Management and outcome of pediatric skull base fractures.

OBJECTIVE: The management of skull base fractures in the pediatric age group continues to be a major challenge even for experienced multidisciplinary teams. This retrospective study was undertaken at a tertiary care academic hospital to evaluate the management and outcome of pediatric skull base fractures. METHODS: Retrospective analysis covering a period of 13.5 years (from 1996 to 2009) and 63 patients (mean age 10.7 years; range 1-18 years) was performed. RESULTS: A road traffic accident was the most frequent etiological factor (38%). The most common skull base fracture type was temporal

Pediatric Nasoorbitoethmoid Fractures: Cause, Classification, and Management.

BACKGROUND: Currently, there is a paucity of information on the presentation and proper management of pediatric nasoorbitoethmoid fractures. The purpose of this study was to examine the incidence, cause, associated injuries, and management of these fractures. Furthermore, the authors sought to assess outcomes after transnasal wiring or suture canthopexy for type III nasoorbitoethmoid fractures. METHODS: A retrospective cohort review was performed of all patients with nasoorbitoethmoid fractures who presented to a Level I trauma center from 1990 to 2010. Charts and computed tomographic imaging

A Rare Presentation of Alveolar Rhabdomyosarcoma of the Sinonasal and Nasopharyngeal Region With Bone Marrow Metastasis.

We report the case of a Hispanic woman in her 50s who presented to the emergency department with persistent hip and left lower extremity pain, followed by progressive anosmia and a right-sided neck mass. On physical examination, the patient had a palpable lymph node in the right submandibular area, and she reported a history of anosmia. A CT scan of the neck and face revealed an enhancing mass in the superior nasal cavity/ethmoid sinus with cribriform plate dehiscence and pachymeningeal enhancement and a right cervical lymph node. A biopsy of the lymph node revealed alveolar rhabdomyosarcoma (

Rostral cranial fossa and sinonasal neoplasms with cribriform plate involvement in 32 dogs and 17 cats.

The rostral cranial fossa (RCF) consists of the sphenoid and ethmoid bones, which accommodate the olfactory bulbs and nerves along the recesses of the cribriform plate. Neoplasms located in the vicinities of the RCF can compress and/or invade the cribriform plate. Here we describe the clinical and pathologic findings of neoplasms involving the cribriform plate in 32 dogs and 17 cats autopsied over a 13-y period. The average ages of affected dogs and cats were 9.2 y and 9.7 y, respectively. No sex or breed predisposition was evident in dogs, but 13 of 18 cats were spayed females and 14 of 18 we…

Sudden Unilateral Visual Loss Due to an Onodi Cell Mucocele.

In a patient whose sudden vision loss is suspected on the side of a previously operated sphenoid or ethmoid sinus, an ischemic insult to the optic nerve may occur due to a compressing mucocele, especially when there is bony dehiscence of the orbital apex. We report a case of a 41-year-old healthy male patient who had a history of previous sinus surgery, and visited the emergency department for an abrupt left ocular pain following visual loss of the affected eye. The patient's left eye's best corrected visual acuity (BCVA) was hand motions with a reduced light reflex. A sphenoethmoidal sinus ce

Anatomical Changes in a Case with Asymmetrical Bilateral Maxillary Sinus Hypoplasia.

Background and Objectives: The maxillary sinus hypoplasia (MSH) is an occasional variation of the maxilla, occurring either unilaterally or bilaterally. Previous studies dealing with MSH have not detailed the consequent anatomical changes of the maxilla and adjacent fossae. Materials and Methods: A 58-year-old female case was scanned in Cone Beam Computed Tomography and found to have asymmetrical bilateral MSH, who was then further evaluated anatomically. Results: The maxillary sinuses were hypoplastic and had mild mucosal thickenings. The orbital floors were curved. The uncinate process and t

Medial maxillary (nasomaxillary buttress) fracture: Two case reports.

RATIONALE: Medial maxillary fractures are defined as fractures of the medial aspect of the maxilla, that is, fractures of the nasomaxillary buttress. The diagnosis of medial maxillary (nasomaxillary buttress) fractures is challenging because of its similarities with nasal, and naso-orbito-ethmoidal (NOE) fractures. PATIENT CONCERN: The patients included a 17-year-old Japanese man and a 37-year-old Japanese woman with facial injuries. Swelling of the affected cheek, nasal hemorrhage, periorbital ecchymosis, and C-shaped deviation of the nasal dorsum with normal nasal tip and root position were

Cranial Pair 0: The Nervus Terminalis.

Originally discovered in elasmobranchs by Fritsh in 1878, the nervus terminalis has been found in virtually all species, including humans. A … g in the nasal cavity passes into the cranium through the middle area of the cribiform plate of the ethmoid bone. Intracranially, fibers joint the telencephalon at several sites including the olfactory trigone and the primordium of the hippocampus to reach preoptic and precommissural regions. The nervus terminalis shows ganglion cells, that sometimes form clus …

Wound healing in the rabbit paranasal sinuses after Coblation: evaluation for use in endoscopic sinus surgery.

