The benefit of exercise training in pulmonary hypertension: a clinical review.

Pulmonary hypertension (PH) is a clinical condition characterised by raised pulmonary artery pressure, which results in increased right ventricular afterload and dyspnoea. This is accompanied by reduced exercise capacity, quality of life and, eventually, death. An increasing range of targeted medications has transformed the treatment of pulmonary arterial hypertension, a specific type of PH. Supervised exercise training is recommended as part of a multifaceted management plan for PH. However, many questions remain regarding how exercise training improves exercise capacity and quality of life.

Limitations of self-reported estimates of functional capacity using the Walking Impairment Questionnaire.

OBJECTIVE: A potential issue with the Walking Impairment Questionnaire (WIQ) is that it is relatively complex. We estimated the number of errors made by patients when self-completing the WIQ, and assessed the benefit of correcting missing, duplicate or paradoxical (i.e., reported lower difficulty for a higher-intensity task) answers. DESIGN: Prospective non-interventional study. MATERIALS: All consecutive new patients with claudication over a 3-month period. METHODS: The WIQ was self-completed before patients performed a constant-load treadmill walking test (maximised to 750 m). MAIN OUTCO

Inspiratory muscle training in adults with chronic obstructive pulmonary disease: an update of a systematic review.

The purpose was to update an original systematic review to determine the effect of inspiratory muscle training (IMT) on inspiratory muscle strength and endurance, exercise capacity, dyspnea and quality of life for adults with chronic obstructive pulmonary disease (COPD). The original MEDLINE and CINAHL search to August 2003 was updated to January 2007 and EMBASE was searched from inception to January 2007. Randomized controlled trials, published in English, with adults with stable COPD, comparing IMT to sham IMT or no intervention, low versus high intensity IMT, and different modes of IMT were

Six-minute walk test in children with chronic conditions.

OBJECTIVES: The 6-minute walk test (6MWT) is a frequently used indicator of functional exercise capacity. The goals of this study were to compare the 6-minute walk performance of three paediatric patient groups with that of healthy peers, to assess differences between published reference values and to investigate which anthropometric characteristics best predict 6-minute walk performance. METHODS: 47 children with haemophilia (mean (SD) age 12.5 (2.9) years), 44 with juvenile idiopathic arthritis (JIA) (mean age 9.3 (2.2) years) and 22 with spina bifida (SB) (mean age 10.3 (3.1) years) were i

Ventilatory and lactic thresholds in subjects with sickle cell trait.

This study investigated 1) whether ventilatory and lactic thresholds (VT and LT, respectively) are different in sickle cell trait carriers (SCTc) and subjects with normal hemoglobin (control group), and 2) whether the first LT and VT and the second LT and VT are respectively coincident in the two populations. Seven SCTc and 8 control subjects performed an incremental exercise test (IET). Blood lactate concentration and cardioventilatory variables were analyzed at rest and during IET. No significant difference in the ventilatory parameters (notably, maximal oxygen uptake [VO (2max)] and the ven

Dyspnoea at rest and at the end of different exercises in patients with near-fatal asthma.

Blunted perception of dyspnoea under resistive loading has been observed in patients with a history of near-fatal asthma (NFA). The perception of dyspnoea at rest and at the end point of various exercises was assessed in such patients. Respiratory function and exercise capacity (6-min walking distance, incremental cycloergometry and inspiratory threshold loading) were assessed in seven NFA and eight non-NFA patients. Dyspnoea (Borg scale) was measured at rest and at the end point of the various exercises. Dyspnoea at rest was significantly lower in NFA patients. Although exercise tolerance was…

Exercise training for bronchiectasis.

BACKGROUND: Bronchiectasis is characterised by excessive sputum production, chronic cough, and acute exacerbations and is associated with symptoms of dyspnoea and fatigue, which reduce exercise tolerance and impair quality of life. Exercise training in isolation or in conjunction with other interventions is beneficial for people with other respiratory diseases, but its effects in bronchiectasis have not been well established. OBJECTIVES: To determine effects of exercise training compared to usual care on exercise tolerance (primary outcome), quality of life (primary outcome), incidence of acu…

Low-Intensity Pulsed Ultrasound (LIPUS) Mitigates Exercise-Induced Myocardial Fatigue by Optimizing Glucose Oxidation Via the Aldolase/AMPK/PDH Signaling Axis.

