Formulary review of 2 new biologic agents: tocilizumab for rheumatoid arthritis and ustekinumab for plaque psoriasis.

BACKGROUND: Two autoimmune biologics were recently approved by the FDA: ustekinumab in September 2009 for the treatment of moderate to severe plaque psoriasis in adults who are candidates for phototherapy or systemic therapy and tocilizumab in January 2010 for adult patients with moderate to severe rheumatoid arthritis (RA) who have not responded adequately to 1 or more tumor necrosis factor (TNF) antagonist therapies. Both agents use new mechanisms of action and add to the growing group of autoimmune biologics. OBJECTIVE: To critically review the phase 3 trials for ustekinumab and tocilizuma

Addressing and meeting the needs of osteoporotic patients with strontium ranelate: a review.

Patients with osteoporosis need a safe and effective treatment that reduces the risk of vertebral and non-vertebral fractures, leading to clinical benefits such as reduced back pain and height loss. Strontium ranelate corrects bone turnover, producing a more physiological state. Double-blind, placebo-controlled studies in postmenopausal osteoporosis show it to be effective in reducing vertebral and non-vertebral fracture risks. Treatment efficacy has been documented across a wide range of patient profiles, and appears to be independent of all the major determinants of fracture risk, including

Efficacy and safety of immune-based combinations in metastatic hepatocellular carcinoma: a systematic review and network meta-analysis.

BACKGROUND: Immune checkpoint inhibitors (ICIs) combined with other agents have emerged as the standard first-line treatment for metastatic hepatocellular carcinoma (HCC), replacing tyrosine kinase inhibitors (TKIs). However, the comparative efficacy and safety of different ICI-based combinations remains unclear. OBJECTIVE: To evaluate the efficacy and safety of ICI-based combinations versus TKIs across different regimens through a systematic review and network meta-analysis (NMA) of phase III randomized controlled trials (RCTs). METHODS: A comprehensive literature search was conducted across

Identification of tofersen PD-response biomarkers in VALOR clinical trial CSF via multiplexed quantitative proteomics.

Tofersen, the first approved genetically targeted therapy for amyotrophic lateral sclerosis (ALS), demonstrates significant lowering of plasma neurofilament in adults carrying mutations in the superoxide dismutase 1 (SOD1) gene; however, additional biomarkers of treatment response in ALS are lacking. Here, we analyze longitudinally collected cerebrospinal fluid (CSF) samples from the phase 3 VALOR clinical trial to identify candidate tofersen treatment-response biomarkers in SOD1-ALS via quantitative proteomics. We observe significant modulation from baseline abundance for 56 proteins in tofer

First-Line PD-1 Inhibitor-Based Therapy for Advanced or Metastatic Esophageal Squamous Cell Carcinoma: A Revised Systematic Review and Meta-Analysis of Phase III Trials.

BACKGROUND AND PURPOSE: Immune checkpoint blockade has changed the first-line treatment landscape for advanced esophageal squamous cell carcinoma (ESCC), but the magnitude and consistency of benefit across phase III trials require careful synthesis. METHODS: We conducted a PRISMA 2020-guided systematic review of PubMed, Embase, CENTRAL, and Web of Science (January 1, 2015 to April 30, 2025) for phase III randomized trials comparing first-line PD-1 inhibitor-based therapy with chemotherapy in advanced/metastatic ESCC. The primary endpoint was overall survival (OS). Secondary endpoints were saf

[Sublobar resection in early-stage NSCLC: towards a new therapeutic standard?].

Until now, the gold standard surgical treatment for early-stage non-small-cell lung cancer (NSCLC) has been pulmonary lobectomy with lymph node dissection. However, several cohort studies have suggested that infra-lobar resection may provide equivalent survival while better preserving quality of life and lung function. The results of two prospective randomized phase III studies comparing sublobar resection (segmentectomy or wedge resection) have recently been published. The JCOG 0802 trial focused on cT1a-b NSCLC and showed significantly better survival in the segmentectomy group, but a higher

Risk of peritoneal recurrence following laparoscopic versus open surgery for stage II/III colorectal cancer: An integrated analysis of three Phase III randomized controlled trials (JCOG2310AS2).

