defect: left ventricular outflow tract and subaortic stenosis.

The anatomy of the left ventricular outflow tract (LVOT) in 77 hearts with atrioventricular septal defect (AVSD), 36 with a separate A V orifice and 41 with a common A V-orifice, were investigated. In all specimens, an anterolateral muscle bundle of the left ventricle was identified between the superior bridging leaflet and the left coronary aortic cusp. It displaced the attachment of the superior bridging leaflet, resulting in its clockwise rotation. The muscle bundle frequently bulged into the LVOT, but was never prominent enough to have caused significant subaortic stenosis. Measurement of
