Autoimmune cholangitis syndrome with a bias towards primary biliary cirrhosis.

The apparent coexistence of primary biliary cirrhosis (PBC) and autoimmune hepatitis in the same patient raises unresolved problems for nosology and therapy. These are exemplified by a 45-year-old Japanese woman with overlapping clinical, serological and histological features of autoimmune cholangitis and autoimmune hepatitis. The classical serological test for PBC, antimitochondrial antibody (AMA) by immunofluorescence, was atypical. By immunoblotting there was reactivity with one of the enzymes of the 2-oxo-acid dehydrogenase complex (2-OADC) family, now recognized as autoantigens responsibl…

p53 Protein overexpression in cholangiocarcinoma arising in primary sclerosing cholangitis.

The protein encoded by the p53 tumour suppressor gene plays an important part in the regulation of cell growth. Abnormalities of this gene represent one of the most common genetic changes in the development of human cancers. This study investigated the expression of p53 protein in cholangiocarcinoma arising in association with primary sclerosing cholangitis (PSC). Of the 14 patients with cholangiocarcinoma studied, 13 had underlying PSC. The expression of p53 protein was detected immunohistochemically in paraffin wax embedded liver specimens, after microwave pretreatment. The expression of p53…

Immunogenetic aspects of primary sclerosing cholangitis: implications for therapeutic strategies.

Primary sclerosing cholangitis (PSC) is a chronic cholestatic liver disease characterized by fibro-obliterative inflammation of the intra- and extrahepatic bile ducts; in 70% of cases, it is associated with inflammatory bowel disease. The disease usually runs a progressive course, ultimately leading to biliary cirrhosis, hepatic failure, and sometimes cholangiocarcinoma, with a median survival of 12 yr after diagnosis. As yet, the etiology of primary sclerosing cholangitis remains unknown, although several recent studies have implicated immunogenetic factors as important pathogenic mechanisms.…

Biliary tract emergencies. Acute cholecystitis, acute cholangitis, and acute pancreatitis.

Acute cholecystitis, acute cholangitis, and acute pancreatitis represent the most common biliary tract emergencies. Most are due to gallstones in the gallbladder and bile ducts. Acute cholecystitis is treated by surgery in most cases. Laparoscopic cholecystectomy combined with endoscopic sphincterotomy may become more common in the future for treatment of acute cholecystitis as well as in cases of acute cholangitis and pancreatitis if the bile ducts are cleared of gallstones. Although the role of either surgery or endoscopic treatment may be more clearly defined in some biliary tract emergenci…

AIDS-related cholangitis: diagnostic features and course in 15 patients.

About 60 cases of cholangitis in patients with the acquired immune deficiency syndrome (AIDS) have been described. We report our experience concerning 15 patients infected with the human immunodeficiency virus (HIV) and who had cholangitis as defined by radiological findings. Cholangitis was the revealing event of AIDS or HIV infection in 4 patients (27%). Twelve (80%) of the patients were homosexual men. The main diagnostic features were abdominal pain (73%), cholestasis without jaundice (100%), intestinal cryptosporidiosis (80%) and abnormal findings on abdominal ultrasonography (87%). Endos…

Short-term biliary dilatation and stenting in primary sclerosing cholangitis.

Operative biliary dilatation and prolonged percutaneous transhepatic biliary stenting have been useful in the management of primary sclerosing cholangitis, both intrahepatic and extrahepatic biliary strictures regressing after 12-16 months of treatment. We report the results of less prolonged treatment in a further four patients, in whom removal of a percutaneous transhepatic biliary stent 3-11 months after treatment led to an improvement in radiological appearances of the extrahepatic bile ducts, with no discernible effect on intrahepatic disease. Extrahepatic stricture recurred rapidly in on…

Sclerosing cholangitis, chronic pancreatitis, and Sjogren's syndrome: a syndrome complex.

The disease complex of chronic pancreatitis, sclerosing cholangitis, and Sjogren's syndrome seems to be a well-defined entity with an autoimmune cause similar to that which occurs in primary biliary cirrhosis. Treatment depends on the component of primary sclerosing cholangitis and, more particularly, on the degree of extrahepatic involvement.

