Gastrointestinal decontamination after poisoning. Where is the science?

The approach to the use of gastrointestinal decontamination procedures in the treatment of ingested toxins has changed in recent years. Many toxicologists and physicians have taken strong positions either for or against the use of emesis, gastric lavage, activated charcoal, or other procedures. What is the scientific basis for these positions? This article reviews and comments on the published studies comparing the effectiveness of these widely used procedures.

Toxicology and management of acute drug ingestions in adults.

We reviewed the current literature on the toxicology and management of acute drug ingestions in adults after a MEDLINE search of English-language journal articles. Information on toxicology was also acquired from relevant textbooks. Present statistics were gathered through information from the U.S. Department of Health and Human Services and the American Association of Poison Control Centers. All patients with acute drug ingestions should be treated with supportive care initially, and when stabilized, undergo active removal of the ingested substance, based on the diagnostic and prognostic asse

The role of adrenergic receptors in the initiation of vomiting and its gastrointestinal motor correlates.

We investigated the role of adrenergic receptors in the mechanisms of initiation of vomiting and its gastrointestinal (GI) motor correlates. The effects of clonidine, UK-14304, St-91, naphazoline, phenylephrine and isoproterenol were examined for their ability to initiate vomiting and its GI motor correlates. Only the alpha-2 adrenoceptor agonists UK-14304, clonidine, St-91 and naphazoline activated vomiting and its GI motor correlates. Tolerance of vomiting, but not its GI motor correlates, readily developed to all alpha-2 adrenergic receptor agonists but St-91. The responses to UK-14304 or c

Acute poisoning from the ingestion of Nicotiana glauca.

A case is presented of a 76-year-old male who ingested Nicotiana glauca and developed severe nicotine-alkaloid toxicity primarily characterized by neuromuscular blockade and respiratory failure. Recovery occurred with supportive care.

Detection of dopamine receptors in the area postrema.

[3H]Spiroperidol labeled dog area postrema membranes with high affinity (KD, 0.1 nM) and stereospecificity. The displacement of the radioligand by different dopamine agonists and antagonists indicated that the labeled sites were dopaminergic in nature. Guanine nucleotides decreased agonist affinity for the labeled sites without affecting the affinity for antagonists. Adenylate cyclase in the area postrema was stimulated by guanine nucleotides and sodium fluoride but not by dopamine. The results suggest that the dopamine receptor in the area postrema can be classified as D2-type receptors.

Role of the chemoreceptor trigger zone in histamine-induced emesis.

1. In unanaesthetized dogs, the emetic action of histamine was studied after its injection into the cerebral venricles through chronically implanted cannulae in order to elucidate the role of the chemoreceptor trigger zone (CT-zone), situated in the area postrema, for this emesis.2. On injection into the lateral cerebral ventricle, about 10 times larger doses of histamine (3 mg) were required regularly to produce emesis, and it occurred after a longer latency than on injection into the fourth ventricle. This is in accord with an action of histamine on the CT-zone.3. After bilateral ablation of

Complications of Intravenous Midazolam-Fentanyl Sedation in Children and Adults Undergoing Oral Surgery: A Retrospective Study.

Objective: This study examines the incidence and predictors of complications following intravenous (IV) sedation in children and adults. Met … prolonged sedation remain concerns, particularly in younger children and females. Prophylactic anti-emetics and cautious Ketamine use may help mitigate risks.

Peri-Operative Outcomes Comparison of General Anaesthesia and Brachial Plexus Block for Open Reduction Internal Fixation of Metacarpal Fractures.

INTRODUCTION: Brachial plexus blockade (BPB) is a procedure of growing popularity amongst surgeons and anaesthesiologists involved with uppe … post-operatively versus 12/48 in the BPB (P < .001). Of all, 25/52 and 5/48 patients required anti-emetics post-operatively in the GA and BPB groups, respectively (P < .001). Need for any parenteral medication post-operatively was also significantly different (P < .001). CONCLUSION: This is the first study to specifically examine anaesthetic techniques …

Neuraxial anesthesia for post-operative pain control after hip surgery in children with cerebral palsy and pre-existing intrathecal baclofen pumps.

PURPOSE: The purpose of this study is to describe the efficacy and adverse events of neuraxial anesthesia for post-operative pain control in … al pain score of 0.7 and mean morphine equivalent use of 0.26mg/kg/day. Four patients required anti-emetics to control nausea and three patients had urinary retention requiring repeat catheterization, but all medical complications resolved prior to discharge. CONCLUSION: Neuraxial anesthesia can effectively control post-operative pain in children with a …

Availability, Toxicology and Medical Significance of Antimony.

Antimony has been known and used since ancient times, but its applications have increased significantly during the last two centuries. Aside … Despite its evident and pronounced toxicity, it has also been used in some drugs, initially tartar emetics and subsequently antimonials. The latter are used to treat tropical diseases and their therapeutic potential for leishmaniasis means that they will not be soon phased out, despite the fact the antimonial resistance is beginning to be documented. The …

Cannabinoids, Blood-Brain Barrier, and Brain Disposition.

Potential therapeutic actions of the cannabinoids delta-9-tetrahydrocannabinol (THC) and cannabidiol (CBD) are based on their activity as analgesics, anti-emetics, anti-inflammatory agents, anti-seizure compounds. THC and CBD lipophilicity and their neurological actions makes them candidates as new medicinal approaches to treat central nervous system (CNS) diseases. However, they show differences about penetrability and disposition in the brain. The present article is an overview about THC and CBD crossing the blood-brain barrier (BBB) and their brain disposition. Several findings indicate tha…
