Parietal spikes can be evoked in some children by tactile stimulation of their feet or other parts. I investigated whether this curious phenomenon remains unchanged for several years and then disappears or whether it presages later epilepsy. Some children demonstrating this phenomenon may experience a spontaneous focus of spikes during sleep or waking and later a convulsive state. The cause of this condition may be hyperexcitability of a small cortical parietal area.

Generalized corticoreticular epilepsy was established in adult cats by parenteral penicillin, and electroencephalographic monitoring was carried out. Ketamine HCl was injected intravenously in doses of 2.5 to 20 mg per kilogram. If doses of penicillin were inadequate to establish typical spike-wave activity, ketamine induced the spike-wave pattern typical of much higher doses of penicillin. At doses of penicillin that established the spike-wave pattern, ketamine potentiated the spike-wave activity and sometimes induced spike-and-wave status. These findings suggest caution in the clinical use o…

In a two-year study of patients admitted to hospital after deliberate self-poisoning or self-injury, a fivefold excess of patients with epilepsy was found compared with general population prevalence rates. Males with epilepsy were particularly over-represented. Patients with epilepsy were prone to make repeat attempts. Anticonvulsants, particularly barbiturates, were used in most cases of self poisoning.

A young woman, with negative family history, committed suicide after 6 years of myoclonus and mental deterioration. The neurons contained atypical myoclonus bodies which differed from the classical type in distribution, histochemistry and light and electron microscopic appearance.

Genetic investigation of 74 patients with temporal lobe epilepsy, resistant to medication, who underwent unilateral temporal lobectomy 1960-1969. Thirty per cent had a positive family history of epilepsy; 49 per cent of other neurological diseases; and 46 per cent of major psychiatric disorders. Neither of the genetic factors per se influenced the therapeutic result or the psychiatric status at follow-up. A multiple epileptic or neurological predisposition affecting first degree relatives unfavourably influenced the prognosis regarding relief from seizures. A multiple psychiatric predispositio…

In a prospective study of thirty-two children with seizures treated with phenytoin (diphenylhydantoin), five had low levels of serum-IgA before treatment. All of these were among the fifteen who had had febrile convulsions in infancy. IgA levels fell significantly during 6 months treatment in the fourteen patients studied sequentially. Treated children with low serum-IgA had normal numbers of lymphocytes with surface IgA. This suggests that phenytoin causes failure of terminal differentiation of B lymphocytes, and is the first known cause of this, the commonest mechanism of immunoglobulin defi

OBJECTIVES: Distinguishing Idiopathic Generalized Epilepsy (IGE) from focal epilepsy (FE) can be challenging when IGE cases present with asymmetric clinical or electroencephalographic (i.e., atypical) features. We aimed to identify factors that lead to discordance in IGE diagnoses among epileptologists.
METHODS: 41 patients with IGE and 6 patients with FE or mixed focal and generalized epilepsy followed at the NYU Comprehensive Epilepsy Center for ≥5 years were identified. IGE cases included typical (n = 18) and atypical (n = 23) presentations. Anonymized summaries of patients at initial pres…

Knowledge in the spectrum of epidermal nevi and epidermal nevus syndrome has been elaborated on and expanded rapidly through the years. In this review, we provide a comprehensive overview of the neurologic manifestations of epidermal nevus syndromes (ENS), with a particular focus on epilepsy, which represents one of the most disabling and frequent systemic complications. Although ENS comprises a heterogeneous group of neurocutaneous syndromes unified by the presence of epidermal hamartomas and extracutaneous involvement, the relationship between genotype, cutaneous phenotype, and seizure pheno…

Neuropeptides commonly function as co-transmitters alongside classical neurotransmitters within the same neuron. In temporal lobe epilepsy (TLE), the expression of numerous neuropeptides in the brain is profoundly altered. These changes may reflect activity-dependent regulation of the respective neuronal populations-often associated with enhanced neuronal firing and co-release of neuropeptides with classical transmitters-but may also exert direct functional effects through activation of peptide-specific receptors. This review summarizes alterations in neuropeptide systems in experimental model…

Pyridoxine-dependent epilepsy (PDE) is a rare autosomal recessive metabolic epilepsy classically presenting with refractory neonatal seizures that respond to pharmacological doses of pyridoxine (vitamin B6). Although traditionally regarded as a neonatal disorder, later presentations are increasingly recognised. We report an adolescent patient with long-standing treatment-resistant epilepsy and learning disability who was ultimately diagnosed with PDE. Whole genome sequencing identified compound-heterozygous pathogenic variants in ALDH7A1 The diagnosis was further supported by elevated urinary …

BACKGROUND: Surgical treatment is possible for drug-resistant epilepsy. Localization of epileptogenic zone in temporal lobe or outside it influences the effectiveness of surgical treatment.
OBJECTIVE: To evaluate the effectiveness of surgical treatment of drug-resistant epilepsy.
MATERIAL AND METHODS: Postoperative outcomes were analyzed in patients with drug-resistant epilepsy between 2016 and 2022.
RESULTS: There were 346 resections and 66 palliative surgeries. Effectiveness of resections (Engel I outcome) was 79% for temporal epilepsy and 62% for extratemporal epilepsy.
CONCLUSION: Our …
