[Mechanical digestive system suturing devices].

Between 1994 (December)-1996 (May) 150 patients have been operated on using one or many stapling devices. The staplers disposable to us were the "Linear Cutter" or GIA (Gastrointestinal Anastomosis), "Linear Stapler" (TA) and "Intraluminal Circular Stapler" or EEA (end-to-end anastomosis) types, produced by ETHICON (Johnson and Johnson Ltd. Company). The principles operations performed were various digestive resections, intervisceralis anastomosis and interventions of reconstructions (in oesophagus surgery, ileal pouch etc.). The advantages of staplers applications are: a) the reduction of the

Comparative clinical outcomes of ventral rectopexy versus STARR in the management of obstructed defecation syndrome: a clinical review.

Obstructed defecation syndrome (ODS) is a multifactorial pelvic floor disorder commonly affecting women, characterized by straining, incomplete evacuation, and dependence on manual maneuvers. Among various surgical options, stapled transanal rectal resection (STARR) and laparoscopic or robotic ventral mesh rectopexy (LVMR/RVMR) are frequently employed. However, the comparative effectiveness of these procedures remains debated. This narrative review synthesizes evidence from 43 studies including randomized trials, cohort studies, registries, and systematic reviews published between 2004 and 202

Mechanical bowel preparation for elective colorectal surgery.

BACKGROUND: The presence of bowel contents during colorectal surgery has been related to anastomotic leakage, but the belief that mechanical bowel preparation (MBP) is an efficient agent against leakage and infectious complications is based on observational data and expert opinions only.An enema before the rectal surgery to clean the rectum and facilitate the manipulation for the mechanical anastomosis is used for many surgeons. This is analysed separately OBJECTIVES: To determine the security and effectiveness of MBP on morbidity and mortality in colorectal surgery. SEARCH STRATEGY: Publica

U.S. trends in elective and emergent major abdominal surgical procedures from 2002 to 2014 in older adults.

BACKGROUND: The U.S. population is aging and projected to undergo an increasing number of general surgical procedures. However, recent trends in the frequency of major abdominal procedures in older adults are currently unknown as improvements in non-operative interventions may obviate the need for major surgery. Thus, we evaluated the trends of major abdominal surgical procedures in older adults in the United States. METHODS: We performed a retrospective cohort study using the National Inpatient Sample from 2002 to 2014 with trend analysis using National Cancer Institute's Joinpoint Trend Ana

Studying Enhanced Recovery After Surgery (ERAS®) Core Items in Colorectal Surgery: A Causal Model with Latent Variables.

BACKGROUND: Previous Enhanced Recovery After Surgery (ERAS®) studies have not always taken into account that ERAS interventions depend on baseline covariates and that several confounding variables affect the composite outcomes. METHOD: A causal latent variable model is proposed to analyze data obtained prospectively concerning 1261 patients undergoing elective colorectal surgery within the ERAS protocol. Primary outcomes (composite of any complication, surgical site infection, medical complications, early ready for discharge (TRD), early actual discharge) and secondary outcomes (composite of

Robot-assisted laparoscopic (RAL) procedures in general surgery.

BACKGROUND: Robotics was introduced in clinical practice more than two decades ago, and it has gained remarkable popularity for a wide variety of laparoscopic procedures. We report our results of robot-assisted laparoscopic surgery (RALS) in the most commonly applied general surgical procedures. METHOD: Ninety seven patients underwent RALS from 2009 to 2012. Indications for RALS were cholelithiasis, gastric carcinoma, splenic tumors, colorectal carcinoma, benign colorectal diseases, non-toxic nodular goiter and incisional hernia. Records of patients were analyzed for demographic features, int

[Esophageal closed transection for surgical treatment of portal hypertension. A 10-year experience].

BACKGROUND: Extensive esophagogastric devascularization (Sugiura-Futagawa operation and complete portoazygos disconnection) are excellent alternate choices for low-risk patients (Child-Pugh A-B) in whom a selective shunt is not feasible, obtaining a low postoperative rebleeding rate and a good quality of life. Esophageal transection is an important step in these procedures because submucosal varices are obliterated, achieving complete portoazygos disconnection. Results during one decade with closed variant of esophageal transection are reported. METHOD: Between 1990 and 2000, 78 patients were

Inguinal neurectomy for nerve entrapment after open/laparoscopic hernia repair using retroperitoneal endoscopic approach.

BACKGROUND: Inguinal neuralgia after open and laparoscopic hernia, repair occurs in about 0.5% of treated patients. If the pain and the functional inability persist, it is possible that the genitofemoral nerve and ileoinguinal nerve are involved in entrapment, and surgical treatment is a possible option. This paper reports a personal endoscopic retroperitoneal approach for ileoinguinal and genitofemoral branches neurectomy. METHODS: A 12-mm trocar is inserted into the lower retroperitoneum and insufflated to create a work space. Neurectomy is performed under endoscopic guidance. RESULTS: Six

Perioperative outcomes in primary neonatal pullthrough versus pullthrough in older children with Hirschsprung disease: a systematic review and meta-analysis.

Primary pullthrough for Hirschsprung disease is the current standard in most uncomplicated standard segment disease. In this review and meta-analysis, we aimed to compare perioperative outcomes and functional outcomes for pullthrough done in the neonatal period compared to that in older age. PubMed, Medline, Embase, CINAHL, SCOPUS, and Web of Science were searched for relevant publications in English from inception to June 29, 2024. Twenty studies were included in the qualitative analysis with a total 3197 patients matching the review criteria. All included studies were observational in nature

Robotic-assisted Swenson procedure for Hirschsprung's disease with a median age of 35 days: a single-center retrospective study.

PURPOSE: The treatment of Hirschsprung's disease (HD) in infants with the robotic-assisted Swenson procedure has been rarely reported. In this investigation, we aimed to explore the safety and the efficacy of robotic-assisted Swenson procedure for the HD in infants. METHODS: From November 2022 to July 2023, 17 cases of HD were treated with the Da Vinci robotic Xi surgical system using a three-port approach. Preoperative, intraoperative, and postoperative data were collected and compared with 43 cases of HD treated with laparoscopy by the same lead surgeon. RESULTS: The robotic-assisted surge

Low HS-Omega-3 index as a predictor of severe postoperative complications in abdominal surgery: a prospective observational study.

BACKGROUND: Omega-3 supplementation may reduce postoperative complications, but variability in formulations and lack of standardized monitoring limit its use. The HS-Omega-3 Index® offers a standardized measure of omega-3 status, but its role in surgical outcomes remains unexamined. This pilot prospective observational study investigated the association between preoperative HS-Omega-3 Index® and postoperative complications in patients undergoing major gastrointestinal or hepatobiliary surgeries, adopting a broad approach to assess its relevance. METHOD: This study included 269 patients underg

Should minimally invasive approaches in rectal surgery be regarded as a key element of modern enhanced recovery perioperative care?

INTRODUCTION: The aim of study was to assess the impact of an enhanced recovery after surgery (ERAS) protocol and minimally invasive approaches on short-term outcomes in rectal surgery. PATIENTS AND METHODS: A consecutive series of patients that underwent open or minimally invasive rectal resections in a single institution between January 2015 and April 2020 were included in the study. An ERAS program was introduced in April 2016. The study cohort was divided into three groups: open surgery without ERAS, open surgery with ERAS, and minimally invasive surgery with ERAS. Outcome measures compar
