Barriers to organ donation in the Jewish community.

It has long been recognized that members of the Jewish community generally do not sign organ donor cards or consent to the donation of the organs of their family members. In order to address this issue, the position of Jewish law on organ donation was examined and a sample of the Jewish population of Toronto was surveyed in an attempt to better understand the reasons for the observed reluctance to donate within this community. The results confirmed that the rate of signing organ donor cards was much lower in the Jewish community than in the general population, and although other reasons do exi

A review of the epidemiology of psoriasis vulgaris in the community.

Standard diagnostic criteria for epidemiological studies of psoriasis are currently lacking. In their absence, clinical examination and diagnosis of psoriasis by dermatologists provides the gold standard to underpin epidemiological research in psoriasis. The methods of data collection used most frequently include cross-sectional studies, case-control studies, cohort studies, and surveys of hospital and private practice attendance. Estimates of psoriasis prevalence made in cross-sectional studies employing clinical examinations as the survey instrument have ranged from 0.3 to 2.5%. Psoriasis in

Streptococcus pneumoniae bacteremia in a community hospital.

We reviewed 71 consecutive patients with Streptococcus pneumoniae bacteremia. The patients were analyzed by age, sex, ethnic background, and clinical presentation. Laboratory data reviewed included a CBC count, electrolyte levels, liver function studies, chest radiograph, HIV status, a sputum culture and Gram's stain, and sensitivities for the S pneumoniae isolated. Modalities of therapy, response to treatment, and ultimate outcome were examined. Many of the patients with pneumococcal bacteremia did not have cough, fever, or chills. HIV positivity was a risk factor for pneumococcal infection a

Cost of community care for older people.

BACKGROUND: There has been no published study that considers actual costs in a representative sample of people aged > or = 65 years. The present study describes the financial costs of formal community services for elderly people with dementia, depression, anxiety disorder or physical disability. METHOD: Psychiatric morbidity, physical disability and services received were assessed by standardised questionnaire in randomly selected Islington enumeration districts. Subjects were interviewed at home (n = 700). RESULTS: Dementia was the most expensive disorder per sufferer in terms of formal ser

Neuroleptic prescribing to the community elderly in Nottingham.

DESIGN: A cross-sectional pilot survey of computerized prescribing databases and written general practitioner records. Data were abstracted by the first author using a standard proforma. SETTING: Six out of 12 general practices situated in an area of north Nottingham known to have a high density of residential and nursing homes cooperated with the exercise (one was excluded because it lacked a computer system, one because the principal had a specialist commitment to old age psychiatry and four were self-excluded). PATIENTS: Patients recorded as receiving repeat prescriptions of oral preparat

Immunodiagnoses of community-acquired pneumonia in childhood.

A diagnosis of bacterial pneumonia requires isolation of the pathogen from blood, lung or tracheal aspirate; however, cultures of blood and pleural fluid samples are usually insensitive. Thus, in the majority of patients the etiology is rarely determined. A total of 840 pleural fluid effusion samples from children with clinical and laboratory diagnoses of acute bacterial pneumonia were evaluated by Dot-ELISA. This method was standardized in order to detect polysaccharide capsular bacterial antigen in pleural fluid samples previously treated with 0.1 M EDTA and dotted on nitrocellulose membrane

[Community-acquired pneumonia: Etiological diagnosis].

Microbiological and immunoserological approaches were used in etiological diagnosis of community-acquired pneumonia. It was concluded that Streptococcus pneumoniae, Staphylococcus aureus, Pseudomonas aeruginosa, Legionella pneumophila and Klebsiella pneumoniae predominated in the etiological structure of present severe community-acquired pneumonia. The most actual causative agents of nonsevere community-acquired pneumonia in persons under 60 were S. pneumoniae, Hemophilus influenzae, Mycoplasma pneumoniae and Chlamydia pneumoniae. Nonsevere community-acquired pneumonia in persons over 60 and/

Campylobacter enteritis in children in an urban community.

OBJECTIVE: To determine the prevalence of Campylobacter species as aetiologic agents of diarrhoea and its antimicrobial susceptibility pattern. DESIGN: This was a laboratory based cross sectional study on the cause of childhood diarrhoea. SETTING: Department of Medical Laboratory Technology, Medical School in Harare. SUBJECTS: Children less than five years old with diarrhoea presenting at primary level health centres in Harare. MAIN OUTCOME MEASURES: Patient's age, culture results, antimicrobial susceptibility pattern. RESULTS: Campylobacter species had the highest isolation rate (9.3%) o

EEG-based communication: prospects and problems.

Current rehabilitation engineering combines new prosthetic methods with recent developments in personal computers to provide alternative communication and control channels to individuals with motor impairments. Despite these advances, all commercially available systems still require some measure of voluntary motor control. Thus, these systems are not useful for individuals who are totally paralyzed. Electroencephalographic (EEG) activity may provide the basis for a system that would completely bypass normal motor output. EEG-based communication technology might provide assistive devices for in

Hemodynamic study of the anterior communicating artery.

BACKGROUND AND PURPOSE: The anterior communicating artery (ACoA) is a site of predilection for intracranial saccular aneurysms causing subarachnoid hemorrhage. ACoA aneurysms are frequently associated with an asymmetrical circle of Willis. In such cases, the ACoA is probably exposed to high hemodynamic stress caused by a considerable shunt flow across the ACoA to the distal segment of the contralateral anterior cerebral artery (ACA). In the present study, the flow pattern and flow-induced shear stress in the ACoA complex that may initiate aneurysmal lesions were studied under steady and pulsat

Community-acquired fungal pneumonia in children.

Pediatric fungal pulmonary infections are being seen with increasing frequency. The dimorphic fungi Histoplasma capsulatum. Blastomyces dermatitidis, Coccidioides immitis, and Cryptococcus neoformans frequently cause infections that are asymptomatic. However, patients may suffer pneumonia and disseminated disease. Diagnosis can be made definitively by isolation of the causative organism, but serology or skin testing is often necessary when this is not successful. Severe or life threatening infections are treated with amphotericin B. Recently, new oral azole antifungals are being used more freq

Prevalence of epilepsy in a rural community of Guatemala.

We conducted a cross-sectional epidemiological study to assess the prevalence and patterns of epilepsy in a small rural village of Guatemala (population 2,111); 1,882 subjects (97.3%) were surveyed. By adminIstering the World Health Organization (WHO) standard questionnaire and performing neurological examinations, we detected 16 cases of epilepsy. The crude prevalence rate for this community was 8.5 in 1,000 general population for this form. The most common type of seizure was generalized tonic-clonic seizures (GTCS, 50%), followed by complex partial seizures (CPS, 37.5%), simple partial seiz

A collaborative community approach to homeless care.

Homelessness is a social, economic, and public health problem of increasing magnitude in the United States. The past methods and approaches to delivering health care to those without homes have been inadequate because of the many complex problems faced by homeless persons today. To facilitate a discussion of a collaborative community approach to homeless care, it is helpful to include a definition of homelessness, describe the homeless population and the health status of homeless individuals, and explain what is meant by health care for the homeless.
