Resting hemodynamics after total versus standard orthotopic heart transplantation in patients with high preoperative pulmonary vascular resistance.

OBJECTIVE: Pretransplant pulmonary vascular resistance > or = 4 Wood-units predisposes to right ventricular failure after heart transplantation. Total orthotopic heart transplantation with bicaval and pulmonary venous anastomoses offers synchronous contractions of the atria and a normal ventricular filling pattern, but requires longer ischemic time than standard orthotopic heart transplantation. To test if total orthotopic heart transplantation improves resting hemodynamics in pts with high preoperative pulmonary vascular resistance, we analyzed 65 pts with standard and 65 with total orthotopi

Vein of Galen Aneurysmal Malformation: A Case Report With Insights Into Radiological Diagnosis Using Ultrasonography and Magnetic Resonance Imaging.

This report details the comprehensive radiological evaluation of a vein of Galen aneurysmal malformation in a neonate, using ultrasonography and magnetic resonance imaging. Vein of Galen malformation is a rare, congenital cerebral arteriovenous fistula and the leading cause of life-threatening high-output cardiac failure in neonates. A late preterm male infant presented with worsening tachypnea and generalized body swelling. Initial cranial ultrasonography revealed a large, midline anechoic structure with intense flow on color Doppler. Subsequent multiplanar magnetic resonance imaging and magn

Development of a novel therapy for systolic heart failure.

Heart failure presents a critical health challenge with a 5-year mortality rate of up to 50%. Conventional treatments often lead to bradycardia or hypotension due to their impact on patient hemodynamics. To address this issue, we utilized high-throughput drug screening combined with structure-activity relationship-based medicinal chemistry to develop a novel drug-like compound that effectively blocks the β-adrenergic receptor (β-AR) mediated apoptosis pathway. This compound demonstrated both safety and efficacy in pre-clinical mouse models without adversely affecting cardiac output. Through th

Right-to-left shunt due to iatrogenic atrial septal defect manifested by aorto-caval fistula: a case report.

BACKGROUND: An aorto-caval fistula is a rare but critical complication of abdominal aortic aneurysm (AAA) rupture, leading to high-output heart failure and increased venous pressure. The anesthetic management of such cases, particularly when complicated by an intraoperative right-to-left shunt, is seldom reported. CASE PRESENTATION: A 71-year-old man with a history of atrial fibrillation and catheter ablation presented with heart failure and abdominal pain, leading to cardiac arrest. Imaging revealed an AAA rupture into the inferior vena cava. During emergency surgery, severe venous bleeding

Diffuse neonatal hemangiomatosis in a very low-birthweight infant treated with erythropoietin.

Diffuse neonatal hemangiomatosis (DNH) is a rare condition characterized by the concomitant development of multiple cutaneous infantile hemangiomas (IH) and visceral hemangiomas. Recently, an association between erythropoietin treatment and an increased incidence of infantile hemangioma was noted. A Japanese male infant was born via cesarean section at 27 weeks of gestation. Following the commencement of erythropoietin treatment for anemia of prematurity, he developed multiple cutaneous hemangiomas, high cardiac output heart failure and hepatomegaly. Abdominal imaging indicated comorbidity of

High-output heart failure caused by a huge renal arteriovenous fistula after nephrectomy: report of a case.

Postnephrectomy renal arteriovenous fistula (AVF) with an aneurysmal lesion is a rare clinical entity that may cause high-output heart failure. In this report, we describe the case of a 68-year-old man who had undergone nephrectomy for renal tuberculosis 43 years previously, in whom an acquired large renal AVF presenting as an aneurysm caused congestive cardiac failure. We also discuss the hemodynamic, hormonogenic (human arterial natriuretic polypeptide; hANP), and radiographic findings before and after surgery for the AVF. The AVF with an aneurysmal lesion was clearly visualized by three-dim

[Fluid overload in critically ill patients. Hemodynamic effect vs hydrosaline balance].

The administration of high volumes of fluids during resuscitation of critically ill patients follows the recommendations of experts' committees that postulate volume expansion to correct deficits that may be associated with certain conditions (sepsis, trauma, surgery, etc), not primarily characterized by hypovolemia. These recommendations suggest that optimizing cardiac output may avoid occult deficit in tissue oxygen delivery and consumption that could lead to multisystem organic failure and death. The lack of benefit in the morbidity and mortality rates of patients with treatments directed t

Anesthetic implications of the child with homocystinuria.