No liver 99Tc-tin-colloid uptake in primary sclerosing cholangitis.

A 99Tc-tin-colloid scintigraphy showed no uptake of the radiopharmaceutical in the liver of a patient with jaundice. The diagnosis was later established as primary sclerosing cholangitis.

Long-standing asymptomatic primary sclerosing cholangitis: report of three cases.

Three patients, two males and one female, with asymptomatic primary sclerosing cholangitis (PSC) are described. The diagnosis was made in each case by endoscopic retrograde cholangiography after investigation of persistent elevation of the serum alkaline phosphatase. All three have remained completely well without any medical or surgical treatment for 3, 7, and 15 years, respectively, despite extensive involvement of the biliary tree. Follow-up liver biopsies in two have shown no histological evidence of progression to secondary biliary cirrhosis. PSC may occur more frequently and may follow a…

Purulent cholangitis and hepatic abscess after choledochoduodenostomy diagnosed by ERCP.

The authors report on a case of purulent cholangitis and hepatic abscess developing 5 years after choledochoduodenostomy, diagnosed by means of ERCP. They believe viscous pus adhering to the wall of the bile ducts to be responsible, as a mechanical factor, for causing the inhibiting bile discharge. After the ERCP examination, the patient became free from complaints, and could be discharged.

Bezafibrate for primary biliary cholangitis: A number needed to treat analysis.

BACKGROUND & AIMS: The clinical benefit of bezafibrate (BZF) in primary biliary cholangitis (PBC) has not been extensively quantified using absolute measures. We assessed the number needed to treat (NNT) with BZF to prevent liver transplantation (LT) or death (including liver-related death) in patients with PBC. METHODS: The NNT was calculated using LT-free survival probabilities among ursodeoxycholic acid (UDCA)-treated patients with PBC from the Dutch PBC Cohort Study and the reported adjusted hazard ratios (aHRs) for the effect of BZF on LT or death (aHR 0.33) and liver-related death (aHR …

Autoantibodies in the diagnostics, prognostics and follow-up of primary biliary cholangitis.

Primary biliary cholangitis (PBC), is a chronic autoimmune liver disease, characterized by cholangiopathy, cholestasis and in the long-term fibrosis, biliary cirrhosis and ultimately end-stage liver disease unless liver transplantation is applied. The diagnostics of PBC is based on biochemical tests, autoantibody measurements and liver histopathology. Abdominal ultrasound, later magnetic resonance cholangiopancreatography (MRCP) or endoscopic ultrasound can be effective methods for imaging the intra- and extrahepatic bile ducts. Concerning autoantibodies, anti-mitochondrial (AMA) and PBC-speci…

Eosinophilic cholangitis as an indeterminate biliary mimic: A systematic review of outcomes.

Eosinophilic cholangitis (EC) is a rare inflammatory biliary disorder that can mimic primary sclerosing cholangitis and cholangiocarcinoma. We performed a PRISMA 2020-guided systematic review to summarize diagnostic features, management, and outcomes. Web of Science, PubMed, MEDLINE, Embase, Scopus, and CINAHL were searched; 55 studies (55 patients) were included. Among cases with a preoperative impression (n = 39), malignancy was suspected in 19 (48.7%). Among cases with reported localization (n = 44), isolated extrahepatic disease occurred in 15 (34.1%) and isolated intrahepatic disease in 7…

Rare Presentation of Eosinophilic Cholangitis in a 32-Year-Old Man.

Eosinophilic cholangitis (EC) is a rare condition characterized by eosinophilic infiltration of the bile ducts, often mimicking diseases like primary sclerosing cholangitis or cholangiocarcinoma. We report the case of a 32-year-old man with severe epigastric pain and elevated liver function tests. Initial imaging revealed common bile duct dilation and multiple strictures, initially suggestive of primary sclerosing cholangitis. Multiple endoscopic retrograde cholangiopancreatographies with brushings showed benign cytology but revealed polysomy on fluorescence in situ hybridization. A biopsy con…