INTRODUCTION: The risk of peritoneal recurrence following LAP remains unclear because of inconsistent data and a limited number of recurrence events. The objective of this study is to evaluate whether laparoscopic surgery (LAP) is a potential risk factor for peritoneal recurrence following curative surgery for stage II/III colorectal cancer (CRC). MATERIALS AND METHODS: Data from three Phase III trials were extracted from an ancillary research database (JCOG2310A). Patients with pStage II/III CRC who underwent curative surgery were identified and included in this study. The frequency of perit

Targeted oxygenation in the respiratory care of premature infants at delivery-effects on outcome: a randomised controlled trial (Torpido 3060) study protocol.

INTRODUCTION: The safest oxygen levels needed for preterm infant respiratory support at birth are uncertain. We aimed to compare the outcomes of infants up to 286 weeks gestation who had respiratory care initiated at birth with fractional inspired oxygen (FiO2) 0.3 or 0.6, which was adjusted to meet specific oxygen saturations (SpO2). METHODS: This randomised controlled phase III trial was stratified by (1) site, (2) gestation and (3) multiplicity. Infants between 23+0 to 28+6 weeks gestation were randomised to initial respiratory support with FiO2 0.3 or 0.6, adjusted to meet common SpO2 tar

Reappraisal of Biologic Efficacy from Phase 3 Trials in Refractory Chronic Rhinosinusitis and Nasal Polyps.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is commonly associated with type 2 inflammation and may be punctuated by exacerbations requiring systemic corticosteroids and eventually a need for surgery. The use of biologics to target type 2 inflammation has greatly ameliorated the management of refractory CRSwNP. Currently available biologics act upstream by blocking the epithelial cytokine thymic stromal lymphopoietin (TSLP) or downstream blocking type 2 cytokines including Interleukin (IL4), IL5, and IL13, or Immunoglogulin E (IgE). Synthesizing the data from phase 3 randomized control t

Implementing a contextually appropriate foot-ankle exercise programme in primary care for the prevention of modifiable risk factors for ulcers in people with diabetes: protocol for a hybrid type 2 study.

INTRODUCTION: Diabetes is a highly prevalent disease that negatively impacts people's health and quality of life. It can result in diabetic peripheral neuropathy (DPN) and foot complications, which in turn lead to ulcers and amputations. The international guidelines on diabetic foot included specific foot-ankle exercises as preventive strategy capable of modifying the risk factors for ulcers. Our aim is to test the effectiveness and to implement a contextually appropriate preventive intervention-a foot-ankle exercises programme alongside educational strategies-in a primary care setting to impr

Towards precision medicine in COPD: Targeting type 2 cytokines and alarmins.

Chronic obstructive pulmonary disease (COPD) is a main global epidemic increasing as population age and affecting approximately 10% of subjects over 45 years. COPD is a heterogeneous inflammatory disease with several endo-phenotypes and clinical presentations. Although neutrophilic inflammation is canonically considered a hallmark of COPD, eosinophilic inflammation can also be present in a subgroup of patients. Several other immune cells and cytokines play a key role in orchestrating and perpetuating the inflammatory pathways in COPD, making them attractive targets for treating this disorder.

Study protocol for a phase III randomised controlled trial of Sailuotong (SLT) for vascular dementia and Alzheimer's disease with cerebrovascular disease.

Vascular dementia (VaD) accounts for 15-20% of all dementia cases. It is a syndrome of acquired cognitive impairment with a complex pathophysiological basis. A novel herbal formulation (Sailuotong; SLT) consisting of Panax ginseng C.A Mey, Ginkgo biloba L and Crocus sativus L extracts was developed to treat VaD. Preclinical animal studies found significant improvements in memory and in pathogenic biochemical parameters. Appropriate safety of SLT was shown in acute and chronic toxicity studies, and early clinical trials of SLT demonstrated enhancements in cognition in VaD patients. A fully powe

Cutaneous T cell lymphoma treated with mogamulizumab monotherapy and mogamulizumab plus etoposide combined therapy: A real-world case series.

Since the efficacy of mogamulizumab has been confirmed by a phase III, randomized study, mogamulizumab is one of the promising first-line therapies for advanced cutaneous T cell lymphoma (CTCL), though its efficacy is not completely satisfactory. Therefore, several anti-lymphoma drugs such as etoposide were recently used to enhance the anti-tumor effects of mogamulizumab for the treatment of mycosis fungoides (MF). In this report, the anti-tumor effects of mogamulizumab and post mogamulizumab therapy were retrospectively evaluated in 11 cases of CTCL in real-world clinical practice. The best r
