Joint-preserving surgical treatment of spontaneous osteonecrosis of the knee.

BACKGROUND: To date, reports of surgical treatment of spontaneous osteonecrosis of the knee (SPONK) refractory to non-operative treatment have primarily focused on knee arthroplasty. This report presents an overview of the characteristics of SPONK and reports our experience with joint-preserving surgical treatment of this condition. METHODS: Fifteen patients who had joint-preserving surgery after failed non-operative modalities were studied. These patients were treated at a single center between January 1998 and September 2006 with a combination of arthroscopy and core decompression, or osteo

A qualitative study of the experiences and expectations of surgery in patients with carpal tunnel syndrome.

The aim of this study was to explore the impact of carpal tunnel syndrome (CTS) on individuals and their expectations of surgical decompression to identify what outcome domains need to be assessed in future clinical trials. This qualitative study used in-depth, face-to-face interviews with nine patients with CTS awaiting surgical decompression. The tape-recorded interviews were transcribed fully, data were coded and categorized independently by two researchers and emerging themes were identified. Patients identified relief of symptoms-tingling, numbness and sleep disturbance, and resumption of

The neurosurgical management of spinal metastases causing cord and cauda equina compression.

The authors report a series of 100 consecutive patients with spinal metastases causing cord or cauda equine compression, who were treated with surgical decompression. Of these, 30% (all women) had breast cancer. The most common primary neoplasm in man was prostatic carcinoma. Pain was the earliest and most prominant symptom, followed by weakness. Bladder dysfunction was recorded in 40 patients. The thoracic region was the most common site of cord compression (76 patients). Surgical treatment involved urgent and extensive laminectomy decompression. Concomitant spinal stabilization was required

Surgical management of spinal intradural hematomas: illustrative cases.

BACKGROUND: Spinal intradural hematoma (SIH) is a rare condition with potential for permanent neurological deficit. SIH can be managed conservatively with serial imaging or surgically with lumbar drainage or open evacuation. We present 3 SIH cases managed with multiple surgical techniques, including a novel lumbar drainage-and-advancement technique, and review the literature. OBSERVATIONS: Patient 1 was a 35-year-old peripartum female with cauda equina syndrome after epidural analgesia. MRI revealed SIH at L4-5, necessitating urgent L3-S1 laminectomy and hematoma evacuation. Patient 2 was a 4

[Reoperation after orbital decompression for Graves' ophthalmopathy].

INTRODUCTION: The aim of our study was to report the incidence of complications requiring early or late reoperation after bone and fat orbital decompression for Graves's ophthalmopathy. PATIENTS AND METHOD: We conducted a retrospective bicentric study in the Maxillofacial Surgery Departments of Marseille and Lyon. Ninety patients were selected from 2006 to 2010, accounting for 168 orbital decompressions. The surgical indication was morphological and functional for all patients. Several surgical techniques were used: bone and fat orbital decompression by trans-palpebral resection, two or three

Outpatient lumbar spine decompression in 233 patients 65 years of age or older.

STUDY DESIGN: A retrospective review including patient follow-up on all lumbar decompression procedures by 1 of 2 surgeons. OBJECTIVE: To evaluate the success of lumbar decompressions performed on an outpatient basis on patients 65 years of age or older. SUMMARY OF BACKGROUND DATA: The efficacy of decompression procedures on the lumbar spine is documented and is often performed on an outpatient basis. Few studies have evaluated these procedures in older adults, and none in a greater population of patients on an outpatient basis. METHODS: A total of 1377 lumbar decompression procedures were

Invasive surgical management of cervical internal jugular venous compression: A literature review.

BACKGROUND: Internal jugular vein (IJV) compression is associated with several central nervous system symptoms, including but not limited to headache, blurred vision, tinnitus, and brain fog. Cranial venous compression is generally treated with stenting; however, the management of cervical IJV compression is not standardised, and there is very limited published data on outcomes. The objective of this review is to assess the current surgical interventions used in patients with cervical IJV compression and their outcomes. METHODS: We performed a literature search on electronic databases for ori

Integrative Migraine Therapy: From Current Concepts to Future Directions-A Plastic Surgeon's Perspective.

