Extremely marginal liver grafts from deceased donors have outcome similar to ideal grafts.

BACKGROUND: Although there is a worldwide need to expand the donor pool, many cadaveric marginal livers are usually discarded for transplantation. Herein, we report the outcome of a series of patients receiving marginal grafts. METHODS: We analyzed all patients who underwent liver transplantation in our unit from August 2006 to March 2011 (n = 125) with the use of a prospectively collected database. Patients with ≥3 of donor (prolonged hypotensive episodes, donor age >55 years, high vasopressor drug requirement, hypernatremia, prolonged intensive care unit stay, elevated transaminases) and gr

Clampless anastomosis with an intraluminal thermosensitive gel: first application in reconstructive microsurgery and literature review.

Microvascular clamps of various designs provide a bloodless field for a safe anastomosis but can cause intimal lesions, occupy space in confined sites and have a risk of backwalling due to vessel flattening. They are often insufficient in their haemostatic effect in plaque-filled atherosclerotic vessels. A new, CE-certified thermosensitive gel (LeGoo™) clinically proven in cardiovascular surgery allows a clampless microanastomosis technique. We operated on a series of five consecutive patients aged 24-71 years with six flaps for lower-extremity reconstruction using a clampless anastomosis tech

Repeated local implantation of autologous peripheral blood mononuclear cells for the treatment of ischaemic digits in patients with connective tissue diseases.

OBJECTIVES: Blood flow in fingers and toes is often impaired in patients with CTDs, mandating amputation in some. Our previous study showed that a single implantation of autologous bone marrow cells is as effective as peripheral blood mononuclear cells (MNCs) to restore impaired blood flow, although neither has long-lasting efficacy. In this study, autologous peripheral blood MNCs were implanted repeatedly to evaluate a better efficacy. METHODS: Three patients with SSc, two with microscopic polyangiitis and one with MCTD were enrolled. All patients had severe RP, ulcers and/or necrosis in the

Brachial plexus palsy caused by secondary fracture displacement in a patient with closed clavicle fracture.

In adults, brachial plexus injury due to clavicle fractures is rare, and is most commonly caused by nonunion, malunited fragments, hypertrophic callus, or pseudoaneurysm of the subclavicular artery or vein. Brachial plexus palsy in acute fractures caused by direct fragment compression is exceptional. Conservative treatment of nondisplaced and displaced midclavicle fractures in adults usually produces satisfactory outcomes. This article presents a case of a 74-year-old man who sustained a closed, midshaft right clavicle fracture complicated by secondary displacement and brachial plexus injury.

Mercury in human hair as an indicator of the fish consumption.

OBJECTIVES: Mercury and most of its compounds are extremely toxic and should be handled with care. It can be inhaled and absorbed through the skin and mucous membranes. The most toxic forms of mercury are its organic compounds such as dimethylmercury and methylmercury. Fish have a natural tendency to accumulate mercury. Methylmercury is produced by microbial methylation of inorganic mercury in water sediment then it infiltrates the food chain and it consequently accumulates in fish. Fish are the main source of methylmercury in human food. Mercury is transferred into a hair; and this can be tha

Magnesium deficiency in human subjects--a personal historical perspective.

Over the past 30 years human magnesium (Mg) deficiency has become an accepted fact in most medical circles. Our index patient had striking neurological manifestations including generalized tremulousness, grimaces and fibrillary twitches of facial muscles, athetoid and choreiform movements of upper extremities, dysphagia, inability to speak, repeated convulsions, and confusion. She had received glucose in water and saline intravenously for several months. A patient with chronic alcoholism was noted to have almost identical symptoms and signs as the index patient. He also responded dramatically

Can Prolonged Ischemia Time in Extremity Transfer Be Resolved Using an Extracorporeal Circulation Model? An Experimental Study.

BACKGROUND: The standard method for transporting tissues during limb transplantation and replantation is cold ischemic transport (CIT). However, CIT cannot completely prevent ischemia-reperfusion injury (IRI). As an alternative, extracorporeal perfusion (ECP) methods that provide an adequate metabolic environment for ischemic tissues could be considered. In this study, we investigated the differences between CIT and ECP in terms of their effects on IRI. METHODS: An ischemia-reperfusion model was used to compare the CIT and ECP groups. This model includes a 6-hour ischemia period followed by a

Bilateral nervus intermedius neuralgia with neurovascular contact: a case report highlighting hypertension and eczema as potential precipitating factors.

BACKGROUND: Nervus intermedius neuralgia (NIN) is a rare craniofacial pain syndrome. Bilateral involvement is extremely uncommon, and the pathophysiology and precipitating factors of NIN remain poorly understood. CASE PRESENTATION: A 57-year-old female presented with a 3-month history of bilateral paroxysmal stabbing deep ear pain triggered by cold wind and auricular pressure, superimposed on a persistent dull ache. She had a 5-year history of hypertension, with poor blood pressure control (140-160/90-105 mmHg) for four months preceding symptom onset. Otoscopic examination revealed bilateral

Bacterial composition and healing tendency with cold atmospheric plasma treatment in diabetic wounds.

Hard-to-heal (chronic) wounds in patients with diabetes represent a considerable, difficult-to-manage burden, and significantly increase the risk of amputations of lower extremities. These lesions are often accompanied by infections. Novel therapeutic approaches are urgently needed in the field of wound management. Cold atmospheric plasma (CAP) is a promising tool in the management of hard-to-heal wounds. Its beneficial use can be attributed to the reduction of bacterial load and direct stimulation of tissue regeneration. The effectiveness of different CAP devices for the treatment of hard-to-

Measuring the impact of climate and air quality on COVID-19 transmission dynamics: a tale of two cities in pandemic preparedness.

