[Diagnosis, treatment, and genetic analysis of five cases of primary atypical hemolytic uremic syndrome].

A retrospective analysis was conducted on the clinical characteristics, renal pathology, genetic testing, and treatment of five patients -tw … of renal function recovery. From a pathophysiological perspective, the timely administration of the complement C5 inhibitor eculizumab can rapidly induce clinical remission, reduce the incidence of end-stage renal disease, and improve prognosis in patients with aHUS.

Reduction by cobra venom factor of myocardial necrosis after coronary artery occlusion.

Components of the complement system are known to play an important role in the cytolytic process and in chemotaxis of leukocytes. Cobra venom factor specifically cleaves C3 activity via activation of the alternative (properdin) complement pathway. It does not act directly on C3. If C3 is involved in tissue necrosis after ischemic injury, cobra venom factor might reduce tissue damage after acute coronary occlusion. Accordingly, in 14 control dogs occlusion of the left anterior descending artery was carried out for 24 h. Epicardial electrograms were recorded 15 min after occlusion, and 24 h late

Midterm Outcomes of 12 Renal Transplant Recipients Treated With Eculizumab to Prevent Atypical Hemolytic Syndrome Recurrence.

BACKGROUND: Atypical hemolytic uremic syndrome (aHUS) is an orphan disease with a high rate of recurrence after kidney transplantation. However, reports of successful prevention of posttransplant aHUS recurrence with eculizumab emerged a few years ago. To further delineate its optimal use, we describe the largest series of kidney transplant recipients treated with prophylactic eculizumab. METHODS: Twelve renal transplant recipients with aHUS-related end-stage renal disease received eculizumab: 10 from day 0 and 2 at the time of recurrence (days 6 and 25). Clinical and histological features, c

Microangiopathic Anemia.

BACKGROUND: Patients suffering from hemolytic anemia, thrombocytopenia, and organ damage may suffer from microangiopathic anemia, also called thrombotic microangiopathy (TMA). This condition is caused by many different pathogenic mechanisms and is always life-threatening due to vessel occlusion in vital organs. Rapid and careful workup is mandatory to identify the cause of TMA. To identify patients suffering from immune-mediated thrombotic thrombocytopenic purpura (iTTP), ADAMTS13 (a disintegrin and metalloproteinase with a thrombospondin type 1 motif, member 13) measurement is mandatory. Disseminated gonococcal infection developing two days after initial eculizumab administration in a patient with neuromyelitis optica spectrum disorder: A case report and literature review.

Disseminated gonococcal infection (DGI) is a rare complication of Neisseria gonorrhoeae infection. Eculizumab, a complement C5 inhibitor used in conditions such as paroxysmal nocturnal hemoglobinuria, increases the risk of invasive Neisseria spp. infections. Although previous reports have described DGI in eculizumab-treated patients, evidence involving individuals with neuromyelitis optica spectrum disorder (NMOSD) is limited, and all cases occurred weeks to months after treatment initiation. In the present case, the patient received a quadrivalent meningococcal vaccine before receiving eculiz…

Evaluation of Eculizumab Use in Renal Transplant Recipients.

Introduction: Eculizumab is a monoclonal antibody that binds to complement protein C5, inhibiting complement-mediated thrombotic microangiopathy. It is approved for several indications including atypical hemolytic uremic syndrome. Additionally, eculizumab is used off-label for antibody-mediated rejection and C3 glomerulopathy in renal transplant recipients. Due to limited data available, the purpose of this study was to describe the use of eculizumab treatment in renal transplant recipients. Design: This retrospective single-center study evaluated the safety and efficacy of eculizumab for on-

Phase 2 study of danicopan in patients with paroxysmal nocturnal hemoglobinuria with an inadequate response to eculizumab.

Paroxysmal nocturnal hemoglobinuria (PNH) is characterized by uncontrolled terminal complement activation and subsequent intravascular hemolysis (IVH). C5 inhibitors prevent membrane attack complex formation, but patients may experience extravascular hemolysis (EVH) and continue to require blood transfusions. Danicopan, an oral proximal complement inhibitor of alternative pathway factor D (FD), is designed to control IVH and EVH. In a phase 2 dose-finding trial, eculizumab-treated transfusion-dependent patients with PNH (n = 12) received danicopan, 100 to 200 mg thrice daily, in addition to th

Low efficacy of vaccination against serogroup B meningococci in patients with atypical hemolytic uremic syndrome.

