Neurological Complications of Endocarditis: A Multidisciplinary Review with Focus on Surgical Decision Making.

Infective endocarditis (IE) is a systemic disease with many potential neurologic manifestations including ischemic and hemorrhagic strokes, cerebral microbleeding, infectious intracranial aneurysms, meningitis, brain abscesses, and encephalopathy. The majority of left-sided (heart) IE patients have brain lesions that may alter management decisions, warranting the systematic use of magnetic resonance imaging. Many patients require surgical treatment of valvular disease, and central nervous system lesions weigh into decision making. Data regarding the timing of surgery are conflicting, but earli

Factors affecting treatment decisions for Class I malocclusions.

INTRODUCTION: Orthodontic treatment planning requires skill and expertise with considerable practice variations. The aims of this study were to review retrospectively the pretreatment records of patients with Class I malocclusion and to identify variables that could play a role in the treatment decision. METHODS: From the available records of 1500 orthdontic patients, the pretreatment records of 202 patients were selected at random. Inclusion and exclusion criteria were applied, and the surviving records were divided into extraction (n = 92) and nonextraction (n = 92) treatment groups. Skelet

A retrospective study of factors associated with treatment decision for nontuberculous mycobacterial lung disease in adults without altered systemic immunity.

BACKGROUND: Nontuberculous mycobacteria (NTM) lung diseases are increasingly recognized as chronic opportunistic infections, occurring in individuals with a wide variety of underlying conditions. In the absence of systemic immunodeficiency, decision of NTM lung disease treatment must relies on a careful risk/benefit assessment, given the requirement of long-term administration of multidrug therapies supported by limited evidence. The primary objective was to identify the factors associated with anti-NTM treatment initiation. Clinical and radiological outcome upon treatment were studied. METHO

Defining the key clinical indicators for ineffective breathing pattern in paediatric patients: a meta-analysis of accuracy studies.

AIMS AND OBJECTIVES: The purpose of this study was to identify the key clinical indicators of ineffective breathing pattern among paediatric patients. BACKGROUND: When nurses perform clinical reasoning, certain characteristics represent the clinical indicators necessary to confirm the presence of a particular diagnosis. Some quantitative studies have reported the prevalence of ineffective breathing pattern in different samples of patients. However, these findings should be synthesised. DESIGN: Meta-analysis of quantitative nursing studies. METHODS: Studies were identified via systematic sea

2021 American College of Rheumatology Guideline for the Treatment of Rheumatoid Arthritis.

OBJECTIVE: To develop updated guidelines for the pharmacologic management of rheumatoid arthritis. METHODS: We developed clinically relevant population, intervention, comparator, and outcomes (PICO) questions. After conducting a systematic literature review, the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to rate the certainty of evidence. A voting panel comprising clinicians and patients achieved consensus on the direction (for or against) and strength (strong or conditional) of recommendations. RESULTS: The guideline addresses treatment with

Sensi-steps: Using Patient-Generated Data to Prevent Post-stroke Falls.

We present Sensi-steps, an application using patient-generated data (PGD) to prevent falls for geriatric and especially poststroke patients. The Sensi-steps tool incorporates a wearable wrist device, pedometer, pressure and proximity sensors, and tablet. PGD collection occurs through Timed Up and Go (TUG) tests and collection of physiological data, which is integrated into the EHR. Fall risk factor active tracking encourages new ways of shared decision-making between patients, caregivers, and practitioners. PGD will be managed at the primary care nurse or Care Manager level (see 3-tier PGD ser

Diagnosis and management of adrenal incidentaloma: use of clinical judgment and evidence in dialog with the patient.

The prevalence of adrenal incidentaloma (AI) in imaging studies, including those of the adrenal glands, is estimated to be 1-5%. Essential factors for the proper management of AI include a correct diagnosis, adequate surgical skills, appropriate perioperative management, and sound dialogue with the patient. Aside from the possibility of overdiagnosis, patients with apparent signs or symptoms attributable to adrenal hormone excess have reasonable indications for surgery. At the same time, milder patients may be candidates for active surveillance without intervention. Even individuals with nonfu

A novel framework for evaluating the impact of individual decision-making on public health outcomes and its potential application to study antiviral treatment collection during an influenza pandemic.