This comparative study investigates the non-linear influence of nine meteorological and air quality factors on COVID-19 transmission in Daegu and Seoul, South Korea, from January to July 2020. Employing Generalized Additive Models (GAMs), percentile-based risk zone identification, and K-means clustering, our analysis demonstrated that environmental factors are highly region-specific, challenging universal public health approaches. In Daegu, the risk profile was strongly shaped by climatic factors (GAM Explained Deviance: 63.0%), exhibiting a U-shaped temperature effect with elevated risk at bo

Trichoderma-Enabled Crop Resilience Under Abiotic Stress: From Field Delivery to Systems-Level Stress Reprogramming.

Abiotic stresses increasingly threaten crop productivity, whereas reliance on chemical and resource-intensive interventions can compromise environmental sustainability. Existing literature identifies Trichoderma spp. as multifunctional biocontrol agents, biofertilizers, and microbial biostimulants capable of influencing plant growth, stress signaling, and rhizosphere processes; however, evidence remains fragmented across strains, crops, formulations, and stress conditions. This review aimed to integrate current knowledge on Trichoderma-mediated resilience to salinity, drought, heavy metals, te

A TIGIT nanotrapping-guided STING-activatable immunometabolic strategy overcomes innate immune silence and T cell exhaustion in breast cancer.

Breast cancer exhibits a profoundly immunosuppressive tumor microenvironment (TME), where innate immune silence prevents antigen sensing and persistent T cell exhaustion limits effector responses, rendering most immunotherapies ineffective. Clinical profiling of 1093 The Cancer Genome Atlas (TCGA) cases identified a glucose-fueled glutathione (GSH)-glutathione peroxidase 4 (GPX4)-dihydrolipoamide S-acetyltransferase (DLAT) axis as a dominant metabolic shield that suppresses oxidative stress, and thereby enforces both stimulator of interferon genes (STING) silence and CD8+ T cell exclusion. Case Report: A case of diffuse midline glioma, H3 K27-altered presenting with long-segment spinal cord lesions.

Diffuse midline glioma H3 K27-altered is a subtype of pediatric-type diffuse high-grade gliomas, characterized as an infiltrative high-grade glioma involving midline structures with H3 K27 mutation. This clinically rare entity poses diagnostic challenges in the early stages and carries an extremely poor prognosis. We report a case of diffuse midline glioma H3 K27-altered, primarily manifesting as spinal cord lesions, and detail its clinical characteristics to enhance understanding of this disease entity. A 21-year-old male presented with "progressive quadriparesis over 7 months." Seven months

Topical Sirolimus for Refractory Cutis Marmorata Telangiectatica Congenita in Adulthood: A Case Report.

Cutis marmorata telangiectatica congenita (CMTC) is a rare congenital vascular disorder characterized by persistent, violaceous, reticulated skin changes that may be complicated by painful ulcerations. Although localized disease often improves with age, generalized CMTC can persist into adulthood and may be associated with limb asymmetry, ocular abnormalities, and neurologic sequelae. Diagnosis can be challenging in atypical adult presentations despite established major and minor criteria. We report a case of a 50-year-old woman with congenital livedo reticularis and Raynaud syndrome who prese

Thermal touch and anatomical insight: Roman terracotta vessels as proto-clinical maps of the human body.

In the Archaeological Museum of Nicosia, Cyprus, a collection of Roman terracotta vessels-dated to the 1st century BCE to 1st century CE-offers a compelling view into the convergence of empirical healing and early anatomical reasoning. These vessels, likely used for localized heat therapy, are distinctly shaped to contour specific body parts: ears, hands, feet, knees, abdomen, and thorax. With spouts for pouring warm liquids and cavities for radiating retained heat, they appear to have functioned as reusable "hot bags" for therapeutic application. Their deliberate anatomical shapes hint at a p

Stress and Thyroid Function-From Bench to Bedside.

Experimental animal and human studies illustrate the effect of various stress forms on the thyroid gland and the regulation of thyroid hormones (THs) through the thyrotropic multiloop control circuit. The hypothalamic-pituitary-thyroid axis (HPT) axis is part of the physiological stress system and mediates key regulators of metabolic activity during stress response. Genetically characterized individuals are more affected in their response to stressors, and their psychological response is extremely amplified. This leads to significant increases in TH serum levels as soon as a negative stressor

Bedside guillotine foot and ankle amputation in the emergency department due to necrotizing fasciitis.

A 54-year-old male with a history of poorly controlled diabetes mellitus presents to the emergency department after being found unresponsive at home. His last known well was approximately nine hours prior to arrival. He was intubated in the field for airway protection and transported emergently. His initial blood glucose was >600 mg/dL. On arrival, he was hypotensive (BP 67/56 mmHg) and hypothermic (35.7 °C). Examination revealed an unresponsive male with an endotracheal tube in place and a cold, pulseless left foot with necrotic wounds and palpable subcutaneous emphysema. He was immediately i

Multiple Hereditary Exostoses instigating a popliteal pseudoaneurysm in a young Middle Eastern male: A case report and literature review.

INTRODUCTION AND IMPORTANCE: Multiple Hereditary Exostoses is a rare autosomal dominant bone disorder that predominantly affects males at an incidence of (1:50,000 to 1:100,000) in Western populations. The etiology is owed to mutations in the EXT gene group, specifically EXT1 and EXT2 which cause the formation of Osteochondromas. Diagnosis is typically established in childhood. Nevertheless, vascular complications are extremely rare while being potentially fatal. Therefore, timely diagnosis and treatment are vital for such patients. CASE PRESENTATION: We present the case of a 37-year-old Midd
