In this Viewpoint, we highlight a possible hemodynamic problem arising following tricuspid valve replacement (TVR) in patients with severe chronic tricuspid regurgitation, represented by "unmasking" of pulmonary hypertension (PH) following the surgery. Persistent left superior vena cava (PLSVC) represents the most common thoracic venous anomaly and is an important clinical entity for cardiologists and electrophysiologists, among others. Enhanced external counterpulsation (EECP) is known to reduce angina pectoris in patients in whom revascularization is not possible. The therapy is associated with few adverse effects. A case with a previously unknown complication - toxic shock syndrome - that occurred twice in an EECP-treated patient is described. Toxic shock syndrome initially resembles the state of septic shock. Cardiac disease in pregnancy is the most important non-obstetric cause of maternal death; such patients should be assessed by a cardiologist before considering pregnancy. Once pregnant, the patient is managed by a combined cardiac and obstetric team familiar with the special risks associated with heart disease in pregnancy. Delivery should be undertaken in a hospital equipped with specialist cardiac, anaesthetic and obstetric facilities. If the patient cannot be expected to survive in good health, puerperal sterilization is advised.

Spontaneous coronary artery dissection is often underdiagnosed. It accounts for a certain amount of acute coronary syndromes. Intravascular imaging, either intravascular ultrasound or optical coherence tomography, represents a relevant ally for interventional cardiologist. Here we report an intriguing case of spontaneous coronary artery dissection not immediately diagnosed due to the explained parachute effect, managed invasively according to an algorithm previously published, with a good long-term outcome.

Tocolysis was carried out in a case of paroxysmal junctional tachycardia (Bouveret) and there was an attack of paroxysmal tachycardia which resolved spontaneously. In a case of Wolff-Parkinson-White syndrome and in two cases of ventricular double extra-systoles, tocolysis was very well tolerated. A short discussion based on these four case histories is presented. The indication for the use of tocolytic drugs was discussed with the cardiologist. The cardiac state of these pregnant women under treatment was very closely monitored.
