Safety of early diverting loop ileostomy reversal after sigmoid colectomy with primary anastomosis for perforated diverticulitis.

BACKGROUND: Evidence increasingly supports sigmoidectomy with primary anastomosis (SPA) and diverting loop ileostomy over Hartmann's procedure for perforated diverticulitis in stable patients. Prompt diverting loop ileostomy reversal (DLIR) is often preferred by patients; however, optimal timing after index surgery remains unclear. The objective of this study is to examine the association of DLIR timing with clinical outcomes and costs. METHODS: Retrospective analysis was performed using National Readmissions Database (2010-2020) of all adults who underwent emergent sigmoidectomy with primary…

Laparoscopic lavage for Hinchey III perforated diverticulitis: factors for treatment failure in two randomized clinical trials.

BACKGROUND: The Scandinavian Diverticulitis (SCANDIV) trial and the LOLA arm of the LADIES trial randomized patients with Hinchey III perforated diverticulitis to laparoscopic peritoneal lavage or sigmoid resection. The aim of this analysis was to identify risk factors for treatment failure in patients with Hinchey III perforated diverticulitis. METHODS: This was a post hoc analysis of the SCANDIV trial and LOLA arm. Treatment failure was defined as morbidity requiring general anaesthesia (Clavien-Dindo grade IIIb or higher) within 90 days. Age, sex, BMI, ASA fitness grade, smoking status, pr…

Self-Expandable Metal Stents for Left Sided Colon Obstruction from Diverticulitis. A Single Center Retrospective Series.

Background and Objectives: The incidence of diverticulitis is increasing in western countries. Complicated diverticulitis is defined as diverticulitis associated with localized or generalized perforation, localized or distant abscess, fistula, stricture or obstruction. Colonic symptomatic strictures are often treated with segmental colectomy. The aim of our study is to report our experience with Self Expandable Metal Stents (SEMS) placement to relieve sigmoid obstruction secondary to diverticulitis, either as a permanent solution or as a bridge to elective colectomy. Material and Methods: From…

Elective minimally invasive surgery for sigmoid diverticulitis: operative outcomes of patients with complicated versus uncomplicated disease.

AIM: The aim of this work was to compare the results of elective minimally invasive surgery between patients with complicated sigmoid diverticulitis and those with uncomplicated disease. METHOD: An institutional review board-approved database was searched for all consecutive patients who underwent elective minimally invasive surgery, including laparoscopic, hand-assisted and robotic sigmoidectomy, for diverticulitis between 2010 and 2017; they were classified according to the modified Hinchey classification as having complicated (abscess, fistula, stricture, obstruction, bleeding or previous …

The stent that time forgot: perforated small bowel diverticulitis seventy years in the making, a case report.

Biliary stents are commonly used for strictures and obstruction, but can migrate distally in up to 20% of cases. While most are identified e … small bowel diverticulosis with localized perforation and adhesions; segmental resection confirmed diverticulitis with perforation and foreign body reaction around a retained biliary stent. This case highlights an exceptionally delayed complication of stent migration and the importance of comprehensive history-taking when evaluating elderly patients with atypic …

Visceral Fat Quantified by a Fully Automated Deep-Learning Algorithm and Risk of Incident and Recurrent Diverticulitis.

BACKGROUND: Obesity is a risk factor for diverticulitis. However, it remains unclear whether visceral fat area, a more precise measurement of abdominal fat, is associated with the risk of diverticulitis. OBJECTIVE: To estimate the risk of incident and recurrent diverticulitis according to visceral fat area. DESIGN: A retrospective cohort study. SETTINGS: The Mass General Brigham Biobank. PATIENTS: A total of 6654 patients who underwent abdominal CT for clinical indications and had no diagnosis of diverticulitis, IBD, or cancer before the scan were included. MAIN OUTCOME MEASURES: Visceral…

Clinical impacts of utilizing ceftriaxone and metronidazole versus piperacillin/tazobactam in patients diagnosed with complicated diverticulitis.

BACKGROUND: The optimal antibiotic regimen to empirically treat complicated diverticulitis has not been well established in guidelines. METHODS: A 5-year retrospective cohort study was conducted with 322 patients admitted to Mayo Clinic hospitals for complicated diverticulitis. Outcomes for 89 patients treated with ceftriaxone and metronidazole were compared to 233 patients treated with piperacillin/tazobactam. Patients were included if they received one of the treatment options for at least 96 h during hospital admission and did not receive any other diverticulitis antibiotic treatment regi…

Decreased Duration of Hospital Stays in Uncomplicated Diverticulitis With Initiation of Enteral Feeds Within 24 Hours.