Migraine is a prevalent and disabling neurological disorder with multifactorial origins and complex clinical manifestations. While pharmacologic therapies remain the cornerstone of management, a growing body of evidence highlights the role of extracranial peripheral nerve compression as a significant contributor to migraine pathophysiology in selected patients. This recognition has expanded the therapeutic role of plastic surgery, offering anatomically targeted interventions that complement or surpass traditional medical approaches for refractory cases. From a plastic surgeon's perspective, op

Surgical illustrative review of the treatment of ischaemic priapism.

Ischemic priapism is a condition where there is prolonged and painful erection of the penis that is not associated with sexual stimulation. When initial bedside management fails to achieve detumescence, surgical management is indicated. This article is an illustrative review of historic and modern surgical techniques for the management of ischemic priapism including distal shunting techniques (Winter's, T-Shunt, Al-Ghorab, and Burnett's), proximal shunting techniques (Quackles, Barry), and penoscrotal decompression.

Neurogenic Thoracic Outlet Syndrome: A Primer for Hand and Peripheral Nerve Surgeons.

Neurogenic thoracic outlet syndrome is a complex condition and is commonly misunderstood. Historically, much of this confusion has been because of its grouping with other diagnoses that have little in common other than anatomic location. Modern understanding emphasizes the role of small unmyelinated C type pain and sympathetic fibers. Diagnosis is primarily clinical, after ruling out other common conditions. Hand therapy is usually the first-line treatment with variable success. Local anesthetic, botulinum toxins, or steroid injections can aid in diagnosis and offer short-term relief. Although

Challenges and Advances in the Diagnosis and Management of Neurogenic Thoracic Outlet Syndrome: A Comprehensive Review.

Neurogenic thoracic outlet syndrome (nTOS) is caused by brachial plexus compression in the thoracic outlet. It accounts for 85%-95% of thoracic outlet syndrome (TOS) cases, which may also be caused by compression of the subclavian artery and vein. Compression occurs in the interscalene triangle, costoclavicular space or subpectoralis minor space, with congenital anomalies and repetitive overhead activities as contributing factors. Diagnosis is challenging due to overlapping symptoms with other conditions. Patients commonly report pain, numbness, tingling and weakness in the neck, shoulder and

Effect of Intracapsular Pressure on Decompression Effectiveness.

AIM: The objective of this research was to analyse the correlation between intracapsular pressure and shrinkage rate of cystic lesion volume at different time points after decompression and to evaluate the relationship between the concentration of interleukin-1α (IL-1α) in cystic fluid and intracapsular pressure. METHODS: Fifty patients with jaw cystic lesions who underwent decompression were included. We measured the intracapsular pressure and IL-1α concentration in the cyst fluid. Moreover, we calculated the rate of shrinkage (RS) of cystic cavity volume at different time points. In additio

Tension pneumothorax decompression with colorimetric capnography: pilot case series.

Tension pneumothorax is a life-threatening condition that can develop when either the visceral pleura is disrupted, or with injury to the tracheobronchial tree. Rapid, accurate diagnosis and appropriate management are required to prevent significant atelectasis, hypoxia, circulatory arrest, and ultimate patient demise. Needle decompression is the current standard of care for the management of tension pneumothorax. Healthcare providers struggle to assess the success of decompression due to a lack of any immediate objective feedback. The gaseous composition of tension pneumothorax is similar to

Posterior cervical spine surgery for radiculopathy.

It is now common knowledge that cervical radiculopathy, frequently caused by disc herniation and/or degeneration, will often improve without surgical intervention. Only a small percentage of patients with the severity of symptoms necessitate surgical treatment. Surgery for radiculopathy is indicated for motor weakness, progressive neurological deficits, and progressive symptoms that do not improve with nonoperative treatment. Advantages and disadvantages exist for both ventral and dorsal approaches in the surgical treatment of cervical radiculopathy. Indications and results for dorsal nerve ro