BACKGROUND: Myocardial energy deficiency is a key bottleneck limiting exercise endurance. Low-intensity pulsed ultrasound (LIPUS) has emerged as a non-invasive metabolic regulator. This study investigates whether LIPUS can activate glucose-centric pathways to alleviate exercise-induced myocardial fatigue. METHODS: Sprague-Dawley rats were subjected to a 4-week exhaustive swimming protocol. LIPUS (1 MHz, 30 mW/cm², 20% duty cycle) was administered precordially for 30 min/day. We assessed exercise tolerance, cardiac injury markers (cTnI, NT-proBNP), mitochondrial ultrastructure, and metabolic f…

Exercise and the lungs: From physiology to pathophysiology - a narrative review.

Exercise represents a dynamic integration of rapidly activated respiratory, cardiovascular and musculoskeletal responses to meet rising metabolic demands. While the human body has a remarkable ability for exertion and endurance, in disease states, and particularly in respiratory disease, these mechanisms are often compromised, resulting in dyspnoea and reduced exercise tolerance. This review outlines normal exercise physiology, including cardiac, ventilatory and skeletal muscle adaptations to enhance oxygen uptake and delivery to working muscle, and examines how these are altered in conditions…

Behavioral Exposure for Interoceptive Tolerance (BE-FIT): A stage II randomized clinical trial protocol.

BACKGROUND: Exercise anxiety is a novel mechanism related to non-adherence to exercise and lifestyle physical activity. We developed a cognitive-behavioral treatment, Behavioral Exposure For Interoceptive Tolerance (BE-FIT), which is a manualized, values-based exposure intervention designed to target exercise anxiety that is delivered as a supplement to outpatient cardiac rehabilitation (CR). METHOD: We describe a Stage II randomized controlled trial (RCT) to test BE-FIT, compared to a Health Education Control (HEC) intervention on exercise and physical activity outcomes at end-of-treatment (

Exercise training in metabolic myopathies.

Metabolic myopathies encompass muscle glycogenoses (GSD) and disorders of muscle fat oxidation (FAOD). FAODs and GSDs can be divided into two main clinical phenotypes; those with static symptoms related to fixed muscle weakness and atrophy, and those with dynamic, exercise-related symptoms that are brought about by a deficient supply of ATP. Together with mitochondrial myopathies, metabolic myopathies are unique among muscle diseases, as the limitation in exercise performance is not solely caused by structural damage of muscle, but also or exclusively related to energy deficiency. ATP consumpt

Influence of metformin on exercise metabolism and capacity: a systematic review and meta-analysis.

Metformin and exercise are often coprescribed in adults with prediabetes and type 2 diabetes mellitus. However, metformin may alter metabolic responses to exercise. This systematic review and meta-analysis evaluated the impact of metformin on exercise metabolism, including lactate, glucose, insulin, and free fatty acids, along with exercise capacity. Electronic databases (Web of Science, PubMed/Medline, Embase, and SPORTDiscus) were searched through January 2025. Twenty-one studies that involved acute (single dose), nonacute (multiple doses), and habitual metformin (prescribed) intake were inc

The iATTEND Trial: A Trial Comparing Hybrid Versus Standard Cardiac Rehabilitation.

The improving ATTENDance (iATTEND) to cardiac rehabilitation (CR) trial tested the hypotheses that hybrid CR (HYCR) (combination of virtual and in-facility CR sessions) would result in greater attendance compared with traditional, facility-based only CR (FBCR) and yield equivalent improvements in exercise capacity and health status. Patients were randomized to HYCR (n = 142) or FBCR (n = 140), stratified by gender and race. Attendance was assessed as number of CR sessions completed within 6 months (primary end point) and the percentage of patients completing 36 CR sessions. Other end points (t
