Topical dapsone in the treatment of acne: a systematic review.

BACKGROUND: Acne is one of the most common dermatological disorders. Initial therapies for acne include topical retinoids, benzoyl peroxide, and topical clindamycin. However, patients who fail initial therapies may benefit from alternative topicals, including dapsone gel. OBJECTIVE: To analyze the current literature studying the efficacy of topical dapsone in the treatment of acne. METHODS: PubMed, Embase, and Cochrane Library were systematically searched for clinical trials examining the efficacy of topical dapsone in the treatment of acne. RESULTS: Fourteen studies were included in the an…

Primary mammary actinomycosis challenged with penicillin allergy.

Actinomycosis is a subacute-to-chronic bacterial infection caused by gram-positive, filamentous, non-acid-fast, facultative anaerobic bacter … treated with doxycycline. However, doxycycline was discontinued due to tremors, and was replaced by clindamycin. The patient had a good clinical response with resolution of the abscess.

Anaerobic osteomyelitis in patients with Gaucher's disease.

Bone involvement in patients with Gaucher's disease is common, and some of its clinical manifestations may resemble acute osteomyelitis. In addition, many studies have emphasized the high risk of secondary infection when surgical procedures are performed at the site of the involved bone. Nevertheless, these infections have never been well documented in the literature. Presentation of a patient with Gaucher's disease and osteomyelitis due to Prevotella (Bacteroides) melaninogenica provided us the opportunity to review 10 other similar case reports documented in the literature since 1966. This r

New concepts in bacterial vaginosis.

Bacterial vaginosis is a polymicrobial, superficial infection caused by an increase in anaerobic bacteria and a concomitant decrease in lactobacilli. Diagnosis is based on the presence of three of the following findings: a homogeneous vaginal discharge; an amine odor released by the addition of 10 percent potassium hydroxide; the presence of clue cells on wet mount; the absence of lactobacilli on wet mount, and an increase in the pH of vaginal secretions to more than 4.7. Oral metronidazole is the drug of choice for the treatment of bacterial vaginosis, but clindamycin is an effective alternat…

The Pharmacokinetics and Target Attainment of Antimicrobial Drugs Throughout Pregnancy: Part III Non-penicillin and Non-cephalosporin Drugs.

INTRODUCTION: Understanding the pharmacokinetics (PK) of antimicrobial drugs in pregnant women is crucial to provide effective and safe trea … ng developed. On the other hand, the ability to reach adequate targets was assessed for vancomycin, clindamycin, rifampicin, rifapentine, ethambutol, pyrazinamide, and isoniazid. For the first six mentioned drugs, no dosage adaptations during pregnancy seem to be needed. Studies on isoniazid provide contradictory results. CONCLUSION: This systematic literatu …

Pulmonary mass-like lesion caused by Toxoplasma gondii in a domestic shorthair cat.

A 2-year-old male neutered domestic shorthair cat underwent investigations for acute onset of lethargy, hyporexia, and cough. Computed tomography of the thorax identified a large mass-like lesion in the left cranial lung lobe and bilateral pleural effusion. Thoracotomy and left cranial lung lobectomy were performed. Histopathology of the pulmonary mass was consistent with a localized Toxoplasma gondii pneumonia, confirmed by positive polymerase chain reaction on the affected lung lobe. After adjunctive medical management with a 28-day course of clindamycin (12.5 mg/kg PO q12h), clinical signs …

Toxoplasmic retinochoroiditis--a historical review and current concepts.

Toxoplasmic retinochoroiditis usually presents during the first three decades of life as a consequence of intra-uterine infection by Toxoplasma gondii. The ingestion of infected undercooked meat, or foodstuffs contaminated by infected cat faeces, constitute the primary sources of infection for the non-immune mother. It is thought that following congenital infection, Toxoplasma cysts remain dormant in otherwise normal retina and that acute retinochoroiditis is the result of reactivation of the parasite, perhaps by cyst rupture. Treatment is indicated for sight threatening disease and comprises

Gardnerella vaginalis-associated balanoposthitis.

The clinical features, microbiologic investigation, and response to therapy of three patients with Gardnerella vaginalis-associated balanopo … concurrent treatment of the partner. Two patients subsequently relapsed but ultimately responded to clindamycin therapy. These men presented with a distinctive clinical syndrome of balanoposthitis associated with G. vaginalis, which is in many respects similar to the syndrome of bacterial vaginosis in women. Our data indicate that balanoposthitis may have a p …

Infectious endocarditis caused by Lactobacillus acidophilus in a patient with mistreated dental caries.

We present a rare case of infectious endocarditis caused by Lactobacillus acidophilus in a patient on long-term steroid use for autoimmune hepatitis. In vitro susceptibility-guided antibiotics with benzylpenicillin plus clindamycin and successive mitral annuloplasty resulted in a favorable outcome. Infectious endocarditis was suspected to be a complication of mistreated periodontal infection. Maintenance of oral hygiene is important in immunocompromised patients.

Lung abscess due to Streptococcus mitis: case report and review.

Streptococcus mitis is a bacterium traditionally regarded as a normal commensal of the oropharynx, skin, and intestinal and genital tracts. To our knowledge, we describe the first case of bilateral lung abscesses caused by S. mitis in an immunocompetent host. The abscesses were successfully treated with clindamycin and gentamicin. Our case illustrates that S. mitis should be considered a cause of pulmonary abscesses.

[Drug-induced esophageal ulcers].

In cases without a history of gastrointestinal or cardiac disease, acute odynophagia prompts the tentative diagnosis of drug-induced esophageal ulcer. Possible causes are tetracycline, clindamycin, emepronium bromide, potassium chloride, etc. Other diseases such as carcinoma can be ruled out by endoscopy and biopsy. To avoid such esophageal lesions drugs should be taken with sufficient fluid and not immediately before bedrest.

Clinical practice guideline for the diagnosis and management of group A streptococcal pharyngitis: 2012 update by the Infectious Diseases Society of America.

The guideline is intended for use by healthcare providers who care for adult and pediatric patients with group A streptococcal pharyngitis. The guideline updates the 2002 Infectious Diseases Society of America guideline and discusses diagnosis and management, and recommendations are provided regarding antibiotic choices and dosing. Penicillin or amoxicillin remain the treatments of choice, and recommendations are made for the penicillin-allergic patient, which now include clindamycin.