BACKGROUND: The C5 complement inhibitor eculizumab is first-line treatment in atypical hemolytic uremic syndrome (aHUS) going along with a highly increased risk of meningococcal infections. Serogroup B meningococci (MenB) are the most frequently encountered cause for meningococcal infections in Europe. Efficacy of the protein-based MenB-vaccine Bexsero in aHUS has not been determined and testing is only possible in patients off-treatment with eculizumab as a human complement source is required. METHODS: Patients with aHUS were vaccinated with two doses of the protein-based MenB-vaccine Bexser

Prevention and treatment of atypical haemolytic uremic syndrome after kidney transplantation.

Atypical haemolytic uraemic syndrome is a rare disorder characterized by an over-activated, dysregulated alternative complement pathway due to genetic mutation and environmental triggers. Atypical haemolytic uraemic syndrome is a serious, life-threatening disease characterized by thrombotic microangiopathy, which causes haemolytic anaemia, thrombocytopaenia, and acute renal failure. Since recurrences of atypical haemolytic uraemic syndrome frequently lead to end-stage kidney disease even in renal allografts, kidney transplantation for patients with end-stage kidney disease secondary to atypica

Adjunctive treatments for pneumococcal meningitis: a systematic review of experimental animal models.

New treatments are needed to improve the prognosis of pneumococcal meningitis. We performed a systematic review on adjunctive treatments in animal models of pneumococcal meningitis in order to identify treatments with the most potential to progress to clinical trials. Studies testing therapy adjunctive to antibiotics in animal models of pneumococcal meningitis were included. A literature search was performed using Medline, Embase and Scopus for studies published from 1990 up to 17 February 2023. Two investigators screened studies for inclusion and independently extracted data. Treatment effect

Early cellular responses to mitogens and adjuvants in the mouse spleen.

The purpose of this study was to evaluate the cellular events in the spleens of mice following the intravenous injection of mitogens and adjuvants. The compounds used were concanavalin A, polyadenylic polyuridylic acid, beryllium sulfate, bacterial endotoxin, tuberculin-purified protein derivative, and dextran sulfate. Nine different strains of mice (some deficient in the C5 component of complement) received a single dose of these compounds, and their spleens were studied at sequential time intervals, ranging from 1 hour to 14 days. Concanavalin A triggered marked blast activity in the T cell

Distinct Pharmacodynamic Complement Biomarker Signatures Define Iptacopan and Pegcetacoplan Treatment in C3 Glomerulopathy.

INTRODUCTION: C3 glomerulopathy (C3G) is driven by alternative pathway (AP) dysregulation. Iptacopan inhibits factor B, whereas pegcetacoplan inhibits C3/C3b, suggesting distinct pharmacodynamic (PD) biomarker profiles. METHODS: We conducted an exploratory, nonrandomized observational cohort study of patients with biopsy-confirmed C3G treated with iptacopan (n = 9) or pegcetacoplan (n = 9), using paired pretreatment and approximately 3-month posttreatment samples. Complement activity, protein levels, activation products, autoantibodies, and genetic findings were assessed. Profiles were classi

Successful treatment with eculizumab in refractory myasthenia gravis with both anti-AChR and anti-LRP4 antibodies: two case reports.

INTRODUCTION: Generalized myasthenia gravis (gMG), an antibody-induced autoimmune disorder of the neuromuscular junction, is characterized by fluctuating skeletal muscle weakness and fatigability. Approximately 85% of patients with MG have antibodies against the acetylcholine receptor (AChR), 5-10% against muscle-specific kinase receptor, and 1-5% against lipoprotein-related protein 4 (LRP4). In addition to the detection of antibodies providing insight into the diagnosis and prognosis of gMG, it also affects the choice of treatment. In the majority of cases, gMG can be managed effectively usin

Proteomic Biomarkers for the Prediction of Transition to Psychosis in Individuals at Clinical High Risk: A Multi-cohort Model Development Study.

Psychosis risk prediction is one of the leading challenges in psychiatry. Previous investigations have suggested that plasma proteomic data may be useful in accurately predicting transition to psychosis in individuals at clinical high risk (CHR). We hypothesized that an a priori-specified proteomic prediction model would have strong predictive accuracy for psychosis risk and aimed to replicate longitudinal associations between plasma proteins and transition to psychosis. This study used plasma samples from participants in 3 CHR cohorts: the North American Prodrome Longitudinal Studies 2 and 3,