The importance of accounting for social and behavioural processes when studying public health emergencies has been well-recognised. For infectious disease outbreaks in particular, several methods of incorporating individual behaviour have been put forward, but very few are based on established psychological frameworks. In this paper, we develop a decision framework based on the COM-B model of behaviour change to investigate the impact of individual decision-making on public health outcomes. We demonstrate the application of our decision framework in a proof-of-concept case study based on the 2

From inflammatory phenotypes to clinical decisions: a stratified diagnostic and therapeutic framework for CIP.

BACKGROUND: Checkpoint inhibitor pneumonitis (CIP) is an uncommon but potentially life-threatening immune-related adverse event that may interrupt effective anticancer therapy and require prolonged immunosuppression. Its clinical presentation, radiographic appearance, inflammatory biology, and response to corticosteroids vary substantially, while infection, radiation pneumonitis, tumor progression, pulmonary embolism, and pre-existing interstitial lung disease may produce overlapping findings. Conventional severity grading remains important for communication and triage but does not fully captu

When the Algorithm Speaks First: Trust, Reliance and the Reconstruction of Clinical Metacognition.

AIMS OF THE JOURNAL ADDRESSED: This paper addresses three of the aims of the Journal: clinical decision making, health philosophy and person-centred healthcare. Retinal imaging serves as the analytic case. The question at issue, what epistemic attitude a clinician may properly hold towards an algorithmic output issued for an individual patient, arises in every discipline and every health system in which such systems are deployed, and the argument is tested throughout against systems that read no images. RATIONALE, AIMS AND OBJECTIVES: Diagnostic artificial intelligence increasingly delivers a

Management of colorectal cancer in older adults: an expert panel recommendation to guide daily clinical practice.

Colorectal cancer predominantly affects older adults (ie, those aged 70 years or older). Along with shifting demographic trends, the number of patients with colorectal cancer in this age group is estimated to sharply increase globally over the next few decades. During the past decade, new treatment modalities and systemic therapeutic options have been introduced through well conducted clinical trials that shaped and fundamentally redefined the treatment landscape. However, older adults with reduced physiological reserves, functional impairment, comorbidities, and geriatric syndromes are under-

Ki-67 as a treatment decision modifier in breast cancer.

Ki-67 is widely used in breast cancer; however, the fraction of patients in whom it meaningfully alters treatment decisions is not well established. Using a guideline-based approach anchored in National Comprehensive Cancer Network (NCCN) recommendations (v2.2026), we identified clinical scenarios in which Ki-67 directly modifies adjuvant therapy decisions and estimated their prevalence using population-level data and complementary datasets, recognizing that fully annotated datasets capturing all relevant clinicopathological variables remain limited. A back-of-the-envelope calculation suggests

Regional Node-positive Bladder Cancer: Therapeutic Decisions Based on Trial Results in Perioperative and Advanced Disease Settings.

Optimal therapeutic management for cN1 M0 bladder cancer consists of perioperative systemic therapy and radical cystectomy. For patients with cN2-3 M0 disease, evidence supports upfront systemic therapy, preferably with enfortumab vedotin + pembrolizumab. Consolidative locoregional therapy may be an option in selected responders. Questions remain regarding the optimal duration of systemic therapy and the role of biomarkers. Multidisciplinary expert opinion can be critical for informed shared decision-making.

[AI-assisted radiology in the management of respiratory infections].

Respiratory infections are among the leading causes of consultation in primary care and emergency settings and account for substantial healthcare resource use. Their management relies heavily on chest imaging, particularly radiography and, more recently, lung ultrasound. Artificial intelligence (AI) applied to these modalities offers new opportunities to improve anomaly detection, reduce interobserver variability, and support clinical decision-making. This article summarizes recent evidence on the contribution of AI in thoracic imaging for the diagnosis of respiratory infections, particularly …

Can we ever have evidence-based decision making in orthopaedics? A qualitative evidence synthesis and conceptual framework.

BACKGROUND: The perception and use of scientific evidence in orthopaedic surgical decision-making is variable, and there is considerable variation in practice. A previous conceptual framework described eight different drivers of orthopaedic surgical decision-making: formal codified and managerial knowledge, medical socialisation, cultural, normative and political influence, training and formal education, experiential factors, and individual patient and surgeon factors. This Qualitative Evidence Synthesis (QES) aims to refine the conceptual framework to understand how these drivers of decision-
