Submuscular Biological Plating Versus Flexible Nailing With External Fixation for Treating Unstable Pediatric Femoral Fractures.

BACKGROUND: The optimal surgical strategy for length-unstable pediatric femoral shaft fractures remains debated. This study compared clinical and radiologic outcomes between submuscular biological plating (SBP) and flexible intramedullary nailing augmented with external fixation (FIMN/EF). METHODS: In this prospective, randomized trial, 32 children (5 to 14 years) with unstable femoral fractures (AO/OTA 32-D/5.1 or 32-D/5.2) were allocated to SBP (n=16) or FIMN/EF (n=16). Primary outcomes included operative time, fluoroscopy time, time to union, complications, and knee range of motion (ROM).

External fixation versus reverse shoulder arthroplasty for proximal humerus fractures in the elderly: a retrospective comparative study.

BACKGROUND: The optimal management of displaced proximal humerus fractures (PHFs) in the elderly remains a subject of ongoing debate. This study aims to compare the clinical and functional outcomes of biologically driven external fixation (EF) versus functional joint replacement via reverse shoulder arthroplasty (RSA) in patients aged 65-80 years. METHODS: A retrospective comparative study was conducted on patients with displaced Neer two- or three-part PHFs treated between 2015 and 2022. The final analysis included 67 patients: Group A (EF; n=34) and Group B (RSA; n=33). Clinical and function

Quantitative assessment of pelvic reduction using a pretensioned external fixator in unstable pelvic injuries: a retrospective study.

INTRODUCTION: Conventional anterior external fixation provides effective stabilization in pelvic injuries but has limited ability to reduce and control posterior pelvic ring displacement, particularly in type C fractures. Although several anterior fixation systems capable of posterior compression have been described, many remain complex and impractical in emergency settings. PURPOSE: To evaluate the effectiveness of a pretensioned pelvic external fixator (PPEF) as a reduction tool in unstable pelvic fractures across different clinical scenarios. METHODS: A retrospective study was conducted i

Evaluation of continuous axial stress on tibial fracture healing in canine models using custom external fixator.

OBJECTIVES: To investigate the effects of physiologically relevant continuous axial stress on tibial fracture healing in canines using a custom external fixator. METHODS: A custom external fixator was developed to apply quantifiable stress. First, maximum axial stress derived from muscle contraction was measured using a film sensor. Subsequently, canine tibial fracture models were established and assigned to non-stress, half-maximum stress, or maximum stress groups. Healing was evaluated via radiography, micro-computed tomography (Micro-CT), biomechanical testing, Western Blot, and Real-time

Increased risk of complications associated with magnetic resonance imaging prior to external fixation in unstable knee dislocations.

INTRODUCTION: Unstable knee dislocations (KDs) may be assessed with magnetic resonance imaging (MRI) and treated with knee spanning external fixation (ex-fix) prior to definitive multiligament reconstruction. The order in which ex-fix and MRI are performed is debated due to the risk of complications. The following study evaluates the risks associated with MRI completion before versus after ex-fix placement, including the risk of redislocation compared to the risk of MRI related injury. METHODS: KDs treated with knee spanning ex-fix were retrospectively reviewed at a level-I academic center ov

Clinical application of external fixation in preventing displacement of high-position airway stents: 2 case reports.

RATIONALE: Airway stenosis and tracheoesophageal fistulas are life-threatening emergencies, often complicated by asphyxia, recurrent infections, and fatal outcomes. Tracheal stent implantation is effective, but stent migration remains a critical complication, especially in subglottic high-position airway pathologies. This report aims to demonstrate an external stabilization technique to prevent stent migration in this high-risk group. Such cases are rarely reported. PATIENT CONCERNS: Two representative clinical cases with subglottic high-position airway pathologies requiring stent implantatio

External Fixation of Lower Extremity Injuries in an Austere Environment: A Technique for Safe Application Without the Use of Fluoroscopy.

External fixation is a fundamental technique for the initial care of major extremity trauma. External fixation is used to provide temporary stability in both the damage control setting and environments where resources for definitive fixation are not readily available or definitive fixation is ill-advised. In the austere or resource-constrained environment-particularly in the battlefield setting or in disaster response-external fixation has demonstrated to be a reliable and efficient method for stabilizing the injured extremity without notable disruption of the soft-tissue envelope. Although in

Innovative approach to distraction osteogenesis using a novel external fixator design. In vivo experimental rabbit model.

Distraction osteogenesis is an established surgical technique for bone lengthening. The procedure involves making osteotomies, in the bone and using a device to gradually separate, or "distract," the bone fragments. New Zealand white male rabbits underwent distraction osteogenesis on the right tibia and lengthening using external fixators at two different time points: a speed of 1.4 mm/day in 15 days and 2.1 mm/day in 10 days. Radiological analysis was used to determine the distraction osteogenesis rates during tibia lengthening in rabbits. The regenerated callus was carefully removed and subj…

Distraction osteogenesis in combined sequential use of external fixation and nailing (lengthening and then nailing): An experimental study in rabbits.

Limb lengthening relies on the process of distraction osteogenesis. The active periosteal bone formation has been detected in clinical practice with a lengthening and then nail (LATN) technique but has not been confirmed by experimental studies to date. The aim of this study is to compare the tissue regeneration of the distraction regenerate during tibial lengthening in rabbits using a LATN technique. This study was performed on 54 mature rabbits of the Soviet Chinchilla breed, which were divided into three groups of 18 animals. In group 1 (control), the tibia was lengthened in an external fix

Orthoplastic Reconstruction of Distal Tibia High-Energy Fractures Using a Circular External Fixator-A Systematic Review.

Background: Compound fractures of the distal tibia (with or without the middle third) represent a challenge for orthopedic and plastic surgeons because of the scarcity of available soft tissue reconstruction and the important comminution of the fractures that usually appear. Methods: The design of this study is based on the PRISMA guidelines. Databases were searched for articles published and available until the first half of 2023. Articles that presented the evolution of patients treated by combining circular external fixators with reconstructive methods were selected. Results: After searchin…

Minimally invasive technique combined with external fixator in the treatment of pediatric flexion-type humeral supracondylar fractures.

Flexion-type pediatric humeral supracondylar fractures are rare, and the reduction technique remains contradictory. A minimally invasive technique using percutaneous leverage reduction combined with an external fixator was described to achieve satisfactory reduction and avoid the open reduction in this study. The operation and clinical results of patients treated with this technique were retrospectively compared with traditional closed reduction. From January 2013 to January 2018, children diagnosed with displaced flexion-type humeral supracondylar fractures were included in this study. Patien

Curettage and external fixation for displaced pathological fracture through non-ossifying fibroma in the radius: A case report.

Non-ossifying fibromas are extremely rare in the upper extremity, such as those in the radius and humerus. The treatment of completely displaced fractures following non-ossifying fibromas in the radius has not been reported. We present the case of a pathological fracture caused by a non-ossifying fibroma in the radius treated using external fixation. The patient was a 10-year-old girl who presented to our hospital with right forearm pain after an accidental fall. She had no remarkable medical history. Radiographs and computed tomography showed a pathological fracture through osteolytic lesions
