Extra-colonic manifestations of familial adenomatous polyposis coli.

The most important clinical manifestation of Familial Adenomatous Polyposis (FAP) is a profuse polyposis predisposing to colorectal cancer. This clinical picture emphasises the fundamental cellular role of APC in colorectal tumorigenesis. As prophylactic colorectal surgery has significantly reduced the mortality associated with this disorder, other less penetrant signs and symptoms of the condition are becoming more clinically relevant. Highlighting these problems will help improve the quality of life for patients with FAP and may have the additional benefit of shedding new light on other poss

Serrated neoplasias and de novo carcinomas in ulcerative colitis: a histological study in colectomy specimens.

BACKGROUND AND AIM: Cancer in ulcerative colitis (UC) originates in dysplastic crypts, adenomatous growths (UCAG), and UC-associated adenomas (UCAD). The aim of the present study was to compare the histological phenotypes between UCAG, UCAD, and sporadic colorectal adenomas in non-colitics (non-UCAD), as well as between UC-associated carcinomas (UCC) and carcinomas in non-colitic patients (non-UCC). METHODS: Three thousand and forty nine sections from 96 colectomy specimens in patients with UC-pancolitis and carcinoma were reviewed. RESULTS: Villous phenotypes were more frequent in UCAG (48%

Radiofrequence ablation of liver cancers.

Primary and secondary malignant liver cancer are some of most common malignant tumors in the world. Chemotherapy and radiotherapy are not very effective against them. Surgical resection has been considered the only potentially curtive option, but the majority of patients are not candidates for resection because of tumor size, location near major intrahepatic blood vessels and bile ducts, precluding a margin-negative resection, cirrhotic, hepatitis virus infection or multifocial. Radiofrequence ablation (RFA), which is a new evolving effective and minimally invasive technique, can produce coagu

Isolated splenic metastasis from colorectal carcinoma: report of a case and review of the literature.

In general, surgical resection for metastatic colorectal adenocarcinoma rarely benefits more than a small percentage of patients long term. In the case of isolated splenic metastasis without evidence of other metastatic disease, splenectomy may increase survival times in patients with previously resected colorectal adenocarcinoma. Currently there are only five case reports involving isolated splenic metastases in the English-language literature. This article presents a sixth case and a review of the previous five cases in the literature, with a discussion on the possible diagnostic and therape

Clinicopathological differences between familial colorectal cancer type X and sporadic cancer in an isolated area of spain.

AIM: Very few studies have compared the epidemiological characteristics of patients with familial colorectal cancer Type X (FCCTX) with those of sporadic colorectal cancer (S-CRC). The aim of this study was to compare clinicopathological characteristics and survival between FCCTX and S-CRC in patients from a historically isolated geographical region. METHOD: A retrospective study was carried out of patients with S-CRC and FCCTX treated in the Canary Islands. Family and personal history of colorectal cancer (CRC) were recorded, together with genetic (microsatellite instability), immunohistoche

High frequency of microsatellite instability and loss of mismatch-repair protein expression in patients with double primary tumors of the endometrium and colorectum.

BACKGROUND: Patients with the familial syndrome hereditary nonpolyposis colorectal carcinoma (HNPCC) exhibit an increased risk for several tumor types, of which the greatest lifetime risk is for colorectal and endometrial carcinoma. HNPCC is caused by a germline mutation in one of several identified mismatch repair (MMR) genes and typically presents with microsatellite instability (MSI) and frequent loss of MMR protein expression in the tumor tissue. The objective of this study was to estimate the proportion of double primary tumors of the endometrium and colorectum that displays tumor charact

A Comprehensive Meta-Analysis and Review of Intraoperative Radiotherapy in Colorectal Cancer: Examining Radiation Dosage, Long-Term Prognosis, and Treatment-Related Complications.

BACKGROUND: The effect of device type and radiation dose in intraoperative radiotherapy (IORT) for colorectal cancer is not well established. To explore this, we reviewed studies reporting postoperative complications, disease-free survival, local control, and long-term survival in patients treated with non-implantable IORT techniques-IOERT, KV-IORT, and HDR-IORT. Most of the available evidence relates to IOERT or HDR-IORT, which is usually delivered at 15 Gy (range 10-20 Gy). The median dose for KV-IORT is typically 12.5 Gy. METHODS: We conducted a systematic search of literature through Nove

Hospital and surgeon volume versus outcomes after colorectal cancer surgery: umbrella review and meta-analysis.