We present a 7 1/2-year-old girl with type I homocystinuria who required anesthetic care during repair of bilateral ectopia lentis. Homocystinuria is an autosomal recessive disorder and represents the second most common error of amino acid metabolism. Defective function of the enzyme cystathionine synthetase results in elevated serum levels of methionine and homocysteine. Physiologic disturbances include an increased risk of thromboembolic events and hypoglycemia. Perioperative measures to prevent thromboembolic events include diet therapy to lower serum methionine and homocysteine levels, ade

Preoperative CT in symptomatic abdominal aortic aneurysms: accuracy and efficacy.

Of 80 patients who underwent operations for abdominal aortic aneurysm at the Gainesville Veterans Administration Medical Center, 23 had a preoperative CT scan. Twenty patients in this group were symptomatic. In determining the presence of rupture, there was only one false-negative examination and no false-positives for a sensitivity of 0.9 and a specificity of 1.0. Nine of the 20 symptomatic patients had a rupture confirmed at laparotomy, and these patients had a 22 per cent mortality. Unruptured symptomatic patients operated upon after an expeditious preparation (usually about 36 hr), includi

Pediatric acute myocarditis: current evidence and practical recommendations.

Acute myocarditis (AM) in children and adolescents remains a diagnostic and therapeutic challenge because of its heterogeneous presentation, overlap with dilated cardiomyopathy, and potential for fulminant deterioration, with approximately 5.9% risk of early mortality. We performed a narrative review integrating contemporary guidelines, consensus documents, and weighted analyses from systematically selected large pediatric cohorts to summarize current evidence on epidemiology, diagnosis, risk stratification, and treatment. Pediatric AM shows a male predominance and a bimodal age distribution.

Exercise capacity is related to attenuated responses in oxygen extraction and left ventricular longitudinal strain in asymptomatic type 2 diabetes patients.

AIMS: Type 2 diabetes mellitus (T2DM) is associated with reduced exercise capacity and cardiovascular diseases, both increasing morbidity and risk for premature death. As exercise intolerance often relates to cardiac dysfunction, it remains to be elucidated to what extent such an interplay occurs in T2DM patients without overt cardiovascular diseases. Design: Cross-sectional study, NCT03299790. METHODS AND RESULTS: Fifty-three T2DM patients underwent exercise echocardiography (semi-supine bicycle) with combined ergospirometry. Cardiac output (CO), left ventricular longitudinal strain (LS), ox

Increased Intra-abdominal Pressure Induces Acute Kidney Injury in an Experimental Model of Congestive Heart Failure.

BACKGROUND: Congestive heart failure (CHF) entails a complex interaction between the heart and the kidney that represents a clinical entity called cardiorenal syndrome (CRS). One of the mechanisms underlying CRS includes increased intra-abdominal pressure (IAP). We examined the effect of elevated IAP on kidney function in rats with low- and high-output CHF. METHODS AND RESULTS: Rats with compensated and decompensated CHF induced by means of aortocaval fistula, rats with myocardial infraction (MI) induced by means of left anterior descending artery ligation, and sham control rats were subjecte

Arterial hypertension and chronic liver disease.

This review looks at the alterations in the systemic haemodynamics of patients with chronic liver disease (cirrhosis) in relation to essential hypertension and arterial hypertension of renal origin. Characteristic findings in patients with cirrhosis are vasodilatation with low overall systemic vascular resistance, high arterial compliance, increased cardiac output, secondary activation of counterregulatory systems (renin-angiotensin-aldosterone system, sympathetic nervous system, release of vasopressin), and resistance to vasopressors. The vasodilatory state is mediated through adrenomedullin,

[Diuresis as an index of the circulatory status during open-heart operations].