BACKGROUND: Bipolar radiofrequency can be used surgically to excise and cauterize tissue simultaneously. It has potential for use in endoscopic sinus surgery (ESS). This study was performed to determine the extent and pattern of injury in the paranasal sinuses with bipolar radiofrequency and evaluate wound healing. METHODS: Eight rabbits underwent Coblation of maxillary sinus mucosa with biopsy immediately, on postoperative day (POD) 3, 7, 14, or 29. Maxillary mucosa was exposed through the nasal dorsum, and a Coblator PROciseXP wand used on a power setting of 7 for 2 seconds. Three of the ra

Paranasal sinus fungus ball: diagnosis and management.

Paranasal sinus fungus ball is an extramucosal mycosis, usually occurring in immunocompetent people as a monolateral lesion. To review the literature data and to report the Policlinico S. Matteo, University of Pavia experience, 81 patients presenting paranasal fungus ball have been treated (January 1994 to May 2005). Twenty-seven men and 54 women (19-91 years old; mean 49.4 years) were considered. Seventy-three patients had a single sinus affected, but eight presented multiple localisations. Maxillary was the most involved sinus followed by sphenoidal and ethmoidal. Moulds have been isolated i

Epidermoids of the paranasal sinuses and beyond: endoscopic management.

BACKGROUND: Epidermoid tumors of the craniofacial skeleton are uncommon, with the exception of acquired cholesteatoma of the temporal bone. These lesions may be primary embryologic in origin or, less frequently, may be iatrogenic or posttraumatic. METHODS: We report three cases of unusually encountered epidermoids, all of which were managed by endoscopic marsupialization via paranasal sinus approaches. We also review the pathophysiology and clinical presentation of these rare entities. RESULTS: The first patient is a 60-year-old man with a congenital epidermoid of the petrous apex removed vi

The anatomy and histology of the nasal cavity of the koala (Phascolarctos cinereus).

The anatomy of the nose of the koala was studied from fixed 0.5 cm thick sections of a whole head. Right and left nasal cavities are separated by a slender septum which does not exhibit 'swell bodies'. Dorsal and ventral conchae are simple curved plates without elaborate scrolls; the ventral concha is recurved to form a bulla. The nasal cavity communicates with confluent rostral maxillary and frontal air sinuses. A ventrocaudal maxillary sinus opens from the ventral meatus close to the choanae. There is a frontal sinus dorsocaudal to the ethmoid region which does not communicate with the nasal

Basilar skull fractures in children.

In this paper we are reporting a retrospective study of patients under 18 years of age managed at the Los Angeles County/University of Southern California Medical Center from January 1979 through December 1987 with the diagnosis of basilar skull fracture. Sixty-two patients with basilar skull fractures were admitted during that 7 1/2 year period. The most common etiology was pedestrain versus vehicle accidents (42%), followed by falls (27%), vehicle accidents (23%), and being hit by an object (8%). The most common physical findings were hemotympanum (58%) and bleeding in the ear canals (47%).

Frontobasilar fractures in children.

OBJECT: Forehead, anterior cranial base and orbito-naso-ethmoidal fractures, combined with brain injuries and dural tears, constitute a frequent pattern of injury in infants and children less than 5 years of age when major anterior craniofacial trauma occurs. Fractures of the orbital roof, despite the common blow-out floor fractures, are considered uncommon events. In children younger than 7 years this pattern of fracture may be a consequence of nonpneumatized frontal sinuses. METHODS: Complete assessment using CT scans combined with neurosurgical, ophthalmological, anesthesiological and cran

Craniospinal giant cell tumors: clinicoradiological analysis in a series of 11 cases.

Primary craniospinal giant cell tumors are rare (5-15% of all giant cell tumors), locally destructive bone lesions which are generally not associated with Paget's disease. Clinical behavior of the craniospinal giant cell tumor (GCT) is unpredictable but often very aggressive and therefore optimal management of the lesions remains controversial. Wide resection of the involved bone is required, preferably with a wide margin of normal tissue, which may be difficult to achieve in the craniospinal region. The authors analysed a series of 11 patients, which included five cases of cranial and six cas

Intraoperative lacrimal intubation to prevent epiphora as a result of injury to the nasolacrimal system after fracture of the naso-orbitoethmoid complex.

Treatment of fracture of the naso-orbitoethmoid (NOE) complex is difficult. There are not only aesthetic issues but also functional consequences related to the lacrimal system. Because prophylactic lacrimal intubation for such fractures remains controversial, we have assessed the effectiveness of intraoperative lacrimal intubation to prevent epiphora as a result of such injuries. Thirteen patients diagnosed with craniomaxillofacial fractures including fractures of the NOE complex were included in the study; 10 had unilateral fractures and 3 bilateral. Computed tomography (CT) showed all patien