BACKGROUND: An inverse relationship between surgical volume and outcomes has been suggested, with higher-volume hospitals and surgeons achieving better results, prompting debate over the centralization of surgical services. However, minimum volume thresholds are unclear, and volume is a poor proxy for quality. Despite the significant global burden of colorectal cancer, the benefits of high-volume care remain uncertain. This umbrella review synthesized the evidence on volume-outcome associations in colorectal surgery. METHODS: An umbrella review (PRISMA 2020) was conducted to evaluate systemat

A systematic review of the impact of post-operative oral fluid intake on ileus following elective colorectal surgery.

INTRODUCTION: Ileus (delayed return of bowel function after surgery) is one of the highest priority research questions in modern day colorectal practice. Current Enhanced Recovery After Surgery (ERAS) guidance either does not include a specific recommendation for volume of postoperative oral fluids/foodstuffs or suggests ad-lib fluids. It is unclear if the volume of intake affects ileus rates. This systematic review aimed to determine the optimal fluid volume for patients to consume day one after elective colorectal surgery. METHODS: The literature was searched across seven databases, Septemb

Open and Closed Approaches to Skin Closure After Nonelective Open Colorectal Operations.

INTRODUCTION: Optimal wound management strategies to reduce surgical site infections (SSIs) in nonelective open colorectal surgery (NOCS) remain controversial and variable. Our aim is to describe SSI and other 30-day outcome measures among patients with varying wound management techniques undergoing NOCS. METHODS: All NOCS patients were extracted from the 2016 to 2018 ACS-NSQIP database. Outcomes of patients managed with all layers closed (ALC) were compared to patients managed with skin open (SO), using propensity score matching (PSM) to control for significant confounding risk factors for S

Incidence of wound dehiscence after colorectal cancer surgery: results from a national population-based register for colorectal cancer.

BACKGROUND: Patient-related risk factors for wound dehiscence after colorectal surgery remain obscure. METHODS: All open abdominal procedures for colorectal cancer registered in the Swedish Colorectal Cancer Registry (SCRCR, 5) 2007-2013 were identified. Potential risk factors for wound dehiscence were identified by cross-matching between the SCRCR and the National Patient Register (NPR). The endpoint in this study was reoperation for wound dehiscence registered in either the SCRCR or NPR and patients not reoperated were considered controls. RESULTS: A total of 30,050 patients were included

From Visual Haptics to Force-Controlled Robotic Surgery: Preliminary Clinical Experience Using the da Vinci 5 System in Colorectal Surgery-Case Series Study.

BACKGROUND: The absence of haptic feedback has been a fundamental limitation of robotic surgery. The da Vinci 5 (dV5) system is the first da Vinci platform to incorporate true force feedback (FFB), enabling real-time transmission of instrument-tip forces to the surgeon. However, the quantitative impact of FFB sensitivity settings remains unclear. This study represents the first Japanese clinical evaluation of dV5 FFB in colorectal surgery. METHODS: We conducted a case-series analysis of 10 consecutive robotic colorectal cancer resections performed using the daV5 system by a single Japan Socie

Risk factors for postoperative abdominal infection of colorectal cancer: A systematic review based on the PRISMA guidelines.

BACKGROUND: Intra-abdominal infections are a common complication of colorectal cancer surgery. Postoperative abdominal infections can cause systemic inflammatory response syndrome, which seriously affects the prognosis of patients. With the widespread application of antibiotics, the detection rate of drug-resistant bacteria has increased annually, resulting in increased pressure on antibiotic treatment selection. To improve the prognosis of postoperative patients with colorectal cancer, it is important to actively search for risk factors leading to postoperative abdominal infection and formula

Robotic-assisted versus laparoscopic surgery for colorectal resection in oncologic surgery: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Robotic-assisted surgery (RAS) is increasingly used for colorectal cancer (CRC), but its clinical and oncologic advantages over conventional laparoscopy (LS) remain uncertain. Prior meta-analyses (e.g., Huang et al., Front Oncol 13;1273378, 2023; Zou et al., BMC Surg 25;86, 2025) have included overlapping RCTs but vary in methodology, scope, and analytical transparency. This review aims to provide an updated, independently re-analyzed synthesis of RCTs published from 2015 to 2025, with full PRISMA compliance, explicit analytic reproducibility, and expanded evaluation of bias and ev

Deciphering Post-surgery Gut Microbial Dynamics in Colorectal Cancer through Multi-cohort Machine Learning.

Surgical resection, and its associated bowel preparation, remain the primary treatment for colorectal cancer (CRC), yet the associated effects on postoperative gut microbiota composition remain poorly characterized. This study profiled gut microbial compositional changes before and after colorectal surgery and identified microbial signatures associated with postoperative recovery. Four external study cohorts were integrated to strengthen biological interpretation of our internal cohort observations. Leveraging Machine learning (ML) combined with SHapley Additive exPlanations (SHAP), we identif