Diverticulitis is a complication of a mostly benign condition called diverticulosis. Diverticulitis is a common illness that medical staff see in emergency departments and hospital inpatient services. The thought process behind this study was looking at the treatment of other diseases like pancreatitis or other critical illnesses and the data supporting timing of feeds. Our study concludes that starting a diet within 24 h of uncomplicated diverticulitis is associated with reduced length of stay without increasing readmission rates and mortality.

Frailty in elderly patients with acute colonic diverticulitis is associated with worse in-hospital outcomes: a nationwide analysis.

BACKGROUND: Frailty has been identified as an independent predictor of mortality in the elderly. We investigated the effects of frailty status on in-hospital outcomes of acute colonic diverticulitis (ACD) in the elderly, using the Hospital Frailty Risk Score. METHODS: We used the National Inpatient Sample (NIS) databases from 2016-2020 to identify patients aged ≥75 years hospitalized with ACD. Using a 1:1 matching method, we created propensity-matched cohorts of frail (Hospital Frailty Risk Score ≥5) and non-frail (Hospital Frailty Risk Score ≤4) patients within the ACD population. RESULTS: …

Complicated appendiceal diverticulitis a extremely rare cause of acute abdomen: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Appendiceal diverticulitis (AD) represents a rare cause of acute abdomen. Diagnosis of AD is a challenge because of its rarity and resemblance to other ileocecal diseases like as cecal diverticulitis (CD) and acute appendicitis (AA). Preoperative imaging can be useful to aid diagnosis. Surgery represents the correct treatment of AD. CASE PRESENTATION: A 48-year-old Caucasian male presented to the Emergency Department with a two-day history of right lower quadrant (RLQ) abdominal pain and fever. Physical examination revealed RLQ abdominal pain and rebound tendernes…

The role of damage control surgery in the treatment of perforated colonic diverticulitis: a systematic review and meta-analysis.

INTRODUCTION: Damage control surgery (DCS) is the classic approach to manage severe trauma and has recently also been considered an appropriate approach to the treatment of critically ill patients with severe intra-abdominal sepsis. The purpose of the present review is to evaluate the outcomes following DCS for Hinchey II-IV complicated acute diverticulitis (CAD). METHODS: A comprehensive systematic search was undertaken to identify all randomized clinical trials (RCTs) and observational studies, irrespectively of their size, publication status, and language. Adults who have undergone DCS for…

Hinchey Ia acute diverticulitis with isolated pericolic air on CT imaging; to operate or not? A systematic review.

BACKGROUND: Colonic diverticulitis is one of the most common gastrointestinal pathologies and its prevalence increases with the aging of the population in Western countries. Approximately 15% of patients with Hinchey Ia acute diverticulitis present with concomitant isolated pericolic air bubbles that denote intestinal perforation and thus prompting a more "aggressive" treatment attitude, including emergency surgery, despite the absence of evidence-based indications. This study is trying to delineate whether this approach is justified or whether a conservative treatment would suffice for this g…

Three-stage laparoscopic surgery in a morbidly obese patient with Hinchey III diverticulitis: a case report.

BACKGROUND: Perforated diverticulitis with purulent peritonitis (Hinchey III diverticulitis) has traditionally been treated with a Hartmann's procedure in order to avoid the considerable postoperative morbidity and mortality associated with one-stage resection and primary anastomosis. Although there have been reports regarding laparoscopic lavage as the initial treatment of perforated Hinchey III diverticulitis, a formal treatment strategy has not been established yet. We performed a three-stage surgery, including laparoscopic lavage and drainage with diverting ileostomy (first stage), laparos…

Meckel's Diverticulum in Crohn's Disease Revisited: A Case of Meckel's Diverticulitis in a Patient with Stricturing Crohn's Disease.

According to anecdotal reports in literature, encountering Meckel's diverticulum in a patient with Crohn's disease is not uncommon, but differentiating between the overlapping complications of Mickel's diverticulum and the natural manifestations of Crohn's disease can be challenging and may impact lifelong therapy. In this report, we present a case of Meckel's diverticulitis in a patient with stricturing ileocolonic Crohn's disease. A 29-year-old male has been suffering from recurrent bouts of abdominal pain and diarrhea which were initially thought to be due to recurrent flares of Crohn's dis…

CT colonography versus colonoscopy in the follow-up of patients after diverticulitis - a prospective, comparative study.

AIM: To assess whether computed tomography colonography (CTC) is a viable alternative to colonoscopy or double contrast barium enema in the follow-up of patients after diverticulitis. MATERIAL AND METHODS: Fifty patients underwent CTC followed immediately by colonoscopy. Results were blinded to the examiners. Findings of diverticular disease and patient acceptance were evaluated. RESULTS: Bowel preparation and distension were good in the majority of CTC and colonoscopy examinations. Diverticular disease was found in 96% of patients at CTC and in 90% at colonoscopy. The rate of agreement betw…
