OBJECTIVES: To describe surgical options that may be indicated when conservative management of overactive bladder is unsuccessful.
METHODS: The literature on current endoscopic and open surgical procedures is reviewed.
RESULTS: The endoscopic approach of hydrodistention under anesthesia may be therapeutic, but normally offers only temporary relief of symptoms. Among open surgical procedures, cystolysis has not withstood the test of time and is no longer used. PURPOSE: The endoscopic treatment of urinary incontinence is limited by the injectable substances currently available. The ideal injectable material should be able to conserve its volume, be nonmigratory and nonantigenic. Toward this goal we developed a system for the treatment of incontinence in which a catheter with an inflatable and detachable self-sealing silicone membrane fits through a standard cystoscopic needle. The Foley catheter is used for drainage of the urinary bladder in < or = 15% of hospitalized patients and is self-retained by an inflatable balloon mechanism. Faulty balloon deflation, which leads to a retained catheter, is a condition treated by the urologist. In this article we describe a cystoscopic approach to deflation, evaluation, and recovery of retained fragments; review the techniques for deflation; and provide an algorithm to approach the retained Foley catheter.

A case of the patent urachus associated with abdominal abscess was reported. A 44-year-old woman was consulted to our clinic with the complaints of bladder irritability and discharge from the navel on October 27, 1987. A fist-sized tumor around the umbilicus was pointed out and the cystography from the umbilicus demonstrated that the contrast medium reached to the urinary bladder. A 71-year-old female was seen initially with the complaint of gross hematuria. Cystoscopic examination revealed non-papillary tumor at the dome of the bladder and a transurethral biopsy showed signet ring cell carcinoma. En bloc segmental resection was performed, and the patient has been well without any evidence of progression. We report a case, which we believe to be the 6th in Japan, of signet ring cell carcinoma of the urachus.

The results of the urodynamic investigations in 217 patients with female urinary incontinence are compared to the results of the standardized questionnaire of Gaudenz regarding stress incontinence and urgency incontinence. Müllerian fusion defects, submucous myomas, and intractable uterine bleeding have been managed traditionally by major surgical intervention. However, the cystoscope-resectoscope provides the operative versatility allowing transvaginal surgical management of these situations. Forty women underwent hysteroscopy and treatment with the cystoscope-resectoscope at the Yale-New Haven Hospital. Ureteroceles appear as transonic intravesical lesions on ultrasonic demonstration of the bladder, producing a 'cyst within a cyst' appearance. Four patients, three with simple ureteroceles and one with an ectopic ureterocele were investigated by intravenous urography (IVU), ultrasound (US) and cystoscopy.
