PURPOSE: Transimpedance matrix (TIM) imaging is an electrical field-based modality that visualizes intracochlear current spread and may detect electrode malposition and subtle cochlear pathology beyond radiological resolution. This study aimed to evaluate etiology-specific TIM heatmap patterns in adult cochlear implant recipients and to assess the utility of TIM as an adjunct to conventional imaging.
METHODS: This observational retrospective case series included 11 adult cochlear implant recipients (15 implanted ears) with defined etiologies: head trauma/temporal bone fracture (5 ears), post-

BACKGROUND: DYT6 dystonia can have an unpredictable clinical course and the result of deep brain stimulation (DBS) of the internal part of the globus pallidus (GPi) is known to be less robust than in other forms of autosomal dominant dystonia. Patients who had previous stereotactic surgery with insufficient clinical benefit form a particular challenge with very limited other treatment options available.
CASE REPORT: A pediatric DYT6 patient unexpectedly deteriorated to status dystonicus 1 year after GPi DBS implantation with good initial clinical response. After repositioning the DBS electrod

Bradykinesia and hypokinesia are the prominent symptoms of substantia nigra degeneration in Parkinson's disease (PD). In segmental dystonia, movements of not affected limbs are not impaired. Here we studied the impact of the mere implantation of stimulation electrodes on the performance of fast movements in these two groups. We investigated 9 PD patients with subthalamic electrodes and 9 patients with segmental dystonia with electrodes in the globus pallidus internum. Patients were studied on the first postoperative day without electrical stimulation of the electrodes. Subjects had to perform

Previous experiments have suggested that mechanical activity of electrode implants may contribute to the observed bone formation that has been attributed to the electrical currents. This was tested by implanting movable and stationary wires in the medullary canal of the rabbit femora or tibiae. The moving implants uniformly showed significant medullary bone formation at the wire, whereas the stationary ones did not. This bone persisted for as long as 8 weeks and was mostly resorbed by 12 weeks as the implants became fixed. The new trabecular bone closely resembled that typically seen at electr

We present a case of temporary cochlear nerve dysfunction due to endocochlear inflammation with subsequent recovery. Retrospective case review at a pediatric tertiary care hospital. A nine-year-old male presented seven years post-cochlear implantation with an electrode array extruded into the external auditory canal. Upon exploration in the operating room, turbid perilymph from the scala tympani was discovered. A new electrode array was implanted. The patient had no discernible neural response telemetry (NRT) responses or auditory perception immediately following the procedure. Continuous but

OBJECTIVE: To determine the optimal stimulus paradigm, electrode orientation, and configuration of an implantable stimulator used to reanimate the posterior-cricoarytenoid (PCA) muscle in case of bilateral vocal fold paralysis (BVFP).
STUDY DESIGN: Acute studies were conducted on 13 canines implanted with Itrel II systems with or without PCA innervation. PCA stimulus-response characteristics were obtained by measuring stimulated vocal fold displacement endoscopically.
RESULTS: The denervated PCA was only 10% to 25% as responsive to stimulation as the innervated PCA. However, the response cou

Between November, 1980, and January 1991, a total of 115 transvenous pacemakers were implanted in 102 patients at our hospital. Infection at the site of implantation developed in three cases or 2.6%. The median time of the onset of infection postoperatively was eleven and a half months, the range being two and a half to twenty-four months. Coagulase positive Staphylococcus aureus was cultured from the infected site of one patient and coagulase negative Staphylococcus epidermidis and saprophyticus in two patients. Staphylococcus aureus septicemia developed in one patient. Conservative medical t

Persistent left superior vena cava with coexisting absent right superior vena cava is rare with less than 150 cases in the literature. Various techniques for pacemaker implantation have been described in this situation. We report a 40-year-old man with sinus and atrioventricular nodal dysfunction who underwent dual chamber pacemaker implantation. We elected to implant the ventricular electrode down a left ventricular branch of the coronary sinus and the lead is stable at 4-month follow-up.

OBJECTIVE: This study compared two different versions of an electrophysiology-based software-guided cochlear implant fitting method with a procedure employing standard clinical software. The two versions used electrically evoked compound action potential (ECAP) thresholds for either five or all twenty-two electrodes to determine sound processor stimulation level profiles. Objective and subjective performance results were compared between software-guided and clinical fittings.
DESIGN: Prospective, double-blind, single-subject repeated-measures with permuted ABCA sequences.

One of the most sought-after applications of neuroengineering is the communication between the arm and an artificial prosthetic device for the replacement of an amputated hand or the treatment of peripheral nerve injuries. For that, an electrode is placed around or inside the median nerve to serve as interface for recording and stimulation of nerve signals coming from the fascicles that innervate the muscles responsible for hand movements. Due to the lack of a standard procedure, the electrode implantation by the surgeon is strongly based on intuition, which may result in poor performance of t

Vagus nerve stimulation (VNS) is a treatment for refractory epilepsy and depression. Previous studies using invasive recording electrodes showed that VNS induces laryngeal motor-evoked potentials (LMEPs) through the co-activation of the recurrent laryngeal nerve and subsequent contractions of the laryngeal muscles. The present study investigates the feasibility of recording LMEPs in chronically VNS-implanted rats, using a minimally-invasive technique, to assess effective current delivery to the nerve and to determine optimal VNS output currents for vagal fiber activation. Three weeks after VNS

Proper execution of voluntary movement requires a sensorimotor transformation based on the initial limb state. For example, successfully reaching to a stable target requires the recruitment of different muscle groups depending on limb position at movement initiation. To test whether this transformation could occur at the spinal level, we stimulated the cervical spinal cord of anesthetized monkeys while systematically changing initial posture and examined the modulation of the twitch response induced in the upper limb muscles. In three monkeys, a multichannel microelectrode array was implanted

BACKGROUND: For the treatment of medically refractory movement disorders such as Parkinson's disease, essential tremor and primary dystonia, deep brain stimulation (DBS) has become one of the main treatment options. The targets for implantation of the stimulation electrodes are various nuclei within the basal ganglia or the thalamic and subthalamic area. Accurate target localisation is of major importance for outcome and patient safety. The goal of this study was to evaluate the role of image fusion in the determination of target co-ordinates.
METHOD: We conducted a retrospective study on 10

Proactive sensory processing based on prediction is a fundamental ability that enables humans to respond appropriately to external events. The brain forms predictions based on sensory context and generates prediction error signals by comparing these predictions with actual sensory events, suggesting a mutually active exchange of information between prefrontal areas that generate predictions and sensory areas that represent incoming information. To elucidate the neural circuits involved in prediction and prediction error, we investigated functional coupling between prefrontal and temporal corti

Objective.Brain-computer interfaces (BCIs) have the potential to reinstate lost communication faculties. Results from speech decoding studies indicate that a usable speech BCI based on activity in the sensorimotor cortex (SMC) can be achieved using subdurally implanted electrodes. However, the optimal characteristics for a successful speech implant are largely unknown. We address this topic in a high field blood oxygenation level dependent functional magnetic resonance imaging (fMRI) study, by assessing the decodability of spoken words as a function of hemisphere, gyrus, sulcal depth, and posi

BACKGROUND: Cochlear implants have gained worldwide acceptance as a reliable method of rehabilitation of profoundly hearing-impaired patients. Due to thorough patient selection major postoperative complications rarely occur and are flap related in most cases. Deafness can develop during chronic suppurative otitis media, either coincidentally or secondary to the medical treatment; normally this condition is regarded as a contraindication for cochlear implantation. In cases with a mastoid cavity after surgical treatment for cholesteatoma, the electrode covered only by the epithelial lining will