An intra-operative investigation of 24 patients showed glomerular filtration (GF) to be closely related to the state of systemic circulation. In cardiosurgical patients, GF is fairly accurate indicator of renal flow and microcirculation of the kidney. Without extracorporeal circulation (EC), decreased GF was associated with the presence of conditions resulting in reduced cardiac output, in spite of satisfactory arterial and central venous blood pressure values, and during EC, it was accompanied by hypotension even in the presence of high volumetric perfusion rate. Combined high-frequency ventilation for management of terminal respiratory failure: a new technique.

Seven patients with severe adult respiratory distress syndrome (ARDS) developed terminal respiratory failure and severe hypoxemia (PaO2 below 50 mm Hg) with death imminent despite maximal ventilatory support with intermittent positive-pressure ventilation (IPPV) and positive end-expiratory pressure (PEEP). High-frequency positive-pressure ventilation (HFPPV) was used in these patients for one day at a rate of 250 breaths/min, with slight improvement of PaO2 to a mean of 80 mm Hg. High-frequency oscillatory (HFO) ventilation was used during the second day at a rate of 2000 breaths/min; this pro

Acute mental stress-induced adrenergic reactivity profiles predict distinct cardiometabolic risk trajectories over three years: the SABPA study.

OBJECTIVES: We previously characterised acute mental stress-induced alpha (α)- and beta (β)-adrenergic-haemodynamic reactivity profiles cross-sectionally and related them to unique cardiometabolic risk outcomes. Whether these profiles predict future cardiometabolic risk remains unclear. We compared three-year cardiometabolic risk outcomes between predominant α- and β-responders and evaluated the prognostic value of each profile for predicting cardiometabolic risk. METHODS: Participants (aged 20-65 years) were stratified at baseline as predominant α- (n = 43) or β-responders (n = 59) defined b

The use of the RenalGuard system in cardiac surgery with cardiopulmonary bypass: a first in man prospective, observational, feasibility pilot study.

OBJECTIVES: As proof of concept, this prospective, observational study assessed the feasibility and early clinical outcomes of performing on-pump cardiac surgery with the RenalGuard system. BACKGROUND: Acute kidney injury (AKI) is reported in up to 30% of patients undergoing cardiac surgery and is a recognised independent predictor of both morbidity and mortality. Forced diuresis with the RenalGuard system reduces the incidence of AKI during percutaneous coronary intervention procedures but its use in cardiac surgery has not been explored. METHODS: Ten consecutive patients who were at risk o

Association of the central venous-to-arterial carbon dioxide difference with low cardiac output-related outcomes after cardiac surgery in children: a prospective cohort study.

INTRODUCTION: Low-cardiac-output syndrome (LCOS) after cardiac surgery may lead to poor postoperative outcomes. The venous-to-arterial carbon dioxide partial pressure difference (VACO2) showed association with poor outcomes in adults with cardiac surgery, but it's validity in pediatric population is uncertain. We evaluated the association of VACO2 with LCOS-related outcomes and the correlation with other surrogate markers such as lactate levels and oxygen extraction ratio. METHODS: This prospective cohort study was conducted at an intensive care unit in a tertiary academic hospital. Children

Effect of head-down-tilt bed rest and hypovolemia on dynamic regulation of heart rate and blood pressure.

Adaptation to head-down-tilt bed rest leads to an apparent abnormality of baroreflex regulation of cardiac period. We hypothesized that this "deconditioning response" could primarily be a result of hypovolemia, rather than a unique adaptation of the autonomic nervous system to bed rest. To test this hypothesis, nine healthy subjects underwent 2 wk of -6 degrees head-down bed rest. One year later, five of these same subjects underwent acute hypovolemia with furosemide to produce the same reductions in plasma volume observed after bed rest. We took advantage of power spectral and transfer functi

Unveiling the hidden risk: a case of severe jaundice triggered by intra-aortic balloon pump after cardiac surgery.

BACKGROUND: The Intra-Aortic Balloon Pump (IABP) is a widely utilized technique to provide circulatory support for critically ill patients with cardiac dysfunction. While IABP therapy offers clinical benefits, the placement of an IABP catheter can potentially lead to a range of complications in certain high-risk patients. This case report presents a rare instance of progressive jaundice associated with IABP implantation. CASE PRESENTATION: The patient was a 56-year-old female who had undergone cardiac valve surgery due to rheumatic combined valve disease. She subsequently developed postoperat
